ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) is a 47-bed hospital campus providing services without regard to patient race, creed, national origin, economic status, or ability to pay. During fiscal year 2018, ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) treated 1,535 adults and children for a total of 3,942 patient days of service. The hospital also provided services for 176,952 outpatient visits, which included 1,465 outpatient surgeries and 17,094 Emergency Room Visits. See Schedule H for a non-exhaustive list of community benefit programs and descriptions.

From September 2015 to April a comprehensive Community Health Needs Assessment (CHNA) was conducted. A internal ministry team made up of ASCENSION ST. JOSEPH'S HOSPITAL staff and key regional partners was established to provide overall guidance and oversight of the CHNA process. Soon after, an external steering committee was created. This group was formed to promote community collaboration, collect, share and interpret data, identify health disparities and determine resources to address those needs. As noted, the external group was made up of persons and organizations representing public health, community mental health, business, education, providers, foundations, community based organizations, and residents. Following is a list of participating organizations: Alcona Health Center AuSable Valley Community Mental Health Catholic Human Services Department of Health and Human Services District Health Department #2 Iosco County Poverty Reduction Coalition Iosco Regional Educational Service Agency Iosco Transit Corporation Oscoda FISH MSU Extension St. Joseph Health System Foundation St. Mary's of Michigan Saginaw Foundation Once preliminary data was collected, it was strategically shared with the external steering committee and network of collaborative partners. Subsequent meetings were held to produce a list of current community programs and resources available, as well as, finalize a needs prioritization exercise. Data was also reviewed by District Health Department #2 Health Officer, Denise Bryan. The assessment process also included a community health needs assessment survey.

ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) has secured a partnership with MiHIA and Good Measures virtual Diabetes Prevention Program (DPP) to offer DPP Iosco County residents in fiscal year 2019. Prioritized Need #2: Health & Lifestyle Education. ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) will leverage the strengths of Ascension Mid-Michigan's current population health programs and services throughout the region, we will develop the capacity (meet people where they are) to offer services and educational programming to increase patient knowledge, and self-management/adoption of healthy behaviors to improve nutrition (access to and consumption of healthy food), weight (optimal weight for youth and adults), and physical activity. St. Joseph has collaborated with Tawas school district to begin a backpack program; FriAscension St. Joseph day Folder program; Fit Kids program; Healthy Kids Rock program. These programs began in early FY18, but have now been placed on hold for reevaluation. Prioritized Need #3: Behavioral Health. ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) will leverage the strengths of Ascension Mid-Michigan's current population health programs and services throughout the region, we will develop the capacity (meet people where they are) to offer therapeutic services and educational programming to increase patient knowledge and adoption of healthy behaviors to improve mental health, and mitigate substance abuse, abuse, and violent behavior. ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) offered education to staff through a mental health first aid program, and provided a de-escalation training for ER staff to promote a safer environment for staff, associates, and patients. Ascension St. Joseph sent over 20 letters to community health centers, physician and dental offices, to obtain broad community support and participation in Michigan's Automated Prescription System (MAPS) to protect providers and reduce prescription drug abuse. Continued work will be done to collaborate with community mental health to expand and enhance limited mental health services via telemedicine utilizing existing telehealth equipment. ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) will not directly address the transportation health need identified within the 2016 CHNA. Additionally, while access is not specifically called out as a strategy, it is embedded in each Implementation Strategy. While critically important to overall community health, the transportation priority did not meet internally determined criteria that prioritized addressing needs by either continuing or expanding current programs, services and initiatives to steward resources and achieve the greatest community impact. Schedule H, Part III, Line 2 Bad debt expense - methodology used to estimate amount After satisfaction of amounts due from insurance and reasonable efforts to collect from the patient have been exhausted, the Corporation follows established guidelines for placing certain past-due patient balances within collection agencies, subject to the terms of certain restrictions on collection efforts as determined by Ascension Health. Accounts receivable are written off after collection efforts have been followed in accordance with the Corporation's policies. After applying the cost-to-charge ratio, the share of the bad debt expense in fiscal year 2018 was $1,053,128 at charges, ($600,283 at cost).
Schedule H, Part III, Line 3 Bad Debt Expense Methodology The provision for doubtful accounts is based upon management's assessment of expected net collections considering historical experience, economic conditions, trends in healthcare coverage, and other collection indicators. Periodically throughout the year, management assesses the adequacy of the allowance for doubtful accounts based upon historical write-off experience by payor category, including those amounts not covered by insurance. The results of this review are then used to make any modifications to the provision for doubtful accounts to establish an appropriate allowance for doubtful accounts.

Considered rural, Iosco County is in the northeast quarter of the lower peninsula of Michigan in the heart of Lake Huron's "Sunrise Side." According to the US Census Bureau, Iosco County has a total area of 1,891 square miles of which 549 square miles is land and 1,342 square miles (70.96%) is water. ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) is located in Tawas City, which serves as the County Seat. Iosco County has an estimated population of 26,255 and is considered to be part of northern Michigan. Approximately 60% of the county is held in public ownership (Huron National Forest and State forest lands). Iosco County trails the state percentage of 26% with only 15% holding a Bachelor's degree or higher. Schedule H, Part VI, Line 5 Promotion of community health Alcoholics Anonymous - Heavy alcohol consumption and substance abuse issues continue to be a problem in the communities we serve, Understanding that a large gap exists between the need for treatment and the availability of services in our rural area ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) maintains its commitment to provide a supportive role and to work collaboratively with community stakeholders whose primary responsibility is to provide prevention and education programs as well as treatment to those affected by alcohol or substance abuse i.e., AuSable Valley Community Mental Health and Catholic Human Services, Michigan Rehabilitation Services, and Northern Michigan Regional Entity. With access a barrier to education and treatment St. Joseph Health System lends support by providing in-kind space to local Alcoholics Anonymous groups. The groups meet Monday, Wednesday and Thursday evenings each week of the year for approximately 1 hour. Alcoholics Anonymous is a fellowship of men and women who help each other maintain sobriety and who offer to share their recovery experience freely with others. Support Groups - ASCENSION ST. JOSEPH'S HOSPITAL (F/K/A ST. JOSEPH HEALTH SYSTEM, INC.) offers several Support Groups with the purpose of addressing physical, social, psychological or emotional issues related to specific diagnoses or life events. Grief Support Group - This group is designed to provide a safe environment to talk about feelings as they relate to loss learn to help oneself discover new ways to cope with these feelings and changes in one's life and identify new sources of strength and hope.
