Program expenses were incurred in the overall management of SHG and its affiliates. Strategic planning, development and the coordination of services involving multiple affiliates and the development of physician manpower in the medically underserved area of Southeastern Massachusetts were the primary services provided and expenses consisted primarily of salaries and benefits of SHG which were transferred to SHS. During FY15, SHS invested in a Behavioral Health Facility.

PART I, LINE 7, COLUMN F In March 2015, the Southcoast acquired a 25% interest in Northeast Behavioral, LLC, which owns a newly constructed 120-bed psychology hospital doing business as Southcoast Behavioral Health (SBH). Southcoast Health System's interest in SBH is recorded as under the equity method of accounting in Southcoast Health System's consolidated financial statements. Amounts reported in Part I, Line 7 represent Southcoast Health System's allocable share of SBH's total community benefit expenses and direct offsetting revenue. The denominator used to calculate the community benefit percent of total expense in Part I, Line 7, column (f), represents Southcoast Health System's allocable share of SBH's total expense (excluding bad debt expense, plus Southcoast Health System's total functional expense as reported in Part IX. Amounts reported in Part III represent Southcoast Health System's allocable share of SBH's Medicare and bad debt amounts. PART III, LINE 2 Bad debt expense is a combination of reserves based on the age of the account, and actual write-offs of balances determined uncollectible in accordance with the Business Office Policies of SBH. PART III, LINE 4 The Southcoast Health System consolidated financial statements do not include a statement describing how SBH accounts for bad debt. SBH accounts for bad debt as follows: ALLOWANCE FOR DOUBTFUL ACCOUNTS is reserved for as follows: 35% of all Medicare deducitbles/coinsurance, 33% of 0-30 day self-pay, 67% of 31-60 day self-pay, and 100% thereafter; 2% of 0-30 day insurance, 5% of 31-90 day insurance, 10% of 91-120 day insurance, 40% of 121-150 day insurance, 70% of 151-180 day insurance, and 100% of >180 day insurance. Additionally, 65% of medicare bad debt write offs are recoverable on the Medicare cost report and as such, they are recorded as an offset to bad debt expense.
PART III, LINE 8 THE COSTING METHODOLOGY USED TO DETERMINE ALLOWABLE COSTS IS BASED ON THE ACTUAL COST OF SERVICES PROVIDED BASED ON THE TOTAL COST-TO-CHARGE RATIO DERIVED FROM SBH'S MEDICARE COST REPORT. PART III, LINE 9B In the event that a patient qualifies for financial assistance but fails to timely pay any remaining balance due (including, if applicable, per the terms of the agreed-upon payment plan), Southcoast may take any of the actions set forth in the Hospitals Billing and Collection Policy, including sending remaining self-pay balances to a third party collection agency within 90-120 days of patients discharge date. Consistent with the Billing and Collection Policy, the Hospital will not undertake any extraordinary collection actions (as defined in that Policy) without first making reasonable efforts to determine a patients eligibility for financial assistance pursuant to this policy.

PART VI, LINE 3 The hospital employed financial counselor meets with every patient that is identified that may have a financial obligation, and reviews the policy and offers financial assistance to all patients to determine if they qualify for charity.
PART VI, LINE 4 SOUTHCOAST HEALTH SYSTEM SERVES A DIVERSE REGION ALONG THE SOUTH COAST OF MASSACHUSETTS, INCLUDING SEVERAL URBAN CENTERS WITH A NUMBER OF DEMOGRAPHIC CHALLENGES, AND RURAL AND SUBURBAN TOWNS. ALTHOUGH THE OVERALL POPULATION HAS GROWN, ACCORDING TO DATA FROM SOUTHCOASTS 2016 NEEDS ASSESSMENT, MOST OF THIS GROWTH HAS OCCURRED IN THE REGION'S TOWNS WHILE THE URBAN POPULATION IN THE CITIES OF FALL RIVER AND NEW BEDFORD IS SHRINKING. PERHAPS THE MOST SIGNIFICANT DEMOGRAPHIC TREND IN THE REGION IS ITS CHANGING RACIAL MAKEUP. THIS IS PARTICULARLY TRUE IN THE CITY OF NEW BEDFORD, WHERE HISPANIC OR LATINO RESIDENTS ACCOUNT FOR 17.5 PERCENT OF ALL RESIDENTS, MUCH HIGHER THAN THE STATE AVERAGE. SINCE 2009, COMMUNITIES THROUGHOUT THE SOUTHCOAST HAVE BECOME MORE DIVERSE. FROM 2009 TO 2014, THE MINORITY POPULATION IN THE REGION INCREASED BY 22.4 PERCENT (10,797 PEOPLE). WAREHAM SAW THE LARGEST PERCENT INCREASE IN ITS MINORITY POPULATION (49.3% OR 1,011 PEOPLE), AND NEW BEDFORD SAW THE LARGEST NOMINAL INCREASE (4,678 PEOPLE OR 17.5%).ADDITIONALLY, AFRICAN-AMERICAN RESIDENTS COMPROMISE A LARGER SHARE OF NEW BEDFORDS POPULATION THAN ELSEWHERE IN THE SOUTHCOAST. INDEED, NON-WHITE RESIDENTS ACCOUNT FOR ONE-THIRD (33.0%) OF NEW BEDFORDS TOTAL POPULATION. AS MAJOR DESTINATIONS FOR THE REGIONS NEWLY ARRIVED IMMIGRANTS, NEW BEDFORD AND FALL RIVER HAVE THE HIGHEST SHARES OF RESIDENTS REPORTING LIMITED PROFICIENCY, 13.7 AND 17.3 PERCENT, RESPECTIVELY. BOTH CITIES ALSO HAVE POCKETS OF UNDOCUMENTED IMMIGRANTS, INCLUDING SOUTH AND CENTRAL AMERICANS IN NEW BEDFORD, ALTHOUGH ESTIMATES VARY WIDELY AS TO THE SIZE OF THESE GROUPS. PORTUGUESE SPEAKERS ARE BEING SUPPLANTED BY SPANISH SPEAKERS IN THE REGIONS CITIES, ALTHOUGH SOME PORTUGUESE-ONLY SPEAKERS REMAIN, PARTICULARLY IN FALL RIVER AND NEW BEDFORD BUT ALSO IN SOME OF THE REGIONS SUBURBS SUCH AS ACUSHNET, DARTMOUTH, FAIRHAVEN, AND WESTPORT. THE AGE COHORTS IN THE SOUTHCOAST GENERALLY REFLECT THEIR COUNTERPARTS AT THE STATE LEVEL. HOWEVER, THE REGION HAS A SLIGHTLY HIGHER SHARE OF RESIDENTS 65 YEARS OF AGE OR OLDER, WHICH IS MORE PRONOUNCED IN THE REGIONS TOWNS. FALL RIVER AND NEW BEDFORD HAVE LARGER SHARES OF THE POPULATION UNDER THE AGE OF 35 WHEN COMPARED TO THEIR METRO AREAS AND THE SOUTHCOAST OVERALL. SOUTHCOAST HAS LONG STRUGGLED WITH LOW LEVELS OF EDUCATIONAL ATTAINMENT. WITHIN THE REGION, FALL RIVER AND NEW BEDFORD HAVE PARTICULARLY LOW LEVELS OF EDUCATIONAL ATTAINMENT. IN BOTH CITIES, LESS THAN HALF OF THE POPULATION 25 YEARS OF AGE OR OLDER HAS EVER ATTENDED COLLEGE. ADDITIONALLY, WHEN COMPARED TO THE ADULT POPULATION STATEWIDE, FALL RIVER AND NEW BEDFORD HAVE MORE THAN TWICE THE PERCENTAGE OF ADULTS WHO HAVE NOT COMPLETED HIGH SCHOOL 29.7 PERCENT IN FALL RIVER AND 28.9 PERCENT IN NEW BEDFORD, COMPARED TO 10.5 PERCENT ACROSS THE COMMONWEALTH. MANY SOUTHCOAST COMMUNITIES HAVE SEEN MEDIAN HOUSEHOLD INCOMES EITHER STAGNATE OR DECLINE FROM 2009 TO 2014. EXAMINING THE 2014 MEDIAN INCOMES FOR SOUTHCOAST COMMUNITIES REVEALS A TREND SIMILAR TO THE PER CAPITA INCOMES THE MAJORITY OF THE REGIONS TOWNS HAVE MEDIAN HOUSEHOLD INCOMES ABOVE THE STATE AVERAGE. ONLY FOUR COMMUNITIES (FALL RIVER, NEW BEDFORD, FAIRHAVEN, AND WAREHAM) HAVE MEDIAN HOUSEHOLD INCOMES BELOW THE STATE MEDIAN. IN PARTICULAR, FALL RIVERS MEDIAN HOUSEHOLD INCOME IS LESS THAN HALF THAT OF THE COMMONWEALTH AS A WHOLE. THE SOUTHCOAST IN GENERAL HAS HIGHER POVERTY RATES FOR ALL FAMILIES, FOR FAMILIES LED BY A FEMALE WITH NO SPOUSE PRESENT, AND FOR FAMILIES WITH CHILDREN. IN FALL RIVER AND NEW BEDFORD, THESE MEASURES OF POVERTY ARE HIGHER THAN THE REGION. FOR EXAMPLE, LESS THAN 10 PERCENT OF ALL FAMILIES STATEWIDE LIVE IN POVERTY (8.3%), COMPARED WITH 12.6 PERCENT OF ALL FAMILIES IN THE SOUTHCOAST AND APPROXIMATELY 20 PERCENT OF ALL FAMILIES IN FALL RIVER (19.6%) AND NEW BEDFORD (20.6%). IN NEW BEDFORD IN PARTICULAR, THE SHARE OF FEMALE-LED FAMILIES LIVING IN POVERTY IS NEARLY DOUBLE THE RATE STATEWIDE 41.0 PERCENT COMPARED WITH 25.9 PERCENT. HIGH RATES OF POVERTY AMONG FEMALE-LED FAMILIES HAVE SERIOUS IMPLICATIONS FOR THE HEALTH OUTCOMES OF THESE WOMEN AND THEIR CHILDREN, SINCE THERE IS NO SPOUSE TO SHARE HEALTHCARE RESPONSIBILITIES AND COSTS. THE REGIONS CHANGING RACIAL MAKEUP AND CONTINUED SOCIOECONOMIC STRUGGLES PLACE UNIQUE STRESSES ON HEALTHCARE DELIVERY, PARTICULARLY IN ADDRESSING HEALTH DISPARITIES BASED ON RACE, INCOME, AND EDUCATION. THE REGION'S HEALTH IS AFFECTED BY THE PHYSICAL CONDITIONS OF THE SOUTH COAST. THIS INCLUDES ACCESS TO AMENITIES THAT FACILITATE HEALTHY EATING AND ACTIVE LIVING, AIR QUALITY, AND SOURCES OF ENVIRONMENTAL CONTAMINATION. THE ANALYSIS SHOWS THAT SOUTHCOAST RESIDENTS FACE ENVIRONMENTAL FACTORS THAT MAY BE A DETRIMENT TO THEIR HEALTH. FOR EXAMPLE: - BRISTOL COUNTY, WHICH COMPRISES MOST OF THE SOUTHCOAST SERVICES AREA, HAS THE SECOND LOWEST FOOD ENVIRONMENT INDEX IN THE STATE, INDICATING SIGNIFICANT BARRIERS TO HEALTHY FOOD ACCESS. - SOUTHCOAST RESIDENTS ALSO HAVE LESS ACCESS TO RECREATION AND FITNESS FACILITIES THAN MASSACHUSETTS RESIDENTS AS A WHOLE. IN BRISTOL COUNTY THERE ARE 13.7 SUCH FACILITIES PER 100,000 RESIDENTS VERSUS 15.9 STATEWIDE. PLYMOUTH COUNTY RESIDENTS HAVE COMPARATIVELY BETTER ACCESS WITH 18.0 FACILITIES FOR EVERY 100,000 RESIDENTS. - THE SOUTHCOAST IS HOME TO A NUMBER OF SITES THAT CONTAIN AND/OR GENERATE CONTAMINANTS THAT CAN NEGATIVELY AFFECT RESIDENTS HEALTH. FALL RIVER IS HOME TO 427 CONTAMINATED SITES, 71 OF WHICH PRESENT SUFFICIENT HAZARDS AS TO LIMIT ACTIVITY ON AND USE OF THESE PARCELS; IN NEW BEDFORD, 78 OUT OF 583 BROWNFIELD SITES ARE SO LIMITED.
PART VI, LINE 5 THE OPERATING AGREEMENT OF SBH STATES: It is the intention of the parties hereto that the Company through the Behavioral Business furthers the charitable and community-based health care purposes of the Southcoast Member and Southcoast by promoting health for a broad cross-section of the community. In furtherance of these purposes, the Southcoast Member may from time to time formulate and implement (and/or directly assist the Operations Manager and/or any and all managers and officers of the Company in the implementation of), policies and procedures for operation of the Behavioral Business and the Company shall adopt and implement such policies and procedures established by the Southcoast Member to promote the public health needs of the community, including policies and procedures regarding admission to the Hospital, financial assistance eligibility criteria, indigent and charity care rendered through the Behavioral Business, performance of community health needs assessments, community education activities and other community health care programs made accessible through the Hospital to the public.
PART VI, LINE 6 SOUTHCOAST HEALTH COMMUNITY BENEFITS PROGRAM COORDINATES ACTIVITES ACROSS VARIOUS AFFILIATES. SOUTHCOAST HOSPITALS EACH YEAR INVESTS MILLIONS IN DIRECT SERVICES AND PROGRAMS THAT ARE DESIGNED TO ADDRESS PRESSING HEALTH ISSUES IN SOUTHCOASTS REGION AND IMPROVE SOUTHCOASTS COMMUNITIES HEALTH. THE SOUTHCOAST COMMUNITY BENEFITS PROGRAM IS UNDER THE OVERALL DIRECTION OF A COMMUNITY BENEFITS COMMITTEE THAT MEETS SIX TIMES ANNUALLY TO REVIEW AND ADVISE ON ACTIVITIES AND EXPENDITURES RELATED TO COMMUNITY BENEFITS. THIS COMMITTEE REPORTS TO THE SOUTHCOAST HOSPITALS GROUP BOARD OF TRUSTEES AND IS CHAIRED BY A TRUSTEE. THE COMMITTEE INCLUDES SOUTHCOAST LEADERSHIP AND STAFF, ALONG WITH REPRESENTATIVES FROM THE VARIOUS COMMUNITIES SERVED BY SOUTHCOAST HOSPITALS. MANY OF SOUTHCOASTS COMMUNITY MEMBERS HAVE EXPERTISE IN MATTERS CONCERNING THE HEALTH AND WELFARE OF THE COMMUNITY AND ARE ACTIVE MEMBERS OF LOCAL AND REGIONAL COALITIONS. THIS BOARD REPRESENTS THE DIVERSITY OF SOUTHCOASTS REGION, WITH MEMBERS WHO ARE ACTIVE LEADERS IN MINORITY COMMUNITIES INCLUDING THE CAPE VERDEAN, HISPANIC AND PORTUGUESE COMMUNITIES.
