Briefly describe the organization’s mission: UNITED MEMORIAL MEDICAL CENTER HAS BEEN ESTABLISHED AS A GENERAL ACUTE COMMUNITY HOSPITAL, SERVING AN ESTIMATED 138,208 RESIDENTS OF GENESEE COUNTY AND ADJACENT AREAS. ITS PURPOSE IS TO OPERATE INPATIENT ANDOUTPATIENT SERVICE, ORIENTED TOWARDS GENERAL MEDICAL/SURGICAL, ORTHOPEDIC, PEDIATRIC, MATERNITY, EMERGENCY CARE, AND CANCER SERVICES. THE HOSPITAL CONTINUALLY MONITORS THE NEEDS OF THE COMMUNITY AND DEVELOPS SERVICES TO MAXIMIZE BENEFITS TO THE COMMUNITY, CONSISTENT WITH THE FINANCIAL RESOURCES AVAILABLE TO THE HOSPITAL. IN 2018, UMMC SERVED 4,887 INPATIENTS AND 243,187 OUTPATIENTS. THE HOSPITAL ACCEPTS ALL PATIENTS, REGARDLESS OF ABILITY TO PAY. THE HOSPITAL HAS A CHARITY CARE PROGRAM THAT IS DESIGNATED TO ASSIST THOSE PATIENTS WHO DO NOT HAVE THE RESOURCES AVAILABLE TO REIMBURSE THE HOSPITAL FOR MEDICAL SERVICES RENDERED. IN 2018, THE CSP CONNECTED WITH OVER 850 MEMBERS OF THE COMMUNITY EDUCATING THEM ABOUT IMPORTANT LIFE-SAVING DIAGNOSTIC SCREENINGS RESULTING IN 59 CLINICAL BREAST EXAMS, 51 SCREENING MAMMOGRAMS, 9 DIAGNOSTIC MAMMOGRAMS, 13 DIAGNOSTIC BREAST ULTRASOUNDS, 2 BREAST BIOPSIES, 1 BREAST MRI, 28 CERVICAL CANCER SCREENINGS, 31 FIT KITS AND 1 COLONOSCOPIES.UNITED MEMORIAL ENJOYS STRONG PARTNERSHIPS WITH GENESEE COMMUNITY COLLEGE FOR WORKFORCE DEVELOPMENT BY ALLOWING THE CLINICAL TRAINING FOR NURSING AND RESPIRATORY THERAPY STUDENTS TO BE CONDUCTED AT THE HOSPITAL AND PROVIDING STAFF MENTORS. THE ADDITIONAL INFORMATION OBTAINED WAS USED BY THE ORGANIZATION TO DETERMINE WHETHER TO QUALIFY PATIENTS FOR CHARITY CARE IN ACCORDANCE WITH THE ORGANIZATION'S POLICY. PART III, LINE 3: BAD DEBT COSTING METHODOLOGY:THE PROVISION FOR BAD DEBTS IS BASED UPON MANAGEMENT'S ASSESSMENT OF HISTORICAL AND EXPECTED NET COLLECTIONS CONSIDERING HISTORICAL BUSINESS AND ECONOMIC CONDITIONS, TRENDS IN HEALTH CARE COVERAGE, AND OTHER COLLECTION INDICATORS. PERIODICALLY THROUGHOUT THE YEAR, MANAGEMENT ASSESSES THE ADEQUACY OF THE ALLOWANCE FOR DOUBTFUL ACCOUNTS BASED UPON HISTORICAL WRITE-OFF EXPERIENCE BY PAYOR CATEGORY. ACCOUNTS RECEIVABLE ARE WRITTEN OFF AFTER COLLECTION EFFORTS HAVE BEEN FOLLOWED IN ACCORDANCE WITH THE HOSPITAL'S POLICIES.BAD DEBT EXPENSE IS RECORDED USING THE VALUATION METHOD AS OUTLINED IN HEALTHCARE FINANCIAL MANAGEMENT ASSOCIATION (HFMA) STATEMENT 15, WHICH REQUIRES BAD DEBT EXPENSE TO BE RECORDED AT THE AMOUNT THAT THE PAYER IS EXPECTED TO PAY. MEDICARE SHORTFALLS, WHICH ARE COSTS INCURRED BY THE HOSPITAL TO PROVIDE QUALITY CARE AND TREATMENT TO ITS PATIENTS, SHOULD BE TREATED AS A COMMUNITY BENEFIT. TO NOT INCUR THESE COSTS WOULD POTENTIALLY LIMIT OR EVEN COMPROMISE THE QUALITY OF SERVICE PROVIDED.
PART III, LINE 9B: AT SUCH TIME THAT A PATIENT EXPRESSES A FINANCIAL CONCERN, THE PATIENT WILL BE OFFERED THE OPPORTUNITY TO APPLY FOR CHARITY CARE. ONCE THE PATIENT SUBMITS THE COMPLETED CHARITY CARE APPLICATION, THE ACCOUNT IS PLACED ON HOLD AND ALL COLLECTION ACTIVITIES ARE SUSPENDED UNTIL AN ELIGIBILITY DETERMINATION IS MADE. IF THE PATIENT IS ELIGIBLE FOR CHARITY CARE, THEN THE PATIENT IS NOTIFIED OF THE LEVEL OF CHARITY CARE AWARDED. IF 100% CHARITY CARE IS AWARDED, THEN NO BILL IS SENT TO THE PATIENT. IF LESS THAN 100% CHARITY CARE IS AWARDED, THEN THE PATIENT WILL RECEIVE A BILL PURSUANT TO THE PRIVATE PAY COLLECTION POLICY. ONLY AFTER PATIENT'S LIABILITY HAS BEEN DETERMINED FOLLOWING PROCESSING OF APPLICATIONS FOR GOVERNMENT ASSISTANCE, CHARITY CARE, AND/OR INSURANCE CARRIER REMITTANCE WILL THE PATIENT STATEMENT BE MAILED FOR PAYMENT RECOVERY. PART VI, LINE 2: DEVELOPED BY A DIVERSE GROUP OF STAKEHOLDERS ACROSS A TRI-COUNTY REGION, THE 2016-2018 COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) WAS CONDUCTED IN PARTNERSHIP BETWEEN UNITED MEMORIAL MEDICAL CENTER IN COLLABORATION WITH GENESEE COUNTY HEALTH DEPARTMENT, ORLEANS COUNTY HEALTH DEPARTMENT, ORLEANS COMMUNITY HEALTH (INCLUDING MEDINA MEMORIAL HOSPITAL), WYOMING COUNTY HEALTH DEPARTMENT, AND WYOMING COUNTY COMMUNITY HOSPITAL SYSTEM. THE PROCESS FOR IDENTIFYING AND PRIORITIZING COMMUNITY HEALTH NEEDS AND SERVICES INCLUDED ANALYZING EXISTING COUNTY AND STATE DATA IN CONJUNCTION WITH LOCAL COMMUNITY FEEDBACK FROM OVER 1,700 RESPONDENTS WHO COMPLETED A TRI-COUNTY COMMUNITY HEALTH ASSESSMENT SURVEY AND FROM THOSE WHO PARTICIPATED IN THE 19 COMMUNITY CONVERSATIONS ACROSS THE REGION. UNITED MEMORIAL MEDICAL CENTER IS CONTINUALLY WORKING TO IMPROVE ACCESS TO CARE IN OUR COMMUNITIES. IN 2016, ROCHESTER REGIONAL HEALTH PURCHASED ATTICA FAMILY MEDICINE EXPANDING ACCESS INTO WYOMING COUNTY. INCREASE ACCESS TO HIGH QUALITY CHRONIC DISEASE PREVENTIVE CARE AND MANAGEMENT IN BOTH CLINICAL AND COMMUNITY SETTINGSPROMOTE MENTAL HEALTH AND PREVENT SUBSTANCE ABUSE 4. PREVENT SUBSTANCE ABUSE AND OTHER MENTAL, EMOTIONAL AND BEHAVIORAL DISORDERSTHE 2016-2018 COMMUNITY HEALTH IMPROVEMENT PLAN OUTLINES THE PRIORITIES IDENTIFIED IN THE CHNA AND PROVIDES INTERVENTIONS TO IMPACT THE HEALTH PRIORITIES AS FOLLOWS: THREE FOCUS AREAS HAVE BEEN OUTLINED IN THE PREVENTION OF CHRONIC DISEASE. IN GENESEE COUNTY HAS CONTINUED WORK TO PROMOTE THE PROGRAM AND INCREASE PARTICIPATION. THE PROGRAM MOVED OUTSIDE OF BATAVIA INTO THE ALEXANDER AND PEMBROKE SCHOOL DISTRICTS IN ORDER TO BRING THE PROGRAM DIRECTLY TO THE COMMUNITY THROUGH PARTNERSHIP WITH THE SCHOOL AND THEIR PEP GRANTS. IN 2018, THE COALITION HAS CONTINUED TO SEE ONLY MILD SUCCESS IN INCREASING PARTICIPATION VIA INCREASED VOLUME OF PROGRAM PROMOTION AND CHANGES IN LOCATIONS. IN ADDITION THE PROGRAM IS WORKING WITH LOCAL SCHOOL DISTRICTS, WITH PLANS IN PLACE TO BRING THE PROGRAM TO PEMBROKE CENTRAL SCHOOL DISTRICT AND ALEXANDER CENTRAL SCHOOL DISTRICT IN 2018. FOCUS AREA 2: REDUCE ILLNESS, DISABILITY AND DEATH RELATED TO TOBACCO USE AND SECONDHAND SMOKE EXPOSURE. UNITED MEMORIAL MEDICAL CENTER WORKS WITH PATIENTS WHO SELF-REPORT TOBACCO USE BY PROVIDING SMOKING CESSATION PACKET INFORMATION TO INCLUDE OPT-TO-QUIT AND/OR WORK WITH REFER-TO-QUIT, REFERRING PATIENTS THAT SMOKE TO THE NEW YORK STATE SMOKER'S QUITLINE. OVER 250 INFORMATION PACKETS REGARDING THE NYS QUITLINE AND SMOKING CESSATION ARE PUT TOGETHER AND DELIVERED TO HOSPITAL DEPARTMENTS AND PROVIDER OFFICES FOR DISTRIBUTION TO PATIENTS WHO USE TOBACCO PRODUCTS. FOCUS AREA 3: INCREASING ACCESS TO HIGH QUALITY PREVENTIVE CARE AND MANAGEMENT. UNITED MEMORIAL MEDICAL CENTER'S CANCER SERVICES PROGRAM (CSP) CONTINUES TO ENROLL PEOPLE IN THE PROGRAM TO ASSIST THEM WITH ACCESS AND COST OF SERVICES (CERVICAL, BREAST, COLORECTAL SCREENING). THE PEER EDUCATION GRANT ALSO ASSISTS WITH EDUCATION AND ASSISTANCE PROVIDED TO INDIVIDUALS WHO ARE UNDERSERVED OR HAVE BARRIERS TO SCREENING. PEERS WILL ASSIST ANYONE WHO REQUESTS HELP IN MEETING SCREENING GUIDELINES THROUGH DIRECT REFERRAL OR RESOURCE SHARING. A DIABETES SELF-MANAGEMENT (DSME) PROGRAM IS OFFERED AT HEALTHY LIVING. IN 2018, CLASSES WERE OFFERED AT VARIOUS TIMES TO ASSIST WITH ACCESS. THERE ARE MORNING, AFTERNOON AND EVENING CLASS SESSIONS OFFERED THROUGHOUT THE YEAR. IN 2018 WE HAD 49 PARTICIPANTS IN DSME CLASSES. 76% OF THESE INDIVIDUALS HAD A REDUCTION IN THEIR A-1C AND 86% OF THOSE PARTICIPANTS ARE HAVING THEIR EYES EXAMINED ANNUALLY. UNITED MEMORIAL MEDICAL CENTER HAS ALSO STARTED THE CENTER FOR DISEASE CONTROL DIABETES PREVENTION PROGRAM. THROUGH EDUCATION AND SUPPORT, THE CDC CURRICULUM FACILITATES INCREASED ACTIVITY AND WEIGHT LOSS FOR THOSE AT RISK FOR TYPE 2 DIABETES. TO ADDRESS ISSUES RELATED TO THE PROMOTION OF MENTAL HEALTH AND PREVENTION OF SUBSTANCE ABUSE UNITED MEMORIAL MEDICAL CENTER REMAINS A PART OF A MULTI-AGENCY TASK FORCE ADDRESSING ACCESS TO CARE IN MENTAL, EMOTIONAL, AND BEHAVIORAL HEALTH. THIS TEAM IS WORKING TO PROMOTE SUCCESSFUL INTERVENTIONS ACROSS COUNTY LINES BY CONVENING WITH A NEWLY FORMED OPIOID PREVENTION TASK FORCE. BROCHURES AND PAMPHLETS, IN BOTH ENGLISH AND SPANISH, INFORMING THE COMMUNITY ARE WIDELY DISTRIBUTED IN THE COMMUNITY AT HEALTH FAIRS, CHURCHES, SCHOOLS AND OTHER PUBLIC LOCATIONS. INFORMATION REGARDING THE AVAILABILITY OF FINANCIAL ASSISTANCE IS ALSO AVAILABLE THROUGH ROCHESTER REGIONAL'S WEBSITE AND PATIENTS WITH A SELF-PAY BALANCE ARE NOTIFIED OF OUR FINANCIAL ASSISTANCE PROGRAM VIA THE PATIENT'S STATEMENT. PART VI, LINE 4: UNITED MEMORIAL IS THE ONLY ACUTE CARE FACILITY IN GENESEE COUNTY AND DEFINES AS ITS SERVICE AREA ALL OF THE COUNTY AND THE NEIGHBORING BORDER COMMUNITIES. THE TRI-COUNTY REGION OF GENESEE, ORLEANS AND WYOMING COUNTIES IS LOCATED MID-WAY BETWEEN TWO MAJOR NEW YORK STATE CITIES: BUFFALO TO THE WEST AND ROCHESTER TO THE EAST. CONSIDERED LARGELY RURAL, GENESEE COUNTY HAD AN ESTIMATED 2017 POPULATION OF 57,511. UNITED MEMORIAL SERVES A TOTAL POPULATION OF APPROXIMATELY 138,208 RESIDENTS IN THE TRI-COUNTY REGION OF GENESEE, ORLEANS AND WYOMING COUNTIES. OF THE ESTIMATED 2017 GENESEE COUNTY POPULATION, 90.4% ARE CONSIDERED WHITE, NON-HISPANICS; 5.2% WERE UNDER THE AGE OF 5; 20.3% WERE UNDER THE AGE OF 18 AND 18.7% WERE AGE 65 AND OLDER. IN 2018, UMMC PROVIDED CARE TO OVER 18,600 PATIENTS WHO VISITED THE EMERGENCY DEPARTMENT, EXPERIENCED MORE THAN 5,200 INPATIENT/OBSERVATION DISCHARGES AND MORE THAN 134,500 OUTPATIENT ENCOUNTERS.
