| Form 990, Part III, Line 4a PROGRAM DESCRIPTION | (CONTINUED FROM PART III) Last year, Suncoast Hospice served 7,917 patients over by 1,100 staff members for a total of more than 470,000 patient days. Suncoast Hospice is holistic in its approach, helping to guide patients and families and supporting them in their decision making. Despite growth in stature and size, Suncoast Hospice remains a community organization with its leadership and staff ever mindful of what its community wants in a Hospice provider. Because of its strong community connection, Suncoast Hospice has always had vibrant volunteer programs and activities. Volunteers, now 1,200 strong, bring a rich array of ideas, talent and experience to the organization. Last year alone, volunteers contributed over 196,000 hours of service in support of Suncoast's mission. Suncoast Hospice relies on a solid team concept in its service delivery. Patient and family care teams consist of physicians, nurses, aides, physical therapists, nurse practitioners, chaplains, social workers and volunteers. At the center of each team are the patients and their families who may choose to accept or decline various care options open to them. Patient and family care teams may include physical and occupational therapists and palliative arts specialists and such options as massage, therapeutic touch, pet therapy, reiki, humor therapy, music therapy and more. Patients may choose to avail themselves of the services offered according to their own needs, wants and belief systems. Families often remark that the patient and family care teams fit remarkably well into their homes and lives. Many have said that care is more than just a word to Suncoast Hospice staff and volunteers, noting the willingness to go the extra mile on behalf of patients and families. As the husband of one patient said of Suncoast Hospice - you always find a way to say yes. Care and service may be covered by Medicare, Medicaid, insurance providers or private funds. No one in need of end-of-life care is turned away. |

Specially trained volunteers greet new patients and families, helping to become acclimated and offering companionship and assistance. Last year, approximately 2,980 individuals and their families were served at one of the Suncoast Hospice Care Centers. |
| Form 990, Part III, Line 4c PROGRAM DESCRIPTION | (CONTINUED FROM PART III) Some of the offerings are for continuing education credits, while others are simply to inform and enlighten audiences. Training is also available for potential volunteers as well as volunteers wishing to help out in areas that require lengthier, specialty-targeted courses. Training and guidance is also provided for those who act as caregivers for family members or friends. Suncoast Hospice staff members demonstrate hands-on practices and teach care techniques, which include self-care for caregivers. Such training is open to the community. There is perhaps no more difficult, heartbreaking challenge in life than having a child diagnosed with a serious, life-threatening illness. Family dynamics change - siblings may feel left out as parents devote time and attention to the sick child and extended family members may feel helpless in attempts to be supportive. Programs and services cover the span from newborn to age 21. Youngsters may receive care from stepping stones 4 kids in their own homes or anywhere else that meets their needs. Focus is on bringing comfort to the children and providing much-needed support for their families. Services include medical care and symptom relief, nursing care, home health aides and palliative arts, such as pet therapy, massage therapy, music therapy and more. Families are supported with visits from trained volunteers, family fun nights for the whole family, assistance with difficult decisions and emotional and spiritual counseling. Children's programs provide excellent, special counseling for youngsters and other family members who need help to find healing after the death of a child or other family member. In addition, special perinatal care is available for families with pregnancies that cannot be carried to term or those whose pregnancies end in the tragedy bearing a stillborn child. Every effort is made to help families find their ways toward healing following such incidents, including special compassionate touches such as unbelievably tiny, ultra-soft baby clothes for parents to wrap their little ones in right after birth or for burial. Suncoast Hospice's Speakers network is made up of staff and volunteers who provide presentations regarding all aspects of Hospice and palliative care. Suncoast Hospice speakers are willing presenters for neighborhood associations, civic groups, professional associations, faith communities, nursing homes, chambers of commerce and other audiences. Speakers cover a wide range of topics from the basics of Hospice and palliative care to volunteer opportunities, special events, children's programs, palliative arts and more. In addition, the community is invited to Suncoast Hospice community service centers to hear nationally known or local speakers, attend programs or perhaps make use of the specialty libraries at all centers. |
| Form 990, Part VI, Line 15a COMPENSATION OF TOP MANAGEMENT OFFICIAL | The organization's top management official is the president and CEO of Empath Health, Inc. (EHI), a related organization. The CEO's compensation is determined and paid by EHI. Therefore, this question is answered "no" in accordance with Form 990 instructions. The executive committee of the EHI board meets annually to review and approve the compensation of the president and CEO. The Executive Committee last reviewed CEO compensation during the year ended September 30, 2018, and the process and decisions were contemporaneously documented in the executive committee minutes. The Executive Committee's review of CEO compensation includes information provided in the most recent external compensation review. External compensation reviews are performed according to the process below. Every 3-5 years HUMAN RESOURCES will employ a well-recognized, independent compensation consultant to review the market ranges for the CEO of EHI.
