AGING AND DISABILITY SERVICES:The Dallas Area Agency on Aging (DAAA) was created in 1973 from the Older American Act mandating the designation and local implementation of Area Agencies on Aging. The Community Council has sponsored the DAAA since it was created in 1973. The DAAA provides services to residents of Dallas County age 60 and older focusing on older adults who are low income, have frail health, have physical or mental disabilities, have language barriers or are at risk for institutionalized care. In 2018, DAAA provided the following: - Health Maintenance: Assistance was provided for hearing aids, incontinent supplies (pull ups, wipes, bed pads), and Nutritional drinks to 265 residents. - Income Support: Assistance to 119 individuals in the form of a third-party payment for services that support the basic needs of the older adults. For example: Rent, Mortgage or Utility payment assistance. - Instruction and training provides experience or knowledge to 889 individuals or professionals working with older individuals. - Chore Services: Assistance with heavy duty tasks for 22 residents such as heavy cleaning, moving heavy furniture, maintenance of yard or hoarding clean-up. - Respite Services: Temporary relief for 19 caregivers through services provided either through the Senior Companion program or through a voucher system. - Legal Services provided to 73 individuals.The Benefits Counseling program offers three components: certification training, education and counseling.The Care Coordination and the Caregiver Support Program assess the needs of older individuals to effectively plan, arrange, coordinate and follow-up on services which most appropriately meet the identified needs of the client and/or caregiver. In FY 18 the Care Coordination and Caregiver program aided 812 older adults, including: Personal Assistance to 10 individuals to assist on a limited basis with activities of daily living; Residential Repair assistance to 265 individuals with minor home repairs based on health and safety needs that allowed the client to remain in their home. It also hosted 36 events for Caregiver information services.The Dallas Area Agency on Aging sub contracts for most of its services prescribed by the Older American Act. The Transportation- Demand/Response is a support program that takes an older adult from one location to another but does not include any other activity. In 2018, it provided 79,901 rides to individuals. In FY '18, 215,834 home delivered meals were provided through VNA or JFS. The Home delivered meal program delivers nutritionally balanced meals to home bound elderly who are unable to prepare their own meals. In FY '18 312,904 Congregate meals were delivered to sites, which provide a noon day meal for older adults in Dallas County.The Ombudsman program is contracted with the Senior Source. There are thirty-four Ombudsman volunteers who identify, investigate and attempt to resolve complaints and concerns made by or on behalf of residents of nursing home and assisted living facilities.Evidenced Based ProgramsEvidence-based Programs: To provide intervention to an older individual based upon the principles of evidence-based disease prevention programming.These programs include activities directly related to establishing or expanding the following interventions based on applying principles of scientific reasoning, behavior change theory, and program planning.2,611 clientsChronic Disease Self-Management: Encourages older adults and adults with disabilities to manage ongoing health conditions. 1. Diabetes Self-Management: Encourages older adults to manage diabetes and increase confidence for healthy living. 2. A Matter of Balance: Helps older adults learn practical strategies to reduce their fear of falling and increase their activity. 3. HomeMeds: Providing in home medication assessment, computerized screening and alert process to identify medication problems and review by pharmacist with the objective to help prevent falls, dizziness, confusion, and other medication related problems. 4. Stress Busting Program for Family Caregivers: Focuses on relaxation and stress management techniques to help caregivers learning to cope while caring for a loved one with Alzheimer's disease or related dementia.

COMMUNITY SERVICES BLOCK GRANT:Community Care and Navigation Services provides Direct Assistance for clients transitioning out of poverty and Emergency Services to clients needing immediate assistance through funding provided by the Community Services Block Grant (CSBG). The CSBG Poverty Alleviation Model includes Navigation & Transitioning Out of Poverty Case Management Services and as a member of the Community Action Partnership and as a Community Action Agency, provides for support and services focused on the entire family. The needs of parents and children are considered in tandem to elevate the well being of the household for greater impact.CSBG Totals:979 Individuals served477 Households servedEmergency services provided - Total 552 as follows:Rent payments- 309Mortgage payments- 21Utility payments- 185Emergency housing placement- 37SKILL QUEST:Our mission is to clear a path for low to moderate income adults living in Dallas, Collin, and contiguous counties to jobs paying a family living wage with benefits and a career path, while providing local employers with a highly skilled workforce ready to work. Our vision is of a future in which every person has access to the highest quality education to develop their full potential at affordable costs.

ENROLL NORTH CENTRAL TEXAS:Community Health Services provides access to insurance and social services programs for underserved families including Enroll North Central Texas Grant- Navigator Grant under the Affordable Care Act (ENCTX). Community Council provides health care access services to 56 counties that represent 9,946,273 people (36.9%) of the Texas population; approximately 16% of the square miles of the state.- In 2018 72,233 consumers assisted with general inquiries and enrollment- In 2018 - 20,012 have received enrollment assistance from a federal certified Navigator (face-to-face appointments)- In 2018 - 3,320 enrolled in a Qualified Health Plan (QHP) In 2018 - 13,398 consumers assisted with enrollment with CHIP/Children's Medicaid Application- The Connecting to Kids program deploys bi-lingual staff in a 5-county area including Dallas, Collin, Denton, Rockwall and Tarrant Counties.- In 2018 - 10,721 families assisted- In 2018 - 18,201 kids assisted- In 2018 - 453 Supplemental Nutrition Assistance Program (SNAP) applications. SNAP offers nutrition assistance to eligible, low-income individuals and families and provides economic benefits to communities. SNAP is the largest program in the domestic hunger safety net.- In 2018 -9 women qualified for the Texas Women Health Program (TWHP). Our Certified Navigators help to determine if the consumer is eligible. Women must apply for the program and be approved to get services and our Navigators help determine qualifications. The three programs featured are Healthy Texas Women Initiative, the Family Planning Program and Breast and Cervical Cancer Services.- In 2018 - 212 women qualified for the Pregnancy Medicaid program and received assistance with enrollment. This aid is for pregnant women with little or no income, offering health-care benefits during pregnancy and up to 2 months after birth of the baby. Benefits include:- Doctor visits- Drugs ordered by a doctor- Labor and delivery- Lab tests and X-rays- Hospital care- Glasses- Rides to the doctorOur Certified Navigators guide the consumer through some basic questions in the prescreening tool to find which benefits and support services they are eligible to receive including 33 Applications for the Temporary Assistance for Needy Families (TANF) program, which is designed to help needy families achieve self-sufficiency, and 63 Applications for Elderly Medicaid.Our certified Navigators held 311 community events to provide education or resource information.

COMMUNITY AND YOUTH SERVICES:Community Youth Development Dallas contributes to the Poverty Alleviation Mandate and address the complex issues of poverty in our community by serving youth through age 17 who live in and/or attend school in the designated Dallas Zip Codes, 75216 & 75217 with the target age range of 10 to 17 years. Specifically, CYD Dallas increases awareness of and access to services; maximizes involvement of people with low incomes, pursue positive individual, family and community level change and engage local community partners and citizens in solutions.Community Youth Development (CYD) was the brain-child of the 74th Texas Legislature in 1995. The overall goal is to have a positive impact on the lives of children and youth and to enhance their critical development.The Department of Family and Protective Services initial purpose of the program was to provide funding assistance to community collaborations designed to alleviate family and community conditions that lead to crime, through prevention programming. Dallas CYD is a collaboration of community-based organizations and resources committed to providing a healthy community that will promote a legacy for its young people that supports positive character development, self-esteem, and pride. It was the Dallas zip code community of 75216 that was identified in 1995, as having one of the highest incidences of juvenile crime in the state of Texas. Community-based organizations and resources began providing prevention program services and juvenile arrest rates and delinquency referrals to the Dallas County Juvenile Department (DCJD) declined. Programs such as after school services, recreation, and gang prevention alternatives prevented many of the children from entering the juvenile justice system. In January 2001, the Dallas CYD program expanded to include another Dallas Zip Code community of 75217. Similar prevention strategies began operating in this community as well. Family members of eligible participants are only eligible for services if the service involves the entire family, including the eligible youth participant. Parental involvement is viewed as an essential component of the CYD program and therefore parents are encouraged to participate in the Parental Involvement offering. Dallas CYD will start to address basic needs and offer emergency services; ultimately working towards self-sufficiency and enhancing economic mobility by implementing and executing the Whole Person, Whole Family Navigation Services model for poverty alleviation. In 2018, 2,508 youths were served. All participants remained 100% arrest free. INFORMATION AND ASSISTANCE:-211:2-1-1 North Central Texas, Dallas Region contributes to the Poverty Alleviation Mandate and address the complex issues of poverty in our community by serving 8 Counties (Dallas, Collin, Denton, Hunt, Kaufman, Ellis, Navarro and Rockwall) 24 hours a day, 7 days a week, 365 days a year, by increasing awareness and access to services; building local solutions specific to local needs and engaging local community partners.This team addresses basic needs and offer emergency services; ultimately working towards self-sufficiency and enhancing economic mobility by implementing and executing the Whole Person, Whole Family Navigation Services model for poverty alleviation. With over 1500 programs, services and Community Based Organizations (CBOs) in the database, 2-1-1 improves outreach to direct services and referrals for working-families, seniors, disabled citizens and caregivers. Part IV, Line 2b: The custodial funds listed in audited financial statements pertain to an employee fund - amounts deducted through a payroll deduction for employees who wish to donate for the purpose of recognizing employees by purchasing a small gift or assisting with a small office party on occasions like: marriage, birth, or adoption of children, and resignation of an employee.
Part X, Line 2: The Council is exempt from federal income taxation under Internal Revenue Code Section 501(c)(3), and contributions to it are tax deductible within the limitations prescribed by law. Accordingly, no provision for federal income tax is reflected in the accompanying financial statements. As of September 30, 2018, the Council had no uncertain tax positions that qualify for either recognition or disclosure in the financial statements.

Form 990, Part VI, Section A, line 4 Any changes and new policies implemented by the organization during the year relate to the addition of the CSBG grant.
Form 990, Part VI, Section B, line 11b The form 990 is provided to the Board before a duly called board meeting at which it will be considered. It is then presented at the board meeting and the board is given the opportunity to review and ask questions. If the quesitons are answered to the board's satisfaction, the 990 is filed.
Form 990, Part VI, Section B, line 12c The board members are given a copy of the conflict of interest policy when they join the board and annually thereafter. The board members sign a statement annually indicating that they have been given a copy of the policy and disclosing whether they have any conflict of interest, and if so the nature of the conflict. Board members who fail to sign the statement or to disclose conflicts are subject to removal from the Board. The conflict of interest policy is outlined in the Employee Handbook and provided to all new hires and annually thereafter or upon revision, which ever occurs first. Employees are required to acknowledge receipt and accept the terms of the Handbook in its entirety; acknowledgement is tracked and addressed accordingly.
Form 990, Part VI, Section B, line 15 The Board reviews salary survey information for other organizations in Dallas County of similar size and budget to determine the reasonableness of the CEO compensation. The board will annually undertake a performance evaluation of the CEO and recommend compensation besed on the salary survey information and the performance evaluation.
