SAINT CLARE'S HOSPITAL OF WESTON, INC. PROVIDES INPATIENT , OUTPATIENT, EMERGENCY ROOM AND ANCILLARY SERVICES REGARDLESS OF AN INDIVIDUAL'S ABILITY TO PAY OR METHOD OF PAYMENT. THE HOSPITAL ALSO PROVIDES EDUCATION PROGRAMS AND SERVICES ON HEALTH CARE ISSUES TO THE COMMUNITY IT SERVES. DURING FISCAL 2016, THE HOSPITAL COUNTED 20,360 TOTAL PATIENT DAYS AND 5,972 TOTAL DISCHARGES AND ITS EMERGENCY DEPARTMENT RECEIVED 14,679 VISITS. (a) The Marathon County LIFE survey was sent to 4000 randomly-selected Marathon County residents. The survey was sent in February 2015 and a report of the findings was later published. The survey asks community members about many aspects of quality of life, access to care, income, the environment, alcohol use, transportation, physical activity and more. (b) In addition to the LIFE survey being mailed to individuals in the general community, an additional 120 surveys were issued to non-profit organizations that directly serve individuals who may have low income or be vulnerable. At the meeting, stakeholders participated in facilitated discussions about the community health data, progress on the previous priority areas and any emerging needs. Using the CHNA completed in June 2013, the hospital developed, adopted, and worked on executing a three-year Implementation Strategy to address priority community health needs. In Fiscal Year 2016 (Tax Year 2015), the hospital implemented the following plan to address the priority needs: 1. Access to Health Care: Increase healthcare utilization by farm families and their employees. * Supported the Rural Health Initiative, which brings resources and prevention services on-site to area farms and agribusinesses. * Facilitated access to financial assistance and health care insurance enrollment. 2. Human Trafficking, Sexual Assault, and Domestic Abuse: Marathon County will be recognized as having a cohesive and comprehensive community-wide strategy to address human trafficking and sexual assault issues. * Supported Sexual Assault Nurse Examiners (SANE) at St. Clare's. SANEs are trained to support patients who have been sexually assaulted or abused, as well as collect and document the related evidence. * Provided training and education in the community through a conference, media stories, and presentations to businesses/organizations. 3. Early Childhood Development: Support children's health and school readiness; decrease food insecurity in elementary aged children; increase the acceptance and relevance of breastfeeding; decrease elective deliveries. * Provided food to children in food insecure homes through Blessings in a Backpack during the school year and in summer. * Introduced a new lactation and breastfeeding program. * Supported the Start Right program, which helps families raise healthy, school-ready children. * Participated in the Early Years Coalition. Using the CHNA completed in June 2016, the hospital developed and adopted FY17-FY19 (tax years 2016-2018) Implementation Strategy. The following summarizes the strategies the hospital will implement during that time period to address the priority needs. Early Childhood Development * Increase breastfeeding rates by pursuing Baby Friendly designation. * Reduce early elective deliveries through community awareness, community education, and provider training. * Reduce unsafe sleeping situations for infants through education and the provision of Pack n' Plays to encourage safe sleep practices. * Decrease food insecurity in elementary aged children by providing food on weekends and during the summer months to students in families with low income. * Support the Marathon County Start Right program in its efforts to ensure all children between the ages of 3-5 will have learned the skills necessary to have a successful start in school. Services include enrolling children in early childhood programs; providing home visits to at-risk families, providing parents with education regarding facilitating their child's development and linking families to resources to support their children's school readiness. Human Trafficking, Sexual Assault and Domestic Violence * Support adolescent and teacher education on dating violence and opportunities for healthy relationships. * Maintain a robust Sexual Assault Nursing Examiner (SANE) Program. The SANE Program at MSCH provides examinations of victims of assault free of charge; gathers forensic evidence for use in prosecution against offenders; provides expert testimony in court; and participates in numerous outreach opportunities providing information about sexual assault and human trafficking including community panels, schools programs, health fairs and business presentations. * Work with community partners to: ensure comprehensive services are available for families that reside in the domestic abuse shelter; ensure the ability of the community to meet the growing needs of non-shelter clients; ensure the ability of the St. Clare's emergency department to provide specific staff trained to respond to assault situations; and hire an additional part-time domestic abuse shelter advocate to help support the growing needs of victims and survivors of abuse and trafficking in Marathon County. * Develop a Human Trafficking Response Team to address the growth of human trafficking in Marathon County. * Host multiple community awareness events to address issues of domestic violence, sexual assault, and human trafficking. Mental Health Services: Access and Availability * Increase awareness, knowledge, skills and commitment of community professionals to support and address mental health concerns (including primary care physicians, law enforcement, school professionals, businesses and other). * To address stressors such as lack of income, inadequate housing and poor social support, St. Clare's will participate in local coalitions, fund initiatives which address root causes, and support community education around root causes. * Destigmatize mental health through (a) the Safe Schools/Healthy Students program, and (b) the use of principles such as strategic disclosure and planned contact. * Reduce young parent's stress and increase their positive mental health through parenting classes, case management and the sharing of resources. * Improve mental health in persons with chronic conditions through several community programs. Substance Abuse * Support the dissemination of information regarding alcohol and substance abuse issues through a weekly email distributed by the AOD Partnership of Marathon County. Convene community focus groups, conferences and educational offerings regarding the misuse of alcohol and substance abuse. * Conduct an alcohol and illegal substance community assessment. * Participate in the community's AOD Partnership/Coalition. * Supply local, regional and state policymakers with information regarding alcohol and substance abuse issues, including best practices regarding effective ways to address these issues. This will be done through one-on-meetings, emails, editorials, group panel presentations, forums and listening sessions with local policymakers, and an annual event to highlight developing trends. * Through the county-based Start Right Program, at-risk parents with identified alcohol or substance abuse issues will receive or be referred to supportive services. The description below includes the needs not being addressed as well as the reason why the needs are not being addressed. In addition to the four health issues selected as top priorities, stakeholders identified several other significant health issues in Marathon County. These issues will be addressed by other organizations in the community. While these issues will not be addressed by MSCH as priority health needs, we will provide support to those community efforts whenever possible. Social and Economic Factors that Influence Health: Stakeholders defined this need as addressing conditions in which "people are born, grow, live and work and age" and the broader forces that shape the health of our communities including "economics, social policies, and politics." Components include employment, community safety, income, educational attainment, family and social support, as well as racism and other forms of discrimination. Both the United Way of Marathon County and the Marathon County Department of Health will be addressing this as one of their top priority issues. Although not directly addressing this priority, MSCH plans to support this need through participation with the above organizations as well as with funding when possible. Access to Care: Community stakeholders identified access to care as an improving problem, but one which still needed to be addressed, especially in regards to dental care. Due to the Affordable Care Act, many more people have health insurance. Bridge Community Clinic continues to provide access to both medical and dental care for those who still face a challenge receiving services. Additionally, the Rural Health Initiative helps to provide access to farmers and agribusinesses in rural Western Marathon County. These target populations have been identified as traditionally neglecting formalized healthcare treatment. Oral Health: The stakeholders identified the need to promote healthy teeth and gums with the goal of being free of tooth decay, gum disease, oral cancer and chronic oral pain. This issue will be addressed by the Marathon County Department of Health, as well as the Bridge Community Clinic, a federally-qualified health center. Health Needs of Aging: Stakeholders identified the need to develop and maintain optimal mental, social, and physical well-being and function into older adulthood. This issue will be addressed by the Marathon Cnty Dpt of Health, Marathon Cnty Partnership for Healthy Aging, and additional community partners rather than MSCH. However, representatives of MSCH participate in all of these coalitions. Other benefits are determined based on the direct and indirect costs (where allocated) associated with a program net of earned revenue. Detail for each program/activity is maintained and determined at the department level. Charity care and means-tested programs are determined based on a cost-to-charge ratio consistent with Worksheet 2, Ratio of Patient Care Cost to Charges. Schedule H, Part II Community Building Activities In the area of community building, Saint Clare's Hospital participates in several community coalitions and other collaborative efforts whose purpose is to improve the health of the community. These include UW-Extension and Marathon County Hunger Coalition, Brestfeeding Coalition, United Way Board, Healthcare Advisory Board and Workplace Volunteer Council. By working together with other organizations in the community, we expect to have a greater impact on health as described in the Collective Impact model. In addition, Saint Clare's supports the Blessings in a Backpack feeding program. Food insecurity has been shown to be linked to increased risk of chronic diseases, particularly among low income populations. Saint Clare's also supported the Boots to Work program, providing heavy-duty work boots to adults entering the work force without adequate funds to purchase this required work equipment. According to the County Health Rankings, employment provides income and, often, benefits that can support healthy lifestyle choices. Unemployment and under employment limit these choices, and negatively affect both quality of life and health overall. And Saint Clare's supported the Reading Buddy program. We know that educational achievement is associated with better long term health outcomes and success in education is often measured by reading level in third grade.

Attempts to resolve these outstanding balances can include, but are not limited to statements being sent, phone calls and letters being sent. Account follow up protocol is based on personal pay balance size owed.
Schedule H, Part III, Line 3 Bad Debt Expense Methodology Saint Clare's Hospital of Weston, Inc. has a very robust financial assistance program; therefore, no estimate is made for bad debt attributed to financial assistance eligible patients.
Schedule H, Part III, Line 4 Bad debt expense - financial statement footnote From the consolidated audited financial statements of Ministry Health Care, Inc. (which includes the activity of Saint Clare's Hospital of Weston, Inc.): The provision for doubtful accounts is based upon management's assessment of expected net collections considering historical experience, economic conditions, trends in health care coverage, and other collection indicators. Periodically throughout the year, management assesses the adequacy of the allowance for doubtful accounts based upon historical write-off experience by payor category, including those amounts not covered by insurance. The results of this review are then used to make any modifications to the provision for doubtful accounts to establish an appropriate allowance for doubtful accounts. After satisfaction of amounts due from insurance and reasonable efforts to collect from the patient have been exhausted, the Health Ministry follows established guidelines for placing certain past-due patient balances with collection agencies, subject to the terms of certain restrictions on collection efforts as determined by Ascension Health. Accounts receivable are written off after collection efforts have been followed in accordance with the Health Ministry's policies. The methodology for determining the allowance for doubtful accounts and related write offs on uninsured patient accounts has remained consistent with the prior year.

Schedule H, Part VI, Line 2 Needs assessment The Organization performs community needs assessments as needed. Planning workgroups consisting of community partners are formed to define the community and its needs through review of Health People Initiatives, census documents, and local government health and welfare agencies. Considering other community resources/assets, data collection methods are used to identify particular problems or subgroups and areas known to have high poverty rates and to be underserved. The data is analyzed and priorities are set based on trending information which may target a specific need. When a need is identified, the Hospital develops a plan to implement a program either through community collaboration or individually. Outcomes from the programs are measured and considered in subsequent community health needs assessments. Community benefit plans are presented to senior leadership and board members to ensure the findings are incorporated into the organization's strategy and policy development. For example, senior leaders required to provide direct service to the poor in their communities, and hours are included as part of their performance goals.
Schedule H, Part VI, Line 3 Patient education of eligibility for assistance The organization has financial counselors located at every hospital. If a patient indicates they are unable to pay their bill at any point during pre-service, admission, discharge or billing process, the financial counselors will assist the patient in either securing a funding source (grants, COBRA coverage, Disability benefits, Medicaid, VA etc.) if eligible or assist them in completing a financial assistance application.
Schedule H, Part VI, Line 4 Community information While there is growing agreement in the United States about what constitutes a non-profit hospitals community benefit, this is a work in progress. The organization provided significant charity care and other community benefits as defined by the IRS. But in addition, we believe that we provide a critically important community benefit, which is not quantified. Our hospital, like most rural hospitals, was created and is maintained in order to provide care locally - care that without our hospital, may not be available locally. Certain specialized services available include: emergency services, surgical services, inpatient medical services & obstetrics. Saint Clare's Hospital of Weston, WI and the surrounding areas include Schofield, Rothschild, Rib Mountain & Wausau. The estimated population for Weston, WI as of June 30, 2016 is 15,499. (1)Weston (Town and Village) experienced a substantial amount of growth in population (31.1%) and households (50.1%) over the last 30 years. The largest increases occurred in the decade from 2000 to 2010. (2)The population of the Village is younger, on average than the general population of the County or State, with a median age of 35.6 years compared to 39.4 and 38.5 for the County and State, respectively. (3)The median household income in the Village (2010) is $55,367, which is slightly higher than median income in the County ($53,471) or State ($51,598). (4)The Village is becoming more ethnically diverse. The Asian population in the Village currently consists of 1,124, down from 1,300 in 2008. There is also an emerging Hispanic population, with about 282 people currently living in the Village. In addition, the Village has experienced an overall increase in African-American residents, with a current population of about 1126.
Schedule H, Part VI, Line 5 Promotion of community health The organization strives to further its exempt purposes by the following: (1) The Organization's governing board consists primarily of persons who reside within the primary service area who are not employees of the Organization. (2) As mentioned, the Organization provides several services which are not otherwise available being in an urban setting. (3) Based on the Organization's mission, emergency rooms are made available to all patients regardless of the ability to pay. (4) The Organization also participates in the education and training of healthcare professional through internship programs. Saint Clare's Hospital of Weston, Inc. is also involved in the following efforts to further its tax exempt status. Examples include:- The Hospital participates in government-sponsored health programs (Medicare, Medicaid, Champus, Tricare). - The role of the emergency department and urgent care units in serving all persons regardless of ability to pay. - Availability of financial assistance and sliding scale discounts and changes to financial assistance or collection policies that would be viewed as constructive. - The hospital offers volunteer service program where community members utilize the hospital as a vehicle to connect with and contribute to individuals and the overall community through volunteering.
Schedule H, Part VI, Line 6 Affiliated health care system Ministry Health Care, Inc. (Ministry or the Health Ministry) is a member of Ascension Health. In December 2011, Ascension Health Alliance, doing business as Ascension, became the sole corporate member and parent organization of Ascension Health, a Catholic national health system consisting primarily of nonprofit corporations that own and operate local health care facilities, or Health Ministries, located in 23 states and the District of Columbia. The System uses four categories to identify the resources utilized for the care of persons living in poverty and for community benefit programs: - Traditional charity care includes the cost of services provided to persons who cannot afford health care because of inadequate resources and/or who are uninsured or underinsured. - Unpaid cost of public programs, excluding Medicare, represents the unpaid cost of services provided to persons covered by public programs for persons living in poverty and for other vulnerable persons. - Cost of other programs for persons living in poverty and for other vulnerable persons includes unreimbursed costs of programs intentionally designed to serve the persons living in poverty and the other vulnerable persons of the community, including substance abusers, the homeless, victims of child abuse, and persons with acquired immune deficiency syndrome. - Community benefit consists of the unreimbursed costs of community benefit programs and services for the general community, not solely for the persons living in poverty, including health promotion and education, health clinics and screenings, and medical research. Discounts are provided to all uninsured patients, including those with the means to pay. Discounts provided to those patients who did not qualify for assistance under charity care guidelines are not included in the cost of providing care to persons living in poverty or in the cost of other community benefit programs. The cost of providing care to persons living in poverty and of other community benefit programs is estimated using internal cost data and is estimated by reducing charges forgone by a factor derived from the ratio of total operating expenses to billed charges for patient care. The amount of traditional charity care provided, determined on the basis of cost, was approximately $495,863 and $500,067 for the years ended June 30, 2016 and 2015, respectively. The amount of unpaid cost of public programs, cost of other programs for persons living in poverty and for other vulnerable persons, and cost of community benefit programs are reported in the accompanying supplementary information.
