Schedule J, Part I, Line 1a Travel for companions is provided to CEO, EVP and Board Members. Health and Social Club membership is provided for to CEO and EVP. Tax indemnification and gross up payments were provided to President/CEO for and Executive Vice President for interest calculated on loan from Credit Union for payment of annual Split-Dollar Life Insurance Premiums. Tax indemnification and gross up payments were also provided to President/CEO on a 457(f) distribution/payment.
