THE FRIENDS SYSTEM OF CARE SERVED ABOUT 1,000 CHILDREN AND THEIR FAMILIES IN 2018, WITH AN AVERAGE DAILY CENSUS OF 250. IN 2018, COMMUNITY MENTAL HEALTH CONTINUED THE STARTUP OF TWO NEW ASSERTIVE COMMUNITY TREATMENT (ACT) CONTRACTS. ONE FOR THE TRADITIONAL POPULATION IN BROOKLYN, AND ONE FOR A SHELTER ACT TEAM WHICH WILL UTILIZE THE TRADITIONAL ACT MODEL TO SERVE CLIENTS LIVING IN HOMELESS SHELTERS IN MANHATTAN. THE 100 SCHOOLS PROGRAM, CREATED AND FUNDED BY 4 DSRIP PERFORMING PROVIDER SYSTEMS (PPS'S) AND ADMINISTERED THROUGH THE JEWISH BOARD OF FAMILY AND CHILDREN'S SERVICES, BEGAN OPERATION IN 2017. THIS PROGRAM PROVIDES CAPACITY BUILDING SUPPORT AND TECHNICAL ASSISTANCE FOR SCHOOL STAFF IN ORDER TO HELP THEM BETTER PROVIDE TARGETED INTERVENTIONS TO PROMOTE STUDENT BEHAVIORAL HEALTH. SINCE ITS INCEPTION IN 2012, THE HEALTH HOME HAS SUCCESSFULLY IMPLEMENTED AN INNOVATIVE MODEL OF CARE COORDINATION FOR THE COMPLEX CHRONICALLY ILL POPULATION OF ADULTS ON MEDICAID WITH PHYSICAL AND BEHAVIORAL HEALTH CO-MORBIDITIES. THIS MODEL OF INTEGRATED BEHAVIORAL AND PHYSICAL HEALTHCARE, AS WELL AS SOCIAL DETERMINANTS OF HEALTH, HAS THE GOAL OF IMPROVING OVERALL HEALTH OUTCOMES AND REDUCING INPATIENT HOSPITALIZATION RATES AND EMERGENCY DEPARTMENT USAGE. THE HEALTH HOME CARE MANAGEMENT PROGRAM SERVED OVER 1,400 MEMBERS IN 2018 IN THE BOROUGHS OF MANHATTAN, BRONX, BROOKLYN AND QUEENS.

PEDIATRIC PALLIATIVE CARE SERVICES ARE ALSO PROVIDED TO CHILDREN IN BROOKLYN, QUEENS, THE BRONX AND MANHATTAN. IT IS FOCUSED ON COORDINATING AND PROVIDING COMPREHENSIVE HEALTH CARE SERVICES TO CHILDREN WITH LIFE LIMITING OR LIFE THREATENING CONDITIONS.

THE EDUCATION AND DEVELOPMENT TEAM FALLS UNDER THE SUPPORT SERVICES PROGRAM. THE DEVELOPMENT OF THE CLINICAL SUPPORT SERVICES TEAM HAS ALLOWED US TO ENTER INTO CASE RATE ARRANGEMENTS WITH MANAGED CARE PAYERS, WHERE WE INTERNALLY MANAGE THE UTILIZATION OF SERVICES WITHOUT REQUIRING AUTHORIZATION FROM THE PLAN; AND FOR WHICH WE MANAGE THE CARE OF THE PATIENT BEYOND THE CERTIFIED HOME HEALTH EPISODE FOR A TOTAL PERIOD OF 60 DAYS, AS WELL AS A CLINICAL CARE MANAGEMENT INFRASTRUCTURE BUILT TO MANAGE MANAGED CARE PATIENTS IN A 60 DAY CASE RATE THAT INCLUDES BONUS AND PENALTIES FOR HOSPITALIZATIONS AND OTHER HEALTH PLAN SIGNIFICANT MEASURES. ADDITIONALLY, THE CLINICAL SUPPORT SERVICES TEAM ALLOWS US TO BRING ALL OF THE CLINICAL ASSETS ACROSS THE VNSNY BUSINESSES INTO A SERVICE DELIVERY MODEL TO SUPPORT ALTERNATIVE PAYMENT MODELS AND NY STATE DSRIP PROGRAMS.
