WITH A MISSION IS TO IMPROVE THE MENTAL HEALTH OF INDIVIDUALS AGES 55 AND OLDER IN CENTRAL ILLINOIS, THE UNIT PROVIDES QUALITY MENTAL HEALTH SERVICES WITH STAFF WHO ARE EDUCATED ON THE MOST CURRENT TREATMENT INTERVENTIONS FOR BOTH PATIENTS AND THEIR FAMILIES. THE DIRECT ADMISSION PROCESS PREVENTS PATIENTS FROM EXPERIENCING LONG WAIT TIMES IN EMERGENCY ROOMS WHEN SUFFERING FROM CONFUSION, DEPRESSION OR OTHER FORMS OF MENTAL ILLNESS. THE UNIT'S STAFF WORK WITH A TEAM OF EXPERTS COMPOSED OF PSYCHIATRISTS, HOSPITALISTS, MEDICAL SPECIALISTS, NUTRITIONISTS, PHYSICAL, OCCUPATIONAL AND SPEECH THERAPISTS TO PROVIDE A MULTIDISCIPLINARY TEAM APPROACH. THE UNIT IS STAFFED 24 HOURS SEVEN DAYS A WEEK WITH REGISTERED NURSES. TWO LICENSED SOCIAL WORKERS AND A CERTIFIED RECREATIONAL THERAPIST ARE ALSO ON STAFF. DISCHARGED PATIENTS HAVE THE OPPORTUNITY TO ATTEND DMH'S INTENSIVE OUTPATIENT PROGRAM FOR CONTINUED CARE. -ACTION: DMH WILL PROVIDE SPECIFIC EDUCATIONAL INFORMATION TO AREA PROVIDERS ON MENTAL HEALTH ISSUES IMPACTING SENIORS AGE 55 AND OLDER AND HOW PROVIDERS CAN EASILY MAKE A REFERRAL TO THE PROGRAM. ALL ASSESSMENTS ARE FREE REGARDLESS IF THE PATIENT IS ADMITTED TO THE PROGRAM. TYPICALLY, INDIVIDUALS WILL ACTIVELY PARTICIPATE IN GROUP THERAPY SESSION FOR 3-6 MONTHS DEPENDING ON INDIVIDUAL NEEDS. ALL PATIENTS IN THE PROGRAM ARE SEEN FOR MEDICATION MANAGEMENT BY A PSYCHIATRIST ON A MONTHLY BASIS.-GOALS: 1. ADDED TO THIS PERCENTAGE WAS AN ESTIMATED PERCENTAGE OF THE POPULATION THAT IS ABOVE THE 200% POVERTY LEVEL AND LESS THAN 600% OF THE POVERTY LEVEL. THIS COMBINED PERCENTAGE WAS THEN DECREASED BY THE PERCENTAGE OF ILLINOIS RESIDENTS WHO HAVE GAINED INSURANCE THROUGH THE AFFORDABLE CARE ACT. THIS ADJUSTED PERCENTAGE WAS APPLIED TO THE HOSPITAL'S CHARGES TO ESTIMATE THE AMOUNT OF GROSS CHARGES THAT MET ITS FINANCIAL ASSISTANCE GUIDELINES. IN SOME INSTANCES, DATA WAS NOT AVAILABLE FOR MACON COUNTY, SO CITY OF DECATUR DATA WAS USED.STEP 3: COLLECTING AND ANALYZING DATAPRIMARY DATA WAS COLLECTED FROM JULY 30, 2018 TO AUGUST 19, 2018. THE COMMUNITY HEALTH NEEDS ASSESSMENT FOCUS GROUP MET ON SEPTEMBER 6, 2018, WHERE COMMUNITY LEADERS PROVIDED INPUT REGARDING COMMUNITY HEALTH STRENGTHS AND CONCERNS, AS WELL AS TOP HEALTH CONCERNS SEEN THROUGHOUT THE MACON COUNTY AREA. THIS DIVERSE GROUP OF INDIVIDUALS WAS CHOSEN TO PARTICIPATE IN THE 2019 COMMUNITY HEALTH NEEDS ASSESSMENT FOCUS GROUP BECAUSE OF THEIR INSIGHT TO THE COMMUNITY'S POPULATION, INCLUDING HEALTH NEEDS. SECONDARY DATA WAS COLLECTED THROUGH SEVERAL SOURCES. THE ILLINOIS PROJECT FOR LOCAL ASSESSMENT OF NEEDS (IPLAN) IS A COMMUNITY HEALTH ASSESSMENT AND PLANNING PROCESS THAT IS CONDUCTED EVERY FIVE YEARS BY LOCAL HEALTH JURISDICTIONS IN ILLINOIS. BASED ON THE ASSESSMENT PROTOCOL FOR EXCELLENCE IN PUBLIC HEALTH (APEX-PH) MODEL, IPLAN IS GROUNDED IN THE CORE FUNCTIONS OF PUBLIC HEALTH AND ADDRESSES PUBLIC HEALTH PRACTICE STANDARDS. ADDITIONAL RESOURCES WERE UTILIZED THROUGHOUT THE PROCESS, INCLUDING BUT NOT LIMITED TO, U. S. CENSUS BUREAU AMERICAN FACT FINDER, SOCIAL IMPACT RESEARCH CENTER AT HEARTLAND ALLIANCE, ILLINOIS DISTRICT REPORT CARD, AND ILLINOIS COUNTY BEHAVIORAL RISK FACTOR SURVEY.STEP 4: SELECTING PRIORITIESTHE FOCUS GROUP PARTICIPANTS DISCUSSED THE PRIORITY IN WHICH COMMUNITY HEALTH NEEDS SHOULD BE ADDRESSED AND DETERMINED IT BEST TO FOCUS ON SERVICES THAT IMPACTED THE COMMUNITY AS A WHOLE VS. PATIENT SPECIFIC SERVICES. STEP 5: DOCUMENTING AND COMMUNICATING RESULTSTHE 2019 COMMUNITY HEALTH NEEDS ASSESSMENT FOCUS GROUP WAS RECORDED FOR FUTURE REFERENCE AND RESULTS WERE TALLIED TO DETERMINE THE AREA'S TOP STRENGTHS/VALUES, BARRIERS/CONCERNS, AND THE TOP 3 HEALTH NEEDS. DATA HAS SHOWN 14.6 PERCENT OF THE PATIENTS AT THE CROSSING CLINIC DID NOT HAVE A PREVIOUSLY ESTABLISHED RELATIONSHIP WITH A PRIMARY CARE PROVIDER.FURTHERMORE, THE COST OF AMBULANCE AND TAXI FARES ARE PAID BY DMH WHEN SUCH TRANSPORTATION IS REQUIRED FOR THE CARE OF PATIENTS WITHOUT THE MEANS TO PAY FOR SUCH SERVICES. ALSO, MEMBERS OF OUR COMMUNITY MAY ACCESS ALL THE SERVICES OF OUR PRIME TIME PROGRAM, INCLUDING FREE VAN TRANSPORTATION TO AND FROM MEDICAL APPOINTMENTS. DUE TO THE HIGH PERCENTAGE OF INDIGENT PATIENTS SEEN IN OUR EMERGENCY ROOM, DMH PROVIDES A SUBSTANTIAL SUBSIDY TO THE EMERGENCY ROOM PHYSICIANS IN ORDER TO ALLOW THEM TO PROVIDE COMPETITIVE SALARIES TO THE PROVIDERS. IN A RESPONSE TO THE COMMUNITY NEEDS ASSESSMENT REGARDING CHILD AND TEENAGE HEALTH PROBLEMS, DMH CONTINUES TO EXPLORE ALTERNATE AND COMPLEMENTARY CARE DELIVERY MODELS, INCLUDING TELEMEDICINE LINKS TO SPECIALTY CARE IN SPRINGFIELD. ONCOLOGY: SCREENINGS, HEALTH FAIRS AND MORE THE DMH CANCER CARE INSTITUTE PARTICIPATES IN COMMUNITY HEALTH FAIRS AND EVENTS THROUGHOUT THE YEAR. DURING THE HEALTH FAIRS, EDUCATIONAL MATERIALS ARE PRESENTED TO RAISE AWARENESS ON SUCH TOPICS AS: COLORECTAL CANCER, LUNG CANCER, AS WELL AS MELANOMA AND SKIN CANCER. THROUGHOUT MARCH 2019, CCI INCORPORATED AN EXTENSIVE MARKETING CAMPAIGN THAT WAS HELD THROUGH THE MONTH OF MARCH THROUGH THE FIRST WEEK OF APRIL, PARTICIPATED IN THE ST. PATRICK'S DAY PARADE AND DISTRIBUTED FLYERS AND PENCILS; AND DISTRIBUTED MATERIALS AT MACON COUNTY HEALTH DEPARTMENT, DECATUR PUBLIC LIBRARY AND CROSSINGS HEALTH CARE CENTER. IN JUNE 2019, DMH PARTNERED WITH CANCER CARE SPECIALISTS OF CENTRAL ILLINOIS AND ST. MARY'S HOSPITAL TO RECOGNIZE ABOUT 1,200 CANCER SURVIVORS AND THEIR FAMILY MEMBERS FOR NATIONAL CANCER SURVIVORS DAY (NCSD). THE EVENT IS CELEBRATION OF LIFE THAT FEATURES DINNER, GUEST SPEAKERS, AND ENCOURAGEMENT FOR THE SURVIVORS. DECATUR MEMORIAL HOSPITAL PARTICIPATES IN A LOCALLY SPONSORED EVENT CALLED COME TOGETHER, LET'S WALK. THIS EVENT TAKES PLACE IN JUNE AND WAS CREATED OUT OF A GRASSROOTS EFFORT BY LOCAL VOLUNTEERS, SERVES AS A FUNDRAISER FOR BREAST, OVARIAN AND CERVICAL CANCER PATIENTS. PROCEEDS FROM THIS BENEFIT ARE USED TO HELP FINANCE CANCER PREVENTION AND SCREENING PROGRAMS FOR WOMEN IN OUR COMMUNITY. DMH CANCER RESEARCH AND THE DMH WOMEN'S HEALTH CENTER ALSO RECEIVES SUPPORT FROM THESE FUNDS FOR OUTREACH PROGRAMS. DMH CANCER INSTITUTE PARTICIPATED IN THE MACON COUNTY RELAY FOR LIFE IN JULY 2019 WITH THE THEME OF CHRISTMAS.
PART VI, LINE 2 CONT DMH ALSO CO-SPONSORED THE ANNUAL KERBYSTRONG 100 AT THE MACON SPEEDWAY IN JULY. THE PRIMARY MISSION OF THIS EVENT WAS TO EDUCATE THE COMMUNITY ABOUT COLORECTAL CANCER SCREENING; PROCEEDS FROM THE EVENT BENEFIT PATIENTS WHO NEED IMMEDIATE FINANCIAL ASSISTANCE AND TO SUPPORT A CANCER NAVIGATOR.THE DMH CANCER CARE INSTITUTE ALONG WITH THE CANCER CARE SPECIALISTS OF CENTRAL ILLINOIS, THE CARPENTERS LOCAL 742 AND THE MACON COUNTY HEALTH DEPARTMENT SPONSOR A FREE PROSTATE CANCER SCREENING EVENT EVERY YEAR IN SEPTEMBER FOR PROSTATE CANCER AWARENESS MONTH. MEN RECEIVE PSA BLOOD TESTING AND DIGITAL RECTAL EXAMS FREE OF CHARGE. RESULTS ARE SCREENED AND SENT TO THE PATIENT'S PHYSICIAN OF CHOICE. THE PROSTATE CANCER SCREENING SAW A 9% INCREASE FROM 153 PARTICIPANTS IN 2018 TO 167 PARTICIPANTS IN 2019. OF THE 167 PARTICIPANTS, 116 RECEIVED NORMAL SCREENING RESULTS AND 51 RECEIVED ABNORMAL SCREENING RESULTS. THERE A NO KNOWN CANCER DIAGNOSES AT THIS TIME. CCI SPONSORED A NEW EVENT THIS YEAR FOR WOMEN, LADIES NIGHT OUT. THIS WAS A MOVIE NIGHT THAT PARTICIPANTS GET ENTRY TO THE MOVIE, DRINK, POPCORN AND BAG WITH INFORMATION.THE EARLY DETECTION CONNECTION IS A BREAST CANCER AWARENESS EVENT HELP IN OCTOBER. COMMUNITY MEMBERS DRIVE TO ONE OF THREE LOCATIONS WHERE DMH EMPLOYEES AND VOLUNTEERS GO TO THE CAR TO DISTRIBUTE EDUCATIONAL MATERIALS, A FREE GIFT, AND A PANERA BAGEL. 1300 BAGS WERE DISTRIBUTED IN 2019.IN NOVEMBER 2019, THE DMH CANCER CARE INSTITUTE HOSTED THE SHINE A LIGHT ON LUNG CANCER EVENT AT THE CANCER CENTER. THE EVENT PROVIDES EDUCATION, INSPIRATION AND SUPPORT FOR THOSE IMPACTED BY LUNG CANCER AND THOSE AT RISK. PHYSICIAN SPEAKERS, ENTERTAINMENT, AND REFRESHMENTS ARE PROVIDED. WE HAD 55 ATTENDEES FOR THE SHINE A LIGHT EVENT IN 2019.IN DECEMBER 2019, WE GATHERED TOGETHER WITH SUPPORT GROUP MEMBERS IN AN EVENT CALLED THE COMMUNITY OF HOPE CELEBRATION. THIS CELEBRATION TAKES THE PLACE OF OUR REGULARLY SCHEDULED DECEMBER SUPPORT GROUP MEETINGS AND ALLOWS ALL GROUPS TO COME TOGETHER AND ENJOY THE HOLIDAY SEASON. WE PROVIDE LIGHT REFRESHMENTS AND ENTERTAINMENT IN THE LOBBY OF THE CANCER CARE INSTITUTE. WE HAD 65 ATTENDEES FOR THE COMMUNITY OF HOPE EVENT IN 2019.MONTHLY CANCER SUPPORT GROUPS ARE OFFERED FREE OF CHARGE FOR PATIENTS AND THEIR CAREGIVERS. IN ADDITION, BEREAVEMENT SERVICES ARE PROVIDED WEEKLY FOR CHILDREN WHO HAVE LOST A LOVED ONE AND MONTHLY FOR ADULTS, AT NO COST. 93 SUPPORT GROUPS WERE HELD IN 2019 FOR ADULTS, CHILDREN, AND CAREGIVERS10 CLASSES (2 EDUCATIONAL SERIES, "STRENGTH FOR THE JOURNEY")15 CLASSES (3 GRIEF SERIES, "GROWING THROUGH GRIEF")24 CLASSES (3 SMOKING CESSATION SERIES, "FREEDOM FROM SMOKING")2 ENHANCE CLASSES FOR WOMEN'S SKIN/HAIR CARE AFTER CANCER TREATMENT IN 2019 (NEW)1 HEALING ARTS CLASS (NEW) GROOMED FOR MEN'S HAIR/SKIN CARE VOUCHERS (NEW)MAINTAINING GOOD NUTRITION IS ESPECIALLY IMPORTANT IF YOU HAVE CANCER, AS BOTH THE ILLNESS AND THE ASSOCIATED TREATMENT CAN AFFECT YOUR APPETITE. ADDITIONALLY, CANCER AND CANCER TREATMENTS CAN ALTER YOUR BODY'S ABILITY TO TOLERATE AND UTILIZE IMPORTANT NUTRIENTS. AT THE CANCER CARE INSTITUTE, WE HAVE A TEAM OF REGISTERED DIETITIANS THAT PROVIDE COMPLIMENTARY SERVICES TO HELP PATIENTS MAINTAIN PROPER NUTRITION BEFORE, DURING AND AFTER CANCER TREATMENT. IN 2019, OVER 150 COMPLIMENTARY NUTRITIONAL VISITS WERE PROVIDED TO PATIENTS. AT THE CANCER CARE INSTITUTE, THE REGISTERED DIETITIAN WORKS WITH PATIENTS CLOSELY TO IDENTIFY AREAS OF IMPROVEMENT THROUGH NUTRITION INTERVENTIONS. THE DIETITIAN WILL COMPLETE A NUTRITION CARE PLAN THAT SUITS THE NEEDS OF THE PATIENT AND THEIR FAMILY. OUR REGISTERED DIETITIANS SEE PATIENTS ON AN INDIVIDUAL BASIS WITH FOLLOW UP AND MONITORING AS OFTEN AS DESIRED BY THE PATIENT IN ORDER TO MEET HIS OR HER NUTRITIONAL GOALS. THE REGISTERED DIETITIANS OCCASIONALLY OFFER GROUP CLASSES ON VARIETY OF NUTRITION TOPICS. WOMEN AND CHILDREN'S HEALTH REALIZING THE IMPORTANCE OF EDUCATION TO THE HEALTH OF WOMEN AND CHILDREN, DMH OFFERS FREE CLASSES TO THE COMMUNITY INCLUDING EDUCATION FOR GRANDPARENTS, SIBLINGS, NEWBORN CARE AND BREAST FEEDING, PRENATAL EDUCATION, AND INFANT CPR. STAFF FROM THE FAMILY BIRTH CENTER AND PEDIATRICS PARTICIPATES IN HEALTH FAIRS DURING THE YEAR TO PROVIDE WRITTEN EDUCATIONAL MATERIALS INCLUDING SAFE SLEEP ENVIRONMENT FOR INFANTS, NUTRITION, IMMUNIZATIONS, AND HEALTH NEEDS FOR WOMEN AND CHILDREN. RECOGNIZING THAT WOMEN ARE OFTEN THE PRIMARY DECISION MAKERS FOR MEDICAL ISSUES IN THE FAMILY UNIT, EDUCATION ABOUT MEN IS ALSO AVAILABLE DURING HEALTH FAIRS TO EDUCATE WOMEN ON THE MEDICAL NEEDS OF THEIR LOVED ONES. A LACTATION CONSULTANT AND SEVERAL LACTATION COUNSELORS PROVIDE EDUCATION BEFORE, DURING AND AFTER CHILDBIRTH. DMH PARTICIPATES IN A PROGRAM TO PROVIDE INDIGENT MOTHERS WITH BREAST PUMPS. COMMUNITY BASED CARE OUR MISSION IS TO IMPROVE THE HEALTH OF THE PEOPLE OF CENTRAL ILLINOIS BY PROVIDING THE HIGHEST QUALITY CARE OF COLLABORATING WITH PATIENTS TO IMPROVE THEIR HEALTH, PROMOTE INDEPENDENCE, AND MAINTAIN DIGNITY IN THE COMFORT OF THEIR HOME. WE DELIVER HOME CARE AND HOSPICE CARE INCLUDING SERVICES FROM NURSING, THERAPY, SOCIAL WORKER, AND BATH AIDES. OUR HOSPICE TEAM PARTICIPATES IN COMMUNITY HEALTH FAIRS THROUGHOUT THE YEAR TO PROVIDE EDUCATIONAL INFORMATION TO THE COMMUNITY.
