PART I, LINE 3C: OHIO VALLEY HOSPITAL USES FEDERAL POVERTY GUIDELINES AS A BASIS FOR ELIGIBILITY TO QUALIFY FOR FREE AND DISCOUNTED CARE. CURRENTLY THESE RATES ARE 200% FOR FREE CARE 250% AND 300% FOR DISCOUNTED SERVICES USING A SLIDING SCALE.
PART I, LINE 7: AMOUNTS WERE DERIVED FROM OUR MEDICARE COST REPORT, USING OUR MEDICARE COST TO CHARGE RATIO FOR FISCAL YEAR ENDING JUNE 30, 2018. THE EDUCATION DEPARTMENT DIRECT EXPENSES WERE DIVIDED BY TOTAL EDUCATION DEPARTMENT TO CALCULATE THE INDIRECT EXPENSES.THE GENERAL SERVICES COST CENTERS DIRECT EXPENSES WERE DIVIDED BY TOTAL EXPENSES OF THAT COST CENTER TO CALCULATE INDIRECT EXPENSES
PART I, LN 7 COL(F): THE AMOUNT OF BAD DEBT EXPENSE FROM FORM 990, PART IX, LINE 25, COLUMN A THAT WAS REMOVED FOR PURPOSES OF CALCULATING THE PERCENTAGE IN SCHEDULE H, PART I, LINE 7, COLUMN (F) WAS $2,551,403
PART II, COMMUNITY BUILDING ACTIVITIES: THE HOSPITAL PROMOTES THE HEALTH OF THE COMMUNITY BY IDENTIFYING NEEDS, CONSTRUCTING FACILITIES, AND STARTING NEW PROGRAMS. THIS PROGRAM IS FULLY ACCREDITED BY THE PENNSYLVANIA STATE BOARD OF NURSING (PASBON) AND BY THE ACCREDITATION COMMISSION FOR EDUCATION IN NURSING. IT IS ALSO APPROVED BY THE DEPARTMENT OF EDUCATION. WE ALSO HAVE A 24-MONTH SCHOOL OF RADIOGRAPHY PROGRAM. STUDENTS WHO SUCCESSFULLY COMPLETE THE PROGRAM ARE AWARDED A CERTIFICATE IN RADIOLOGIC TECHNOLOGY AND AN ASSOCIATE DEGREE IN HEALTH SCIENCE WITH A CONCENTRATION IN RADIOLOGIC TECHNOLOGY FROM LA ROCHE COLLEGE. THEY ARE THEN ELIGIBLE TO TAKE THE NATIONAL CERTIFICATION EXAMINATION ADMINISTERED BY THE AMERICAN REGISTRY OF RADIOLOGIC TECHNOLOGISTS (ARRT).
PART III, LINE 2: BAD DEBT WAS CALCULATED USING THE MEDICARE COST TO CHARGE RATIOS IN ORDERTO REPORT AT COST.
PART III, LINE 4: PATIENT ACCOUNTS RECEIVABLE ARE REPORTED AT NET REALIZABLE VALUE. ACCOUNTS ARE WRITTEN OFF WHEN THEY ARE DETERMINED TO BE UNCOLLECTIBLE BASED UPON MANAGEMENT'S ASSESSMENT OF INDIVIDUAL ACCOUNTS. IN EVALUATING THE COLLECTABILITY OF PATIENT ACCOUNTS RECEIVABLE, THE CORPORATION ANALYZES ITS PAST HISTORY AND IDENTIFIES TRENDS FOR EACH OF ITS MAJOR PAYOR SOURCES OF REVENUE TO ESTIMATE THE APPROPRIATE ALLOWANCE FOR DOUBTFUL ACCOUNTS AND PROVISION FOR BAD DEBTS. FOR RECEIVABLES ASSOCIATED WITH SERVICES PROVIDED TO PATIENTS WHO HAVE THIRD-PARTY COVERAGE, THE CORPORATION ANALYZES CONTRACTUAL AMOUNTS DUE AND PROVIDES AN ALLOWANCE FOR DOUBTFUL ACCOUNTS AND A PROVISION FOR BAD DEBTS, IF NECESSARY. FOR RECEIVABLES ASSOCIATED WITH SELF-PAY PATIENTS, WHICH INCLUDES BOTH PATIENTS WITHOUT INSURANCE AND INSURED PATIENTS WITH DEDUCTIBLE AND COPAYMENT BALANCES, THE CORPORATION RECORDS A SIGNIFICANT PROVISION FOR BAD DEBTS IN THE PERIOD OF SERVICE ON THE BASIS OF ITS PAST EXPERIENCE, WHICH INDICATES THAT MANY PATIENTS ARE UNABLE OR UNWILLING TO PAY THE PORTION OF THEIR BILL FOR WHICH THEY ARE FINANCIALLY RESPONSIBLE. THE DIFFERENCE BETWEEN THE BILLED RATES AND THE AMOUNTS ACTUALLY COLLECTED AFTER ALL REASONABLE COLLECTION EFFORTS HAVE BEEN EXHAUSTED IS CHARGED OFF AGAINST THE ALLOWANCE FOR DOUBTFUL ACCOUNTS. THE CORPORATION'S ALLOWANCE FOR DOUBTFUL ACCOUNTS FOR SELF-PAY PATIENTS WAS 62% OF SELF-PAY ACCOUNTS RECEIVABLE AT JUNE 30, 2018 AND 55% OF SELF-PAY ACCOUNTS RECEIVABLE AT JUNE 30, 2017. IN ADDITION, THE CORPORATION'S SELF-PAY ACCOUNT WRITE-OFFS (NET OF RECOVERIES) WERE APPROXIMATELY $2,396,000 IN 2018 AND $1,566,000 IN 2017. THE INCREASES WERE DUE TO A SHIFT IN THE PERCENTAGE OF SELF-PAY ACCOUNTS RECEIVABLE AGED GREATER THAN 90 DAYS AND AN INCREASED EXPOSURE TO SELF-PAY BALANCES FROM HIGH DEDUCTIBLE THIRD-PARTY INSURANCE PLANS. THE CORPORATION HAS NOT CHANGED ITS FINANCIAL ASSISTANCE POLICY IN 2018 OR 2017. THE CORPORATION DOES NOT MAINTAIN A MATERIAL ALLOWANCE FOR DOUBTFUL ACCOUNTS FROM THIRD-PARTY PAYORS, NOR DID IT HAVE SIGNIFICANT WRITE-OFFS FROM THIRD-PARTY PAYORS.
PART III, LINE 8: THE AMOUNTS WERE TAKEN DIRECTLY FROM THE MEDICARE COST REPORT. NO SHORTFALL NOTED.
PART III, LINE 9B: ALL PATIENTS PRESENTING WITH NO INSURANCE OR LIMITED COVERAGE ARE PROVIDED WITH THE GUIDELINES FOR THE OHIO VALLEY CARE PROGRAM. PATIENTS CAN QUALIFY FOR FREE CARE BASED ON THE INFORMATION THEY PROVIDE. IF A PATIENT IS PLACED INTO A BAD DEBT STATUS THEY MAY STILL ESTABLISH INDIGENCE AT ANY TIME AND BE COMPATIBLE WITH THE OHIO VALLEY CARE PROGRAM POLICY AND PROCEDURE GUIDELINES. DOCUMENTATION SUPPORTING THE DETERMINATION OF INDIGENCE WILL BE AVAILABLE UPON REQUEST. THE DOCUMENTATION WILL INCLUDE NOTES ON THE PATIENT'S ACCOUNT IN OUR PATIENT ACCOUNTING SYSTEM AND OTHER INFORMATION THAT CAN BE RETRIEVED AS NEEDED TO SUPPORT THE INDIGENCE STATUS OF THE PATIENT OR RESPONSIBLE PARTY.IF AN ACCOUNT QUALIFIES FOR INDIGENCE THE DEBT MAY BE DEEMED UNCOLLECTIBLE WITHOUT APPLYING ANY FURTHER COLLECTION EFFORTS. THERE WILL NOT BE ANY OTHER LEGAL SOURCE OF RESPONSIBILITY FOR PAYMENT.
PART VI, LINE 2: OHIO VALLEY HOSPITAL PARTNERED WITH STRATEGY SOLUTIONS, INC TO FACILITATE THE FIRST YEAR OF THEIR COMMUNITY HEALTH NEEDS ASSESSMENT.
