OTHER PROGRAMS INCLUDE THE COMMUNITY ENGAGEMENT ACADEMY, HANDS HELPING HANDS CONFERENCE, DO RIGHT KIDS, A WELLNESS SERIES EMPHASIZING DIABETES, STRESS, NUTRITION AND HEALTHY COOKING, MEN'S HEALTH WORKSHOPS, COMMUNITY HEALTH UNIVERSITY, AND THE FORTHCOMING BLACK WOMEN'S HEALTH MOVEMENT. THE COMMUNITY ENGAGEMENT ACADEMY (CEA) TRAINS RESIDENTS TO BECOME COMMUNITY LEADERS. TRAINING INCLUDES COMMUNITY ORGANIZING, ADVOCACY SKILLS, AND PROJECT SUSTAINABILITY. PARTICIPANTS LEARN TO INFLUENCE, ENGAGE, AND MOBILIZE COMMUNITY MEMBERS ON THEIR BLOCK, THEIR CHURCH OR ORGANIZATION TO ADDRESS IMPORTANT COMMUNITY ISSUES. THEY ALSO LEARN HOW TO DEVELOP ACTION PLANS TO MAKE CHANGE AND WORK WITH OTHERS THROUGH COLLABORATIVE LEADERSHIP. CEA HELD 1 CLASS THIS FISCAL YEAR WITH 27 REGISTRANTS. OVER 230 PERSONS ATTENDED 2 BLACK MEN'S HEALTH WORKSHOPS WHICH QUESTIONED WHY ARE BLACK MEN DYING? OVERALL, ATTENDEES WERE IMPRESSED BY THE EVENT. ALL ATTENDEES INDICATED THEY WOULD RECOMMEND IT TO OTHERS AND WOULD ATTEND SIMILAR EVENTS. OTHER OUTREACH ACTIVITIES INCLUDED FOCUS GROUPS (10 THIS YEAR) NEWSPAPER COLUMNS, ONLINE COMMUNICATIONS, LOCAL AND NATIONAL PRESENTATIONS, COMMUNITY WORKSHOPS, OUR INTERNSHIP PROGRAM (18 INTERNS THIS YEAR), AND PROGRAM SPONSORSHIPS. THE HEALTH GAP ALSO PARTNERED IN 70 COMMUNITY EVENTS TO PROMOTE OUR PROGRAMS AND OTHER IMPORTANT HEALTH ISSUES. WE DISTRIBUTED OVER 25,000 PIECES OF HEALTH LITERATURE AND REACHED 214,667 PEOPLE THROUGH HEALTH AWARENESS CAMPAIGNS, OUTREACH, AND ADVOCACY EFFORTS. THE DO RIGHT WELLNESS SERIES INCLUDES NUTRITION TRAIN THE TRAINER, HEALTHY STEPS, AND RELAX AND RESTORE. THE CURRICULA'S FOCUS IS AROUND THE KEY THEMES OF EAT RIGHT, MOVE RIGHT, AND LIVE RIGHT. OVER 182 PERSONS REGISTERED FOR THE SERIES. NUTRITION TRAIN THE TRAINER (NTT) TEACHES PARTICIPANTS HOW TO CHOOSE AND PREPARE HEALTHIER FOOD CHOICES AND HOW TO EXPAND THE REACH OF NUTRITION EDUCATION BY TRAINING PARTICIPANTS TO TEACH OTHERS. HEALTHY STEPS BUILDS THE CAPACITY AND CONFIDENCE IN PRE-DIABETIC AND DIABETIC INDIVIDUALS IN MANAGING THEIR CONDITIONS IN ORDER TO LEAD HEALTHIER LIVES. DO RIGHT RELAX AND RESTORE (R&R) EXPLORES HOW STRESS EFFECTS THE MIND, BODY, AND SOUL. PARTICIPANTS LEARN AND PRACTICE HOLISTIC SKILLS TO FREE THEMSELVES FROM UNPRODUCTIVE ANXIETIES AND UNHEALTHY BEHAVIORS SO THAT THEY CAN EXPERIENCE PEACE AND JOY IN DAILY LIFE, RELATIONSHIPS, AND THE COMMUNITY. COMMUNITY HEALTH UNIVERSITY EMPOWERS INDIVIDUALS TO NAVIGATE THE HEALTH CARE SYSTEM, IMPROVE THEIR HEALTHCARE EXPERIENCE AND INCREASE PATIENT ENGAGEMENT, RESULTING IN FEWER READMISSIONS. 14 PERSONS ATTENDED THE PILOT PROGRAM IN PARTNERSHIP WITH UC HEALTH. DO RIGHT KIDS IMPROVES THE HEALTH AND WELL-BEING OF DISADVANTAGED, OVERWEIGHT OR OBESE CHILDREN, AND CHILDREN AT RISK FOR OBESITY AND BECOMING OVERWEIGHT. THE PROGRAM PROVIDES A 12-WEEK CURRICULUM EACH SEMESTER THAT EDUCATES CHILDREN AND THEIR FAMILIES ABOUT THE IMPORTANCE OF HEALTHY EATING AND PHYSICAL ACTIVITY. THE INSTRUCTION IS HANDS-ON AND ACTIVITY-BASED. THE PROGRAM HAS THREE COMPONENTS: IN SCHOOL, AFTER SCHOOL, AND FAMILY NIGHTS. WE IMPLEMENTED DO RIGHT KIDS IN TWO CINCINNATI SCHOOLS FOR 128 STUDENTS. EACH SCHOOL HOSTED FAMILY NIGHTS WITH COOKING AND FITNESS DEMONSTRATIONS FOR FAMILY MEMBERS. FITNESS DEMONSTRATIONS INCLUDED ZUMBA, HIP-HOP DANCE WORKOUTS, AND GROUP FITNESS. NEXT YEAR, THE PROGRAM WILL EXPAND TO AREA RECREATION CENTERS. |

O 87 EXHIBIT BOOTHS FEATURING HEALTH INFORMATION, DEMONSTRATIONS, FOOD, AND WELLNESS PRODUCTS. O ENTERTAINMENT FEATURING LOCAL RECORDING ARTISTS TYSHAWN COLQUITT AND NATURAL PROGRESSION. THEY CANVASSED THE NEIGHBORHOOD WITH INFORMATION ABOUT THE PROGRAM, AND PLANNED PERIODIC COMMUNITY EVENTS. THE COACHES RECRUITED 200 PROSPECTIVE WALNUT HILLS HOUSEHOLDS. MEANWHILE MT. AUBURN RESIDENTS REACHED WELL OVER 370 TOUCH POINTS THROUGH ITS MANY COMMUNITY EVENTS. CHOICE NEIGHBORHOODS IMPLEMENTATION GRANT - THE HEALTH GAP COMPLETED THE CONTRACT WITH THE COMMUNITY BUILDERS, INC. ON THE CHOICE NEIGHBORHOODS IMPLEMENTATION GRANT FUNDED BY THE UNITED STATES DEPARTMENT OF HOUSING AND URBAN DEVELOPMENT. THIS 5-YEAR GRANT INCLUDED NUMEROUS PARTNERS AND IMPLEMENTED A TRANSFORMATIVE PLAN FOR COMPREHENSIVE AND SUSTAINABLE REVITALIZATION FOR THE CINCINNATI NEIGHBORHOOD OF AVONDALE. AS A HEALTH PARTNER, WE PROVIDED PROGRAMS TO IMPROVE AVONDALE CHOICE RESIDENTS' HEALTH BEHAVIORS, NUTRITION, AND COMMUNITY ENGAGEMENT SKILLS. HIGHLIGHTS FROM THIS YEAR'S PROGRAM INCLUDE THE FOLLOWING: 1. AVONDALE CHOICE DO RIGHT GET COVERED SESSIONS WERE HELD TO INFORM AND ENROLL RESIDENTS IN HEALTH INSURANCE UNDER THE AFFORDABLE CARE ACT AND CONNECT THEM WITH A PRIMARY CARE PHYSICIAN. 2.
