AGENCY STAFF PERFORM ANNUAL SITE VISITS WITH MONTHLY DOCUMENTATION TO VERIFY THE QUALITY AND USE OF FOOD AND PROPER MANAGEMENT OF THE DISTRIBUTION PROGRAM. FUNDS ARE ASSIGNED A PROJECT NUMBER AND ALL EXPENSES ASSOCIATED WITH A SPECIFIC GRANT ARE CODED TO THE APPROPRIATE PROJECT NUMBER. PART I, LINE 3 THE PRESIDENT/CEO'S COMPENSATION IS DISCUSSED AND APPROVED BY THE BOARD OF DIRECTORS. THE CFO'S COMPENSATION IS DETERMINED BY THE PRESIDENT/CEO. THE BOARD APPROVES THE COMPENSATION BUDGET ANNUALLY. THIS PROCESS WAS LAST PERFORMED IN JUNE 30, 2019.
PART I, LINE 4B THE ORGANIZATION AND ERIC S. COOPER, PRESIDENT/CEO, ENTERED INTO A SUPPLEMENTAL EXECUTIVE RETIREMENT PLAN TO BE MAINTAINED BY THE ORGANIZATION. THE PLAN WAS EFFECTIVE APRIL 1, 2009 AND ENDING ON THE EARLIER OF THE TERMINATION OF ERIC S. COOPER'S SERVICE CAPACITY OR JULY 31, 2018. UPON THE PLAN'S TERMINATION ON JULY 1, 2018 THE ORGANIZATION DETERMINED THE ORIGINAL PLAN HAD NOT BEEN EXECUTED PROPERLY AND THERE WAS IN FACT NO 457(B) DEFERRAL IN PLACE. HOWEVER, THE ORGANIZATION HAD BEEN CONTRIBUTING MONEY TO THE PLAN FOR THE PREVIOUS TEN YEARS, RESULTING IN A BALANCE IN THE DEFERRAL ACCOUNT OF APPROXIMATELY $400,000. THE BOARD OF DIRECTORS VOTED ON A ONE-TIME PAYMENT TO BE MADE TO ERIC S. COOPER IN REGARDS TO THE 457(B) PLAN TERMINATION.
