Braxton County Memorial Hospitals convenient location enables area residents to receive outpatient services provided by registered nurses when the presence of a physician is not required. In 2019, 821 procedures were performed with a physicians order and received a wide array of treatments close to home through BCMHs outpatient nursing department. By reducing unnecessary stays in the hospital, this time-saving service has been very well-received. BCMHs same day surgery area accommodated 611 surgery patients in 2019. Most of these individuals returned home hours after their procedure. Though some were admitted to the hospital for additional care. Other outpatient procedures from the laboratory, radiology and cardiopulmonary departments of BCMH totaled 102,013 in 2019. Wellness programs and primary family care were also provided through the Braxton County Community Health Center where 8,762 men, women and children had their healthcare needs met by qualified providers in 2019. Additionally, 5,074 home-delivered healthcare visits were made to patients through the BCMH Home Health.

The Emergency Department of Braxton County Memorial Hospital provides 24-hour medical care to area residents as well as travelers. With its close proximity to I-79, BCMH has served as the gateway for many strangers in need of emergency healthcare. In 2019, 8,894 individuals were provided with compassionate healthcare from professional physicians and nurses. BCMH continues to comply with medical criteria and resource requirements necessary for designation as a level IV trauma center, thereby serving the community in a very unique way by maintaining this designation. BCMH is involved with a state-wide trauma system which enables good communication and work relationships with emergency medical services and other healthcare facilities. The statistics noted above represent individual lives touched at Braxton County Memorial Hospital in 2019. These numbers do not reflect the families and friends of those patients.

As a small, community-based healthcare facility, Braxton County Memorial Hospital offers convenient accessibility and personal attention. While meeting the healthcare needs of individuals close to home of 325 patient admissions to BCMH in 2019, many received compassionate care at the hands of their own family, friends and neighbors. For some swing bed patients recovering from an acute illness or surgery, an extended stay involving skilled nursing care and physical therapy was warranted. Ancillary services including cardiopulmonary, radiology and laboratory were provided to all inpatients as appropriate.

X 2 Most of the entities that comprise the System are tax-exempt organizations and not subject to federal or state income taxes in accordance with Section 501c3 of the Internal Revenue Code. On such basis, they will not incur any liability for income taxes, except for possible unrelated business income. AHS, WVUIS and GHC are organizations subject to federal and/or state income taxes. The System does not have any material uncertain tax positions as of December 31, 2019 and 2018.

Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 2 On January 1, 2019, West Virginia University Hospitals, Inc. became the sole member of Braxton County Memorial Hospital BCMH a not-for-profit critical access hospital located in Gassaway, WV.
Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 5 The 2018 Community Health Needs Assessment CHNA was performed to identify the perceived needs of the community it serves and how those needs can be better served. Braxton County Memorial Hospital BCMH utilized a community survey to obtain primary data from their stakeholders. According to the CHNA, written surveys were mailed to 200 homes in the Hospitals service area, which includes the surrounding counties of Braxton, Clay, Gilmer, Lewis, Nicholas and Webster counties. Survey recipients were selected at random from an admissions report detailing only name and address. Thirty-one percent, or 62 surveys were returned. Through the survey, BCMH learned what the stakeholders perceived to be the three most common health problems that impact the local community. These included 1. Heart Disease, 2. Diabetes, and 3. Drugs. BCMH also identified what is perceived to be the three most common risky behaviors that impact and influence the local communitys poor health. These behaviors included 1. Drug Abuse, 2. Smoking and 3. Alcohol Abuse. The CHNA can be found here https//wvumedicine.org/braxton/about-us/reports/. Due to the transfer of ownership noted above BCMH did not complete/adopt an Implementation Plan in 2019 as is allowed in the exceptions of 501r3.
Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 11 Braxton County Memorial Hospital joined the West Virginia University Health System in 2019. Integration into the West Virginia University Health System will give BCMH access to additional resources across all areas of practice which will help address issues identified in the most recent CHNA.
Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 13b Braxton County Memorial Hospital BCMH offers discounted care only to individuals who qualify for free care. BCMH offers a Prompt-Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 50 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible for either prompt pay discount, the patient/guarantor must meet certain requirements.
Group Braxton County Memorial Hospital, Inc Line Part V, Section B, Line 24 All patients are charged an amount equal to gross charges regardless of payment method. Form and Line Reference Explanation
Part I Line 3c Braxton County Memorial Hospital BCMH uses the 200 Federal Poverty Guideline to determine free care eligibility. BCMH offers a Prompt - Pay Discount Program - patients who have no third party coverage uninsured or uninsurable may be eligible for a 20 self-pay discount while patients who have a guarantor balance may be eligible for a 20 discount. To be eligible for either discount the patient/guarantor must meet certain requirements.
Part I Line 6a Braxton County Memorial Hospital community benefit numbers are reported in total with the other hospitals within West Virginia United Health System. The 2018 amounts can be found at the following web address http//wvumedicine.org/about/community-benefit/.
Part I Line 7 Column f - Total community benefit expense for 2019 is 4,200,905 and is 22.78 of total net expenses. To calculate net expense, bad debt of 1,773,381 was deducted from total expenses of 20,216,243 as shown in Part IX line 25 of the core Form 990.
Part I Line 7 Worksheet 2 from the IRS Schedule H instructions was used to derive the Cost-to-charge ratio, which was used to calculate Charity Care, Unreimbursed Medicaid and other means-tested government programs at cost. To calculate the Medicare allowable cost shown in Part III question 8, we used the allowable costs from the Medicare Cost report.
Part III Line 2 Bad Debt Expense at Cost on Line 2 was calculated by multiplying bad debt expense of 1,773,381 by our cost-to-charge ratio of 62.82 for a total of 1,114,038.
Part III Line 3 Estimated bad debt attributable to patients eligible for charity care on Line 3 was calculated by pulling all outstanding patient balances of 5,000 or greater from our patient accounting system. That report totaled 73,943, which was then multiplied by the cost-to-charge ratio of 62.82 for a total of 46,451.
Part III Line 3 We feel that at a minimum, the estimated bad debt attributable to patients eligible for charity care of 46,451 should be considered community benefit due to the fact that anyone with outstanding balances of 5,000 or greater in our demographic usually qualifies as catastrophic if the patient completes the application process. In our Charity Care Policy, we define catastrophic care as any illness or injury that will likely require continuous or frequent treatment for more than one year and would have qualified for charity care if paperwork was completed.
Part III Line 4 Braxton County Memorial Hospitals financial statements are prepared on a consolidated basis as a member of the WV United Health System. The footnote for Accounts Receivable is as follows Patient accounts receivable are reported at net realizable value. For accounts receivable associated with services provided to patients who have third-party coverage, the System estimates net realizable value based on the estimated contractual reimbursement percentage, which in turn is based on current contract provisions and historical paid claims by payor. For self-pay accounts, including uninsured and patient responsibility accounts, the net realizable value is determined using historical collection experience, adjusted for estimated conversions of patient responsibility portions, expected recoveries and changes in trends to estimate implicit price concessions. Management continually reviews the estimated net realizable value of accounts receivable by monitoring cash collections, economic conditions and trends, changes in payor mix, changes in federal or state healthcare coverage and other matters. The System grants credit without collateral to its patients, most of whom are local residents and are insured under third-party payor agreements, primarily with Medicare, Medicaid and various commercial insurance companies. The System records accounts receivable net of estimated price concessions and such amounts have historically been within managements expectations. The mix of accounts receivable at December 31, 2019 is as follows Medicare - 27, Medicaid - 17, Blue Cross - 24, Commercial, managed care and other - 26, Patients - 6, totaling 100.
Part III Line 4 For receivables associated with self-pay patients, which includes both patients without insurance and insured patients with deductible and copayment balances, the System records a significant provision for bad debts in the period of service on the basis of past experience, which indicates that many patients are unable or unwilling to pay the portion of their bill for which they are financially responsible. The difference between the billed rates and the amounts actually collected after all reasonable collection efforts have been exhausted is charged off against the allowance for doubtful accounts.
Part III Line 8 The amount reported in Part III Line 6 8,512,531 was calculated using the total Medicare allowable cost from the Medicare cost report.
Part III Line 8 Braxton County Memorial Hospital is reimbursed 100 by Medicare reimbursement includes DSH and IME for services provided to Medicare patients.
Part III Line 9b Braxton County Memorial Hospital does have a debt collection policy. When a patient has been approved for financial assistance under our charity care policy, they will not be sent to bad debt. Additionally, if a patient is being evaluated for charity care, the patient will not be sent to a bad debt agency pending charity guarantor status until the pending status has been finalized approved/denied. All other patients with outstanding balances will be processed through billing and collections pursuant to our Financial Policy.
Part VI Line 2 Braxton County Memorial Hospital considers several components in assessing how the organization determines the need of the communities it serves. Through feedback from stakeholders, outside consultants and collaborating with various departments, a needs assessment takes into account several factors including physicians to population ratios, physician availability in the entire service area, and general health risks that are perceived to be the highest priority by the stakeholders and hospital leadership. The service area for BCMH includes Braxton, Clay, Gilmer, Lewis, Nicholas and Webster counties.
Part VI Line 3 The West Virginia United Health System employs financial analysts to meet with patients at its hospitals to discuss financial eligibility for our charity care program. Braxton County Memorial Hospital has qualification requirements, as well as, applications for charity care available at registration areas. Patients that do not qualify for state or federally funded programs can apply for the charity care program at Braxton County Memorial Hospital.
Part VI Line 4 Established in 1981, BCMH is a non-profit, critical access, 25 bed hospital that has extensive outpatient capabilities as well as inpatient services. The CHNA explains that the hospital offers 24-hour emergency care and is recognized as a designated level IV trauma center, as well as offering quality inpatient acute care, skilled nursing beds, respite care, inpatient and outpatient surgery, and home health care. The hospital has an extensive outpatient facility which included outpatient nursing and lab services, in addition to EKG, pulmonary function and stress testing through cardio-pulmonary services. Radiological services include 16-slice CT scans, open MRIs, x-rays, enhanced nuclear medicine, digital mammography and bone density testing.
Part VI Line 4 Braxton County is in the central part of West Virginia and is approximately 62 miles from the nearest level I trauma center in Charleston, WV. In 2019, Braxton County had a total population of 14,154, an average household income of 54,385 and an unemployment rate of 15.
Part VI Line 5 BCMH serves as an educational site for students in various levels of education, including nursing, medical School, high school and students who are in college and majoring in other healthcare related studies.
Part VI Line 5 BCMH continued to serve as a local designated site in the community for the American Red Cross as well as a meeting facility, provided at no cost for non-profit, civic and healthcare related organizations including Alcoholics Anonymous, the WV Chapter of the Association of Professionals in Infection Control APIC, and the Braxton County Local Emergency Planning Council LEPC.
Part VI Line 5 BCMH nurses participate in Stop the Bleed training in Braxton County public schools. BCMH employees also participate in various career and health fairs throughout the local community.
Part VI Line 6 Braxton County Memorial Hospital is a part of the WV United Health System WVUHS. WVUHS is a not-for-profit corporation formed to serve as part of an integrated health science and healthcare delivery system. WVUHS serves as the parent corporation to an affiliated group of healthcare providing enities. The strategic plan of the System states intent to build a regional health care delivery system in its service areas, while offering a variety of options for providers who want to participate. The System maintains a demonstrated commitment to assist rural communities in preserving and improving the health care available to the patients it serves.
Part VI Line 6 System management is focused on recruitment of staff and employees to meet the growing needs of the aging population in the Systems service areas. Other hospitals in the System include West Virginia University Hospitals, Inc. in Morgantown, WV, United Hospital Center in Bridgeport, WV, Camden-Clark Memorial Hospital in Parkersburg, WV, City Hospital, Inc. in Martinsburg, WV, Jefferson Memorial Hospital in Ranson, WV, Potomac Valley Hospital in Keyser, WV, St. Josephs Hospital of Buckhannon, Inc. in Buckhannon, WV, Reynolds Memorial Hospital, in Glen Dale, WV, West Virginia Healthcare Cooperative, doing business as, Summersville Regional Medical Center in Summersville, WV, and Community Health Association, doing business as, Jackson General Hospital in Ripley, WV.
Part VI Line 6 The System also includes the physician practices of United Physicians Care, Inc. and Camden-Clark Physician Corp that operate in conjunction with the System hospitals along with United Summit Center, a behavioral health center located in Clarksburg, WV. In addition to these practices, the System also includes WVUH-East, Inc. and Camden-Clark Health Services which operate as management companies for their respective hospitals. The System also includes University Healthcare Foundation in Martinsburg, WV, United Health Foundation in Bridgeport, WV, Camden-Clark Foundation in Parkersburg, WV, St. Josephs Foundation of Buckhannon, Inc. in Buckhannon, WV, and Reynolds Memorial Foundation in Glen Dale, WV which perform various fundraising activities for their respective hospitals.

Form 990, Part I, Line 1 and surrounding communities. The Hospital provides charity care to patients who meet certain criteria under its charity care policy, without charge or at amounts less than established rates.
Form 990, Part VI, Section B, Line 11b Draft copies of the Form 990 are sent to the board members for review. Questions and discussion are then asked or held during the board meeting. If no errors are found, the board approves the filing of Form 990.
Form 990, Part VI, Section B, Line 12c Monitoring and enforcing compliance with conflict of interest policy on an annual basis.
