Schedule J, Part I, Line 1a	CBIA provides travel for companions when there is a valid business purpose for their attendance at out of town meetings. CBIA pays its president and senior vice presidents an amount equivalent to the employer 401k match on earnings in excess of qualified plan compensation limits. These amounts are grossed up for taxes and reported as W-2 income.
Schedule J, Part I, Line 3	The compensation for CBIA's president and senior vice presidents is set by the compensation committee. The members of the committee are CBIA's president, chair of the board of directors, two vice-chairs and immediate past chair. The committee follows a documented compensation philosophy. The committee reviews salary data provided by and independent compensation consultant to establish appropriate compensation and benefit plans for these employees. The committee also administers the variable pay plans for these employees.
Schedule J, Part I, Line 4	CBIA's president and senior vice presidents participate in two non-qualified pension plans. These plans were frozen July 1, 2013. Former CBIA president John Rathgeber received a lump sum benefit from these plans at the time of his termination. The total benefit paid was $1,199,663.
