Addiction Treatment for Adults, Youth and FamiliesThe goal of addiction treatment at Hazelden Betty Ford Foundation is lifelong recovery and an improved quality of life for each patient. The Foundation serves both adults and youth and approaches treatment in a holistic, person-centered way, addressing our patients' addiction as well as co-occurring mental health and medical conditions. Our comprehensive treatment integrates the use of evidence-based clinical therapies, peer support and medicine, and is individualized to meet patients' unique needs as they progress from clinical management of their disease to community-supported self-management. Our interdisciplinary care teams consist of addiction counselors and technicians, psychologists and other mental health professionals, psychiatrists and other physicians, nurses and nurse practitioners, family specialists, spiritual care professionals, nutritionists and wellness specialists. In addition to addiction treatment, available at multiple levels of care, Hazelden Betty Ford Foundation provides assessment and evaluation services, intervention services, clinically supervised sober living options, continuing care services and mental health services. Hazelden Betty Ford Foundation also understands that recovery from addiction involves rebuilding relationships with family members and friends, who need help and support of their own as well. Through our family, parent, sibling and children's programs, loved ones receive education, guidance and support to achieve healthier relationships and lives.In 2018, its sites were located in Center City, Chaska, Maple Grove, Plymouth and St. Paul, Minnesota; Chicago, Illinois; Naples, Florida; New York, New York (two facilities there - one each in the Tribeca and Chelsea neighborhoods); Newton, Massachusetts; Beaverton and Newberg, Oregon; Rancho Mirage, Los Angeles and San Diego, California; and Denver, Colorado. Publication of Educational MaterialsHazelden Publishing is the leading publisher of state-of-the-art resources for preventing, treating and managing addiction and closely related issues. Translating research into practice, we help organizations treat the whole client by offering information, guidance, tools and support to meet their unique needs. With 137 new offerings in 2018, our publications continue to provide inspiration, guidance and encouragement to millions around the world every day. In 2018, Hazelden Publishing generated 59 new products, 17 new distributed products, 14 new services and trainings, four new e-books, six new translation rights, 28 specialty products, one new mobile app, and eight new subscriptions. If not for our mission and commitment, much of this material would not be published and available to consumers because of its limited appeal to mainstream publishers. Hazelden Publishing's materials also reach underserved populations, including many customers in the community treatment, faith-based and corrections markets.In 2018, through its BookAid program, the Hazelden Betty Ford Foundation sent 1,853 packages that reached the lives of many thousands of people through libraries serving programs and institutions in need. BookAid resources were sent to individuals and organizations in the United States and around the world. The value of these products is in excess of $91,000, and they cost roughly $6,985 to ship.

Higher Education and Medical & Professional TrainingHazelden Betty Ford Graduate School of Addiction Studies The Hazelden Betty Ford Graduate School of Addiction Studies educates future leaders in addiction counseling to provide evidence-based, integrated care for substance use and co-occurring disorders. Accredited by the Higher Learning Commission, it offers three degrees: Master of Arts in Addiction Counseling, Master of Arts in Addiction Counseling: Advanced Practice, and Master of Arts in Addiction Studies: Integrated Recovery for Co-Occurring Disorders. The graduate school emphasizes public engagement as a means of serving our constituent communities. In 2018, students provided more than 58,580 hours of clinical services at agencies reaching underserved and economically disadvantaged populations. The school also offers a full scholarship each year to a student who works at Chisago County Health and Human Services, and we provide continuing education to these Chisago County professionals at no cost. Eighty-four students graduated with master's degrees from the school in 2018. Overall, more than 92 percent of graduates who seek careers in the addiction treatment field find employment in their home communities or at regional or national treatment agencies. Faculty and graduate school leaders participate in public service events throughout the United States and make research, educational and scholarly contributions to the field of addiction treatment.Medical & Professional EducationThe Professionals in Residence (PIR) program and the Summer Institute for Medical Students (SIMS) give medical students, residents and health care and legal professionals an in-depth experience with the dynamics of the disease of addiction. Intensive one-week programs blend classes presented by Hazelden Betty Ford Foundation physicians and expert clinicians with time spent interacting with patients or family members and staff on treatment units or family programs. Participants learn about the latest research and evidence-based methods from our multidisciplinary faculty. A rotation on addiction for psychiatry, family medicine and internal medicine residents is available in Minnesota and California. A one-year accredited Addiction Medicine Fellowship is available at the Betty Ford Center in Rancho Mirage, California, and a Course on Addiction and Recovery Education (CARE) is available worldwide. The Minnesota and California sites offer customized programming for larger groups. In 2018, our Medical and Professional Education programs welcomed a total of 1,702 participants.

Community Care and Community Benefit: In meeting the Foundation's mission and underlying charitable purpose, the Foundation invests time, financial resources and energy to help people and communities understand and address addiction as a treatable chronic disease, and spread the word that recovery is possible. "Be of service" is an integral core value which the Foundation demonstrates in a variety of ways. The Foundation provides treatment services to individuals and families who seek and qualify for care but are unable to pay the full cost. The Foundation's patient aid policy provides that the Foundation will annually establish a target amount of patient financial assistance. In addition to its annual provision for charity care, the Foundation provides a variety of other benefits for the public good including:Butler Center for Research: -The Butler Center for Research (BCR) is dedicated to improving recovery from addiction by conducting clinical and institutional research, collaborating with other research centers and communicating scientific findings. BCR's vision that sustained recovery for all who seek help will be achieved through advancements in knowledge and integration of research into practice. -In 2018, the BCR participated in the following activities: -As part of the Foundation's commitment to evidence-based practice, a variety of projects were conducted by the BCR to inform clinical care and academic programming, identify needs, drive data-based decision making, guide strategic planning and contribute to the field's knowledge of addiction treatment. Projects included data analyses, white papers, webinars, podcasts and industry trend reports, among others. -In 2018, the BCR collaborated with researchers from the Mayo Clinic who obtained a government grant to study the use of acamprosate, an FDA-approved medication targeting alcohol craving. Funded by the National Institute on Alcohol Abuse and Alcoholism (NIAAA), this project will work to identify biomarkers that may predict patient response to the use of the medication, with the hope of finding new genes linked to alcoholism treatment response. Detailed planning was conducted in 2018 in preparation for a 2019 launch. -The BCR also collaborated on another NIAAA grant-funded study with researchers from Virginia Commonwealth University. Titled, "Genes, addiction and personality," this study is part of a genome-wide association study to learn more about the relationships between personality, mental health, substance use and genes. Data collection began in 2018 and will continue through 2019. -Research Updates were disseminated to professionals, educators, researchers, students and other stakeholders. These two-page summaries of scientific findings from the field of addiction treatment research are provided at no cost as a community benefit to policymakers, treatment professionals and the public at large. Research Updates are also available on the BCR web page at www.hazeldenbettyford.org/education/bcr/addiction-research. Five new Research Updates were created in 2018, including: Using Telehealth for Addiction Treatment, Grandparents Raising Grandchildren, Substance Use Disorders Among Military Population, Co-occuring Mental Health and Substance Use Disorders and Trauma-Informed Care for Substance Use Counseling.-In addition to the new Research Updates, the BRC published a series of informational summaries to help get consumers and clinicians on the same page. The BCR released two such summaries in 2018: "Motivational Interviewing and "Cognitive Behavioral Therapy." Both papers are available online at www.hazeldenbettyford.org/education/bcr/addiction-research.Community Education and Relations: -The Foundation presented free public talks at our Minnesota Speaker Series held in St. Paul on select Tuesdays; at our Oregon Speaker Series held twice a month in the Portland metro area; at our monthly Second Sunday speaker series held in Center City, Minnesota; and at our Awareness Hour series held on select Saturdays in Rancho Mirage, California.-The Speakers Bureau helped place the organization's many spokespeople at events throughout the country, with 31 different employees delivering 217 presentations in 20 states and 42 cities. -The Foundation also provided education and support at free, open-to-the-public Caring Families groups, facilitated once a month in St. Paul, Minnesota; once a month in Plymouth, Minnesota; and; once a week in Fort Myers, Florida-as well as weekly in an online format at www.TheDailyPledge.org.-The Visitors Program in Center City, Minnesota, scheduled and conducted 75 tours for 399 individuals. At the Betty Ford Center in Rancho Mirage, California, staff scheduled and conducted 60 tours.-Volunteers at facilities in California, Illinois, Florida, Minnesota, New York, and Orgeon donated 13,884 hours, valued at more than $340,000.-In addition, educational scholarships from the Foundation totaling $5,200 were awarded to students at six high schools in the Twin Cities and Center City areas of Minnesota as well as two western Wisconsin high schools.-In Florida, the Foundation is an active member of the Collier Community Drug Response Team, an action-oriented work group that analyzes data and mobilizes support to overdose hotspots. And, for the 24th year, we co-sponsored and hosted our Women's Health Conference, a free, twice-annual community event that brings together medical providers and public health departments in Center City, Minnesota.Children's Programs:-The Foundation's Children's Program provides prevention and education services to children and families with addiction. Programs are located in California, Minnesota and Colorado. No child has ever been turned away because of lack of financial ability to pay, and over 90 percent attend on scholarships. In 2018, 1,188 children and family members participated in the program.-Staff of the Children's Program also participated in the Red Ribbon Campaign at numerous schools in California and Colorado. Early prevention education and recovery awareness events were held in communities and schools, along with training sessions for school administrators, principals, counselors and teachers to provide information, resources and tools regarding the family disease of addiction. The training session, called Tools for the Journey, is also open to therapists, counselors, non-profits and other organizations that work with children.

Web and Social Networks-The corporate website, www.HazeldenBettyFord.org, continued to grow in 2018. "Thought for the Day" remained the most popular feature on the website, with an average of 24,000 visitors each day. "Thought for the Day' provides daily inspirational readings from one of six Hazelden Publishing meditation books.-Over 2 million individuals from across the world visited the website seeking information on a variety of addiction-related topics, including treatment, recovery, education, public advocacy and products in our online bookstore. -The Foundation's social community - an online network at www.TheDailyPledge.org-features discussion boards, chatrooms, blogs and online meetings accessible at no cost to anyone anywhere in the world. TheDailyPledge.org continued to offer a home on the web for people in or seeking recovery-and for family members, as well. More than 1,000 times every month, members took the time to post a personal pledge to remain committed to recovery for another day. In 2018, the community-with 9,200 total registered members-welcomed nearly 20,000 visitors who viewed almost 200,000 pages.-The Foundation's Facebook, Twitter, Instagram, YouTube, LinkedIn and other social media accounts, which have more than 84,000 followers, along with its award-winning mobile apps, offered additional access to recovery resources, providing daily inspiration, information and fellowship.Resource Center:-In 2018, the Foundation's Resource Center received 85,167 calls and more than 5,000 online inquiries from people seeking information, assistance, and products related to substance use and addiction.Hazelden Betty Ford Library-In addition to its core mission of fulfilling the informational needs of the Foundation's staff and students, our Addiction Research Library-accessible at www.hazeldenbettyford.org/education/bcr/addiction-research-library serves the public by working daily with information sharing and advocacy efforts in the regional Minitex, national Docline, and international SALIS (Substance Abuse Librarians and Information Specialists) networks, facilitating on our website the preservation and access of archived versions of the ETOH and CORK addiction-related journal databases, free to all internet users and available nowhere else online, and providing scholarships, via the A.A. Heckman Endowed Fellowship Fund, to help individuals study the addiction-related archives housed on our campus in Center City, Minnesota.

Part II-B, Line 1: The Hazelden Betty Ford Institute for Recovery Advocacy had two main areas of focus in 2018: the need to reform business practices and quality standards in the addiction treatment field; and the ongoing effort to confront the nation's opioid crisis. By informing public audiences and key stakeholders about important issues, the Institute's goal is to improve the formulation of policy at its earliest stage. The Institute also supports each Hazelden Betty Ford Foundation facility, its employees and other interested stakeholders by responding to requests and providing information on relevant federal, state and local legislation. The Institute discussed its policy ideas and recommendations with Members of Congress, the Administration and their staff on July 12, when a group of 11 Hazelden Betty Ford Foundation board and staff members fanned out across the nation's capital and visited almost 30 offices. The Institute also visited 10 Congressional offices on April 23-24, and corresponded with public officials in Washington on several other occasions as well, via phone, email, letters and face-to-face meetings. Two Hazelden Betty Ford Foundation leaders testified before Congressional committees in 2018: - President & CEO Mark Mishek on July 24, before the House Energy & Commerce Committee Subcommittee on Oversight and Investigations, regarding advertising and marketing practices within the addiction treatment industry; and, - Director of Communications & Public Affairs Jeremiah Gardner on May 8, before the House Energy and Commerce Committee Subcommittee on Health, regarding H.R. 3545. The Institute retained the lobbying firm Peck, Madigan & Jones to assist with federal lobbying efforts during the second half of the year and spoke to a number of bills targeting the opioid crisis and several enabling more integrated, comprehensive addiction care-including H.R. 3545, legislation to modernize the law governing the use of addiction treatment medical records. Many of the bills we supported were eventually folded into H.R. 6, the SUPPORT for Patients and Communities Act. In other policy activity, the Institute continued to contribute to the Parity Implementation Coalition, which advocates for robust implementation of the federal "parity" law that expands access to addiction treatment through private health care insurance. It also continued contributions to the Partnership to Amend 42 CFR Part 2, which advocates for aligning conflicting federal laws related to the sharing of medical records to allow appropriate access to patient information that is essential for providing whole-person care. Membership in coalitions such as those and in associations related to the addiction field and the preservation of the services the organization provides-such as the National Association of Addiction Treatment Providers-allows the Institute to join forces with others sharing similar concerns and interests. In 2018, the Institute also supported several public forums that highlighted solutions to the opioid crisis and the underlying, longstanding problem of addiction, as well as the need for robust access to care. At events in Oregon, Minnesota and Florida, it joined forces with Hazelden Betty Ford Foundation colleagues as well as local, state and national experts and policymakers to frame up action plans to address the addiction crisis. These one-day HOPE 2018 events focused on emerging solutions, best practices, innovative thinking and lessons learned in other cities and states, with participants working together to chart a course forward for their communities. In addition, the Institute hosted, sponsored or provided speakers for numerous other events focused on opioids, access to care, other policy areas and/or stigma, including school and community forums as well as national and state conventions. The Institute also distributed its weekly Advocacy Update email to a national list of subscribers, while also producing blogs and audio-video content, providing or securing media interviews to support its advocacy agenda, and generating thousands of social media impressions and unique website visitors. The Institute regularly receives requests for information on addiction, treatment and recovery issues from federal, state and local legislatures and governments, as well as from the media and the public. By serving as a reliable resource to many, the Institute was able to influence policy and public attitudes in diverse ways. The Hazelden Betty Ford Institute for Recovery Advocacy's mission is to provide a leading and trusted voice on emerging topics and public policy issues that dominate the national dialogue on addiction, and to facilitate conversation among those in recovery, those still suffering and society-at-large. It is and will remain committed to smashing stigma, shaping public policy and educating people everywhere about the problems of addiction and the promise of recovery.

Part III, Line 4: Hazelden Betty Ford Foundation maintains a collection of historically significant books and archives on recovery and addiction which can be used for research as well as being on public display.
Part V, Line 4: Earnings from the endowment funds are used for the purposes as specified by donor restrictions, including patient aid, operations, research, student aid, prevention programs and special projects.
Part X, Line 2: In accounting for uncertain tax positions, the Foundation recognizes the tax benefit from an uncertain tax position only if it is more likely than not that the tax position will be sustained on examination by taxing authorities, based on the technical merit of the position. Examples of tax positions include the tax-exempt status of the Foundation and various tax positions related to potential sources of unrelated business taxable income. The Foundation had no uncertain tax positions as of December 31, 2018 and 2017.

Betty Ford Center Part V, Section B, Line 11: Since Betty Ford Center provides substance use treatment and provision of mental health services and is not a general hospital providing primary care or preventative care, the health needs identified through this CHNA are more focused on behavioral health than in most hospitals' CHNAs.This is not to suggest that other health needs in the community do not exist; especially prevalent issues include the following: disability, major disease, obesity, prevention of sexually transmitted diseases/infection and sleep deprivation.As a drug and alcohol rehabilitation facility, Betty Ford Center's mission and purpose is focused around substance abuse treatment, not general physical health. Thus, there is a relative lack of expertise to address needs like arthritis, obesity, and STDs. Other organizations in the region-such as the three hospitals in the community, the federally-qualified health centers (FQHCs), etc.-are much better suited to tackle these issues, and many have strong existing programs designed to address these needs.Instead, Betty Ford Center will focus considerable resources and expertise on the local needs relating to behavioral health; specifically:access to care for low-income and or/uninsured, mental health care and substance abuse treatment.The identified need of access to care will be met through three strategies:-Financial assistance for the uninsured, under-insured, and/or low-income community-Contribute to local efforts to "grow our own" behavioral health workforce locally, thereby increasing the number of providers available -Make Betty Ford Center more physically accessible to people with mobility issues.The identified need of mental health care will be met through four strategies:-Provide Betty Ford Center patients with a multi-disciplinary treatment team that is competent with both substance use and mental health treatment-Hire new staff that will enhance Betty Ford Center's mental health competencies-Contribute to local efforts to "grow our own" behavioral health workforce locally, thereby increasing the number of providers available-Provide education and outreach efforts to the community about the relationship between substance use and mental health.The identified need of substance use treatment will be met through two strategies:-Provide education and outreach regarding substance use-Use the expertise and specialized knowledge at the Betty Ford Center to train professionals in substance use treatment.Dut to the timing of the completion of the current process, the organization has not conducted any current year activites to address these needs. However, during the current year they undertook the following activities with respect to the prior CHNA:-Betty Ford Center opened an outpatient clinic in San Diego in 2016 in order to serve more patients. In 2018, nearly 70% more patients received service compared to 2017.-Collaborated with schools and school districts in the service area to reach more students. Staff of the Children's Program participated in the Red Ribbon Campaign at numerous schools in California.-Early prevention education and recovery awareness events were held in communities and schools, along with training sessions for school administrators, principlas, counselors andteachers to provide information, resources and tools regarding the family disease of addiction. The training session, called Tools for the Journey, is also open to therapists, counselors, non-profits and other organizations that work with children.

Part III, Line 2: The bad debt expense is calculated as 75% of the accounts receivable over 90 days.
Part III, Line 4: The Foundation determines the transaction price based on standard charges for goods and services provided, reduced by contractual adjustments provided to third party payers and discounts and/or implicit price concessions provided to patients in accordance with the Foundation's policy. Generally, patients who are covered by third-party payers are responsible for related deductibles and co-insurance, which vary in amount. The Foundation also provides services to uninsured patients, and generally offers those uninsured patients a discount from standard charges. The Foundation estimates the transaction price for patients with deductibles and co-insurance and from those who are uninsured based on historical experience and current market conditions. The Foundation determines its estimated contractual adjustments and discounts based on contractual agreements. Subsequent changes to the Foundation's estimate of the transaction price are recorded as adjustments to patient service revenue in the period of change. Subsequent changes that are determined to be the result of credit and collection issues not assessed at the date of service are recognized as bad debt expense.
Part III, Line 9b: The Betty Ford Center will not engage in extraordinary collection actions before it makes a reasonable effort to determine whether a patient is eligible for financial assistance under the financial aid policy. Actions that may be taken against a patient for nonpayment include utilizing a collection agency for debts older than 120 days. If a collection agency identifies a patient as meeting the Betty Ford Center's financial assistance eligibility criteria, the patient's account may be considered for patient aid. Collection activity will be suspended on these accounts and the Betty Ford Center will review the financial assistance application. If the entire account is adjusted, the account will be returned to the Betty Ford Center. If a partial adjustment occurs, the patient fails to cooperate with the financial assistance process, or if the patient is not eligible for patient aid, collection activity will resume.

Part VI, Line 4: Betty Ford Center's "community" is defined as the Coachella Valley, where the Betty Ford Center is located. The Coachella Valley is a unique community, geographically isolated by mountain ranges so that it almost functions as an island. Approximately 414,000 people live in this part of Eastern Riverside County in Inland Southern California.The Coachella Valley as the community was determined by the senior leadership of the Betty Ford Center. It includes all residents in a defined geographic area surrounding the hospital and does not exclude low-income or underserved populations. The decision was based on geography and patient origin; approximately 19% of patients in 2017 live in the Coachella Valley (more than 400 patients). Specifically, the three most common hometowns of patients include Rancho Mirage (122 patients), Palm Desert (78 patients), and Indio (60 patients), all of which are within the Coachella Valley.About a quarter of residents (24.4%) are minors and another third (31.4%) can be considered seniors (ages 55 and older). In fact, 5.3% of the Coachella Valley is age 80 and over (more than 21,500 people).The percent of adults that are seniors is significantly higher in the Coachella Valley than in California as a whole. Specifically, 32% of California adults are age 55 and older, while in the Coachella Valley, approximately 42% of adults are age 55 and older.Approximately 11.9% of Coachella Valley adults identify as lesbian, gay, bisexual, or other sexual orientation. This is significantly higher than the rate in the state of California as a whole, where only 6.7% identify as lesbian, gay, bisexual or other sexual orientation. Thus, Betty Ford Center must have strong competencies in LGBT-specific care and be able to provide culturally competent care to this population.The majority of the Coachella Valley speaks either English (54.4%) or Spanish (41.0%) at home, and thus, Betty Ford Center must provide culturally competent care in Spanish if they are to truly meet the needs of their population. Less than 5% of the population speaks a language other than English or Spanish, and thus, those languages are less necessary to meeting the needs of this community.Local children are more likely to be Hispanic than local adults. This illustrates the unique nature of the Coachella Valley, which includes many white retirees who relocated to the area as well as local Hispanic families with children.About 38.9% of local adults (ages 18+) have a high school degree or less. This indicates that more than 60,000 local adults lack a high school degree. Thus, Betty Ford Center must be sure to provide materials to individuals with low literacy levels in order to serve this segment of the population. About a quarter of locals live in a household with an annual income under $20,000.Poverty was calculated using the number of people living in the home and the household income level. More than a quarter of local adults live below the federal poverty level (FPL), as do more than a third of local children.Thus, Betty Ford Center must continue to offer significant financial aid if they are to meet the needs of this substantial low-income community.Financial aid is especially important for uninsured adults, who are unable to bill their insurance for services. Approximately 13.9% of working age adults in the Coachella Valley remain uninsured. Virtually all seniors have health insurance due to Medicare.

Part I, Line 2: Hazelden Betty Ford Foundation provides the assistance as part of its program services activity. All scholarships are provided to individuals and monitored through the Graduate School of Addiction Studies, the Professionals in Residence and Medical Education training programs and the Community Relations department to ensure the monies are used for the purpose intended. The tuition paid for those programs and the funds given to assist with the tuition is tracked by each individual.

Form 990, Part VI, Section A, line 1 The Executive committee of the Board of Trustees has the authority to act on behalf of the Board between the Board's regular meetings, subject to the limitations of power listed below. The Executive committee consists of the Chair of the Board, the immediate past Chair, if still a member of the Board, Vice Chairs of the Board, President (ex-officio non-voting), Secretary, Treasurer, and the Chair of each standing committee. Limitations of Powers: The Executive committee does not have the power to elect a trustee, to alter fundamental policies approved by the Board, to make appropriations which are not in accordance with the general policies approved by the Board, to modify, revoke or renew any contract previously entered into by the Board or to change the person or persons specifically designated by the Board to enter into or execute on behalf of the organization a particular contract, obligation, agreement or writing authorized by action of the Board of Trustees.
Form 990, Part VI, Section B, line 11b Hazelden Betty Ford Foundation sent its Form 990, with Schedule B suppressed, via email to Board members for review and questions prior to filing. It was also reviewed by the CFO prior to filing.
Form 990, Part VI, Section B, line 12c Hazelden Betty Ford Foundation requires an annual disclosure of any potential conflict of interest by Board Members, officers and key employees. If a Board Member discloses a potential conflict, that person then excuses him or herself from voting on that particular issue. All new Board Members are provided conflict of interest education and periodic training throughout their term. All Board Members sign an annual conflict of interest disclosure form.
Form 990, Part VI, Section B, line 15 Hazelden Betty Ford Foundation has an established process for determining the compensation of the President and CEO, other officers and key employees. The Hazelden Betty Ford Foundation by-laws delegate responsibility for determining compensation for the President and CEO, other officers and key employees to the Executive Committee of the Board of Trustees. The Executive Committee develops, reviews, and modifies, as necessary, the compensation philosophy for Hazelden Betty Ford Foundation. Hazelden Betty Ford Foundation has the following processes in place: 1. Each year the Board Members complete a conflict of interest disclosure. The documents are reviewed by Hazelden Betty Ford Foundation's General Counsel and if a Board Member has a conflict of interest they are not involved in any compensation decisions. 2. The Executive Committee engages the use of an outside independent compensation consultant to identify comparable market data for comparable positions for the President and CEO and other disqualified individuals consistent with the approved compensation philosophy. Generally, the market analysis includes competitive data from published survey sources, supplemented with compensation data from custom 990 analyses. Where possible, competitive data are pulled from similarly sized organizations. The compensation consultant analyzes the pay packages of the President and CEO and other disqualified individuals by reviewing base salary, total cash compensation (salary plus annual incentive at both target and actual levels) and total direct compensation (salary plus annual and long-term incentive). In addition, every two to three years the Executive Committee reviews the total compensation package of the President and CEO and other disqualified individuals. The total compensation package includes total direct compensation plus severance, employee benefits, perquisites and any other pay. 3. Comparable market data are reviewed and discussed by the Executive Committee and minutes record those discussions and any resulting compensation decisions. A compensation review for the President and CEO was most recently done in March 2018. The compensation review for other officers and key employees was completed in August 2017.
