NANTICOKE MEMORIAL HOSPITAL PROVIDES QUALITY MEDICAL CARE REGARDLESS OF RACE, CREED, SEX, SEXUAL ORIENTATION, NATIONAL ORIGIN, HANDICAP, AGE, RELIGION, DISABILITY, GENETICS OR ABILITY TO PAY. ALTHOUGH REIMBURSEMENT FOR SERVICES RENDERED IS CRITICAL TO THE OPERATION AND STABILITY OF NANTICOKE MEMORIAL HOSPITAL, IT IS RECOGNIZED THAT NOT ALL INDIVIDUALS POSSESS THE ABILITY TO PURCHASE ESSENTIAL MEDICAL SERVICES. THEREFORE, IN KEEPING WITH NANTICOKE MEMORIAL HOSPITAL'S COMMITMENT TO SERVE ALL MEMBERS OF THE COMMUNITY, HEALTH ACTIVITIES AND PROGRAMS NEEDED TO SUPPORT THE INABILITY TO PAY CO-EXIST. THESE ACTIVITIES INCLUDE WELLNESS PROGRAMS, COMMUNITY EDUCATION PROGRAMS, SPECIAL PROGRAMS FOR THE UNDER SERVED, SCREENINGS ASSISTANCE AND A WIDE VARIETY OF BROAD COMMUNITY SUPPORT ACTIVITIES. (SEE SCHEDULE O) NANTICOKE ALSO WORKS WITH CHOOSE DELAWARE, THE STATE HEALTH INSURANCE EXCHANGE PROGRAM, AND PARTICIPATES WITH COMMONWELL FOR PATIENT EMR PORTABILITY. AN ANNUAL REVIEW OF THE IMPLEMENTATION PLAN SHOWED COMPLETION OF THIS WORK AT APPROXIMATELY 60 PERCENT. PART VI, LINE 3: PATIENT EDUCATION OF ELIGIBILITY FOR ASSISTANCE-NANTICOKE MEMORIAL HOSPITAL PROVIDES INFORMATION IN THEIR PATIENT INFORMATION AND VISITORS GUIDE UNDER THE "FINANCIAL ARRANGEMENT/INSURANCE" SECTION CONCERNING THE AVAILABILITY OF FINANCIAL COUNSELORS TO DISCUSS PAYMENT ARRANGEMENTS. AT THIS TIME, PATIENTS ARE INFORMED ABOUT ALL THE PROGRAMS AVAILABLE TO THEM, BOTH VERBALLY AND IN WRITTEN FORMAT OF POSSIBLE ELIGIBILITY FOR DELAWARE MEDICAID ASSISTANCE. PART VI, LINE 4: NANTICOKE HEALTH SERVICES PRIMARILY SERVES WESTERN SUSSEX COUNTY, DELAWARE. THE PRIMARY SERVICE AREA INCLUDES BRIDGEVILLE TO LAUREL AND GEORGETOWN TO SEAFORD, DE. THE SECONDARY SERVICE AREA INCLUDES DELMAR, BRIDGEVILLE, AND GREENWOOD, DE AND FEDERALSBURG, MD AND ITS SURROUNDING AREAS. THIS POPULATION INCLUDES A LARGE NUMBER OF THOSE 55 YEARS OF AGE AND OLDER. THIS AGE GROUP IS GROWING AND NANTICOKE SERVES A LARGE MEDICARE POPULATION. RESIDENTS OF THE AREA GENERALLY HAVE A LOWER MEDIAN HOUSEHOLD INCOME, LOWER LEVEL OF ADVANCED (POST HIGH SCHOOL) EDUCATION, AND A LOWER PERCENTAGE WORKING IN PROFESSIONAL CAREERS. NANTICOKE SERVES A SIGNIFICANT NUMBER OF MEDICAID PATIENTS. MUCH OF THE WORKFORCE IS EMPLOYED BY HEALTHCARE, RETAIL, SERVICE OR AGRICULTURAL BUSINESSES. THE POPULATION IS LARGELY WHITE BUT THE AREA CONTINUES TO EXPERIENCE A RAPIDLY GROWING HISPANIC POPULATION. DISPARITIES REMAIN IN HEALTH EDUCATION, EARLY DETECTION AND MORBIDITY FOR THE AFRICAN AMERICAN AND HISPANIC COMMUNITIES IN WESTERN SUSSEX COUNTY.
PART VI, LINE 5: IN ADDITION TO THE CHNA AND THE ASSOCIATED IMPLMENTATION PLAN, NANTICOKE WORKS IN MANY AREAS OF POPULATION HEALTH MANAGEMENT. AS MENTIONED IN THE NOTES ABOVE (PART VI, LINE 2), NANTICOKE REPRESENTATIVES MONITOR NEED USING PATIENT DATA. THEY DEVELOP COMMUNITY EDUCATION, AWARENESS, OFFER SCREENINGS OR OTHER INITIAITVIES AS IDENTIFIED. NANTICOKE PROVIDES EDUCATIONAL OFFERINGS, SUPPORT GROUPS, AND FREE SCREENINGS TO THE COMMUNITY THROUGHOUT THE YEAR.
