Explanation:

The organization is largely staffed by volunteers and a 4 paid employees. The organization underwent some personnel changes at the beginning of 2019. The organization has always sought an extension for filing its Form 990 and has never been late in filing. It was assumed that the extension had been filed, when if fact it wasn't. As soon as that was discovered, the return was prepared. Because of past compliance, the organization respectively asks that any late filing penalty be waived.
