Barrett Hospital & HealthCare is a 18 licensed bed acute care critical access hospital. Our patients are served by an inpatient hospital, 24-hour emergency department, and a provider based clinic. Our ancillary services include diagnostic imaging, laboratory, inpatient and outpatient surgery, physical and occupational therapy, home health, hospice, cardiac rehab, pharmacy, cardiopulmonary, respiratory, and diabetic education.The hospital provided 1,879 days of acute care services, 313 observation days, and 510 swing bed days. In addition, the hospital provided 28,167 clinic visits, 22,763 outpatient visits, 4,531 emergency room visits, 3,159 home health visits, and 1,653 hospice visits.

Part II-B, Line 1: The Organization is a member of several organizations that take part in lobbying. A portion of the dues paid to the various organizations is for lobbying costs. These various organizations advocate on behalf of their members by supporting legislation that is in their collective interests. In addition, during the fiscal year, the Organization paid $50,000 to MHA in support of Montana Ballot Initiative I-185 to eliminate the sunset date for Medicaid expansion in Montana. $1,000 was paid to Montanans for the Six Mill in support of the Montana University System ballot initiative.

Barrett Hospital Development Corporation Part V, Section B, Line 5: A Steering Committee was convened to assist Barrett Hospital & HealthCare in conducting the CHSD assessment process. A diverse group of community members representing various organizations and populations within the community (ex. public health, elderly, uninsured) came together in November 2016. The Steering Committee met twice during the CHSD process; first to discuss health concerns in the community and offer their perspective in designing the survey instrument and again to review results of the survey and focus groups and to assist in the prioritization of health needs to address.In January 2017, surveys were mailed out to the residents in Barrett Hospital & HealthCare's service area. Barrett Hospital & HealthCare provided the National Rural Health Resource Center with a list of outpatient and inpatient encounters. Those zip codes with the greatest number of encounters were selected to be included in the survey. A random list of 700 residents was then selected from Prime Net Data Source. Additionally, three focus groups interviews were conducted in March 2017. Participants were identified as people living in Barrett Hospital & HealthCare's service area. The focus group participants were selected to represent various consumer groups of healthcare including senior citizens and local community members. Each interview lasted up to 90 minutes in length and followed the same line of questioning.Barrett Hospital & HealthCare adopted their implementation strategy in September of 2017.The Director of Beaverhead County Public Health, participated in the assessment, and the assessment was conducted by the Montana Office of Rural Health.

Barrett Hospital Development Corporation Part V, Section B, Line 11: Barrett Hospital & HealthCare's Community Steering Committee identified the most important health needs to be addressed by reviewing the CHNA, secondary data, community demographics, and input from representatives representing the broad interest of the community. The implementation planning committee determined which needs or opportunities could be addressed considering Barrett Hospital & HealthCare's parameters of resources and limitations. The committee then prioritized the needs/opportunities using the additional parameters of the organizational vision, mission, and values, as well as existing and potential community partners. Participants then created a goal to achieve through strategies and activities, as well as the general approach to meeting the stated goal (i.e. staff member responsibilities, timeline, potential community partners, anticipated impact(s), and performance/evaluation measures).The prioritized health needs as determined through the assessment process and which the facility will be addressing relates to the following healthcare issues:1. Patient Financial Services re-branded staff as Patient Financial Advocates and re-trained staff to support the new brand for the department. Internal ads were circulated explaining the role of the Patient Financial Services department as Patient Financial Advocates. Barrett Hospital continued to promote their Financial Assistance Policy. Financial assistance flyers were updated and redistributed to schools, churches, assistance offices, food banks, etc. throughout Beaverhead County and Western Madison County. Effective January 2019, a financial aide screen tool was made available on the hospital website. Strategy 1.2: Help patients better maximize and understand their medical benefits.Barrett Hospital developed education flyers for patients including "Questions to Consider when Selecting an Insurance Plan and "Is my Provider in Network with my Insurance Plan? and distributed the flyers throughout Beaverhead County and Western Madison County. Flyers are also available throughout the hospital and clinic. These flyers were updated again in March 2019 after a review by the Patient Advisory Council. Strategy 1.3: Strengthen advertising and outreach efforts at BHH.Barrett Hospital began the process to redesign the facility website for ease of use and to provide the information requested by our community. The new website designed was approved and will be implemented in Fiscal Year 2019. The Community Health Needs Assessment and Implementation plan are posted to the hospital website. A calendar of upcoming events was added to the website in January 2019.Goal 2: Promote health, wellness, and healthy lifestyles in Beaverhead CountyStrategy 2.1: Enhance BHH's presence in the community as a leader in health and wellness programs/initiatives.Barrett Hospital & HealthCare continues to offer several programs including the Health Improvement Program (HIP), free sports physicals for students, child birth classes, lactation education, community health fair with lab screenings, care seat safety classes, trauma prevention and cancer support groups. Barrett Hospital's Cardiac Rehab Nurse and Inpatient Physical Therapist presented information to the local Rotary chapter about their Cardiac Rehab program in September 2017. An OB Nurse also presented information to the local Rotary chapter in October 2017.The Cardiopulmonary department started providing post-sleep study CPAP education for patients. The facility Trauma Nurse Coordinator provided education to 26 staff and community members about the "Stop the Bleed" program. Three additional courses were provided to the community.Barrett Hospital began partnering with Allevant, Post-acute services specialist, to expand the Swing Bed program at Barrett Hospital to meet the transitional care needs of our patients. Strategy 2.2: Improve community awareness of existing health and wellness resources in the community.The hospital is developing a strategy for disseminating and advertising a community resource list of available services within the community. Barrett Hospital will partner with Public Health to hold meetings of community health and wellness stakeholders to update resources and continue community-wide discussion on health and wellness. Barrett Hospital is currently working with Sue Hansen, Director of Public Health, on an implementation strategy.Barrett Hospital has started to include regular social media posts regarding various health resources including fitness resources in conjunction with Beaverhead County Public Health.Strategy 2.3: Explore expanding BHH worksite wellness program to include local employers/ community members.Barrett Hospital is continuing to watch legislation by the US government, specifically surrounding EEOC Wellness Regulations, to ensure compliance with the latest legislation. Barrett Hospital consulted with Gallagher Benefits to complete a "Wellbeing Inventory" of the current state to determine opportunities in the current wellness program. Barrett Hospital is awaiting results to determine potential areas of expansion and potential areas of external interest. Goal 3: Decrease alcohol and drug abuse in Beaverhead County by enhancing preventative strategies and programs.Strategy 3.1: Support alcohol and drug abuse prevention programs for youth in Beaverhead County.The hospital continues to support Public Health's efforts to explore partnering with schools and SWCDP Prevention Specialist Lead to restart a 'DARE like program in the schools. The community is applying for grant dollars to develop this program. Barrett Hospital also will support the continuation of the ACES program in Beaverhead County. Jana Barnes continues to be involved in the ACES program and shares information with providers and hospital staff regarding the program. Strategy 3.2: Improve BHH chronic pain management strategies and community awareness of chronic pain services.Barrett Hospital & HealthCare is exploring strategies for offering Chronic Pain Management education for community members and providers and implement if feasible. The facility is also exploring strategies for assessing likelihood of addiction with emergency department patients. The facility will consider providing on-call physical therapist screening of injuries for emergency department visits.Goal 4: Improve access to mental health services and suicide prevention resources in Beaverhead County.Strategy 4.1: Address cost as a barrier for community members seeking out mental health services.Barrett Hospital & HealthCare is exploring multiple options in this arena including collaborating with local mental health providers and exploring feasibility of alternative delivery models for behavioral health services. The hospital will continue to advocate for patients by addressing cost as a barrier to seeking mental health services through legislative contacts.Strategy 4.2: Collaborate with local community organizations on addressing mental health issues.Western Montana Mental Health was closed increasing the need for local mental health providers. Barrett Hospital met with other community stakeholders in December 2017 to determine a plan of action to provide care for patients. The LAC received some funding and has opened a Help and Resource Center that will be staffed 4 hours per day. Barrett Hospital also met with Lieutenant Governor Clooney and Senator Welborn to discuss budget cuts and the impact to mental health services.
Schedule H, Part V, Section B, Line 11 continued: Strategy 4.3: Improve access to mental health services at BHH.Barrett Hospital is exploring expanding tele-psychiatry services including greater access to child psychiatry (finding more providers and expanding hours). We will continue to offer integrated behavioral health services at BHH and monitor/ improve contracted Western Montana Mental Health Crisis Response Team services.Barrett Hospital submitted a grant application to assist in the implementation of an integrated behavioral health model to the Montana Healthcare Foundation. Implementation of the program will start in 2020.Four items that were identified in the CHNA will not be addressed by the current implementation plan. Some of the services identified are offered in limited capacity at the facility. At this time, it is not practical or financially feasible to offer expanded services in these areas. The services are as follows: a) Dermatology services b) Senior well checks/ community paramedicine/ home health c) Cancer services d) Outpatient services expanded hoursCurrently, dermatology services are provided by some primary care providers. Dermatology is a specialty service shortage in this area. The hospital will continue to re-evaluate the feasibility of expanding these services in the future. Barrett Hospital currently partners with regional oncologists to travel to Dillon to provide clinic services as well as offering some chemotherapy services. As part of the CHNA Implementation Plan, Barrett Hospital is working to strengthen their advertising and outreach programs through the strategies outlined above. Through this process, we anticipate increased awareness of services the hospital provides including cancer services and walk-in clinic hours. ENT (ear/nose/throat) Services were also identified as a service need by the community. Barrett Hospital previously has been involved in an unsuccessful regional partnership with an ENT. Barrett Hospital is unable to support having a full-time ENT specialist based on need for services. CHNA respondents indicated they did not get or had to delay getting needed healthcare services. Top reasons for delaying or not receiving needed healthcare services were: It cost too much, my insurance didn't cover it, and wait was too long for an appointment. Barrett Hospital conducts continuous reviews of their pricing to remain competitive with the current market. The hospital is involved in various cost savings initiatives to reduce their costs and the cost of care for our patients. The hospital will continue to work with our insurance partners to offer education to the community regarding insurance coverage whenever possible.Barrett Hospital & HealthCare is committed to providing high value healthcare and services for our community. As an organization, we make every effort to develop relationships with a broad range of collaborative partners within our community and neighboring communities to improve the quality of care and services available to our patients. Therefore, the Medicare shortfall is considered a community benefit. Medicare allowable costs were obtained from the 2018 Medicare cost report. The Medicare Cost Report is completed based on the rules and regulations set forth by the Centers for Medicare and Medicaid Services.
Part III, Line 9b: Once a patient applies for financial assistance their accounts are held at Barrett Hospital & HealthCare or at our extended business office until a determination has been made on their eligibility for financial assistance. Accounts are not sent to collections during the period the patient is eligible for assistance. If an application does not contain all necessary documentation, the patient is notified of all missing elements and an extended deadline is set for the patient to provide additional documentation. All applicants for financial assistance are notified in writing upon approval or denial of financial assistance.

Part VI, Line 3: Barrett Hospital & HealthCare is committed to implementing and applying their billing and collection policy in a consistent manner while making every effort to assist patients with limited means to receive the support necessary for their financial obligations whether the patient is insured or uninsured.Financial Assistance Policy (FAP) information can be found on our website. The FAP is distributed in multiple patient areas of our clinic (exam rooms/front desk) and hospital (public areas/registration desks). Our Financial Assistance Policy is advertised monthly in a local publication and copies have been mailed out to area businesses and churches in our service area to be posted in their buildings. There is written notification regarding our FAP on every bill that is sent to the patient/guarantor. When calling patients to educate them on their benefits, we advise patients that if they have concerns about the estimated patient costs that they can discuss them with our financial counselors. All patients calling in to discuss their bills or setup payment plans are routed through the financial counselors for process consistency and to provide the best resources available to the patient. A financial assistance notification also prints on after visit summaries and discharge instructions that are provided to patients after all visits.
Part VI, Line 4: Barrett Hospital & HealthCare serves the majority of Beaverhead County and western Madison County residents. The primary service area includes 9 zip codes throughout Beaverhead County and 5 zip codes in Madison County. The size and rural makeup of Barrett Hospital's service area play a significant role in the services Barrett Hospital & HealthCare provides. Beaverhead County is the largest county in the state of Montana and consists of a population of approximately 9,345 with a population density of 1.7 people per square mile. The nearest major hospital is 64.4 miles from Barrett Hospital. The population aged 65+ in Beaverhead County is 20.6% which is higher than the demographics for the State of Montana (17.2%) and national demographics (14.9%) for this population group. The upsurge in population aged 65+ continues to increase the need for senior services including healthcare. Socioeconomic factors also play a role in the health of the population that Barrett Hospital & HealthCare services. Beaverhead County has a higher percentage of the population that falls below the federal poverty levels and higher uninsured rates than National and State of Montana measures. 17.1% of the population of Beaverhead County is considered low income and falls below the federal poverty levels compared to 14.6% for the State of Montana. The median income for Beaverhead County is $42,577 which is lower than the Montana median income of $46,766. Beaverhead County also has an uninsured rate of 19% as compared to the State of Montana at 17% and the national measure at 13.3%.

All surplus revenues are reinvested into the facility, equipment, or programs at Barrett Hospital & HealthCare. The funds are utilized for quality initiatives to improve patient care, expand our facility, expand available services, and advance training programs. As a not-for-profit health system, Barrett Hospital & HealthCare reinvests profits into the community by helping manage chronic conditions including diabetes, promoting wellness, and providing programs to serve the poor and uninsured. In addition, Barrett Hospital is invested in the education and training of physician residents, medical students and other medical professional students at our site including 23 physician resident/students and PA/NP students and 13 occupational therapy, nursing, EMT, laboratory scientists and diagnostic imaging students. Approximately 86 volunteers contribute their time and efforts to the hospital. The hospital also relies on community members for various advisory councils including the Patient Advisory Council. Barrett Hospital & HealthCare is the only hospital in Beaverhead County. Medical staff privileges in the hospital are open to all qualified physicians and providers. Barrett Hospital operates an emergency room that is accessible to anyone needing care regardless of their ability to pay. The emergency room has approximately 4,500 visits annually.

Form 990, Part VI, Section A, line 3 HealthTech Management Services provides general management services including the employment of our CEO, Ken Westman. Ken Westman received $250,000 in compensation from HealthTech Management Services during the 2018 calendar year.
Form 990, Part VI, Section A, line 6 The sole member of the organization is the Public Hospital District, Beaverhead County (d/b/a Barrett Hospital and Healthcare) a Montana statutory public hospital district.
Form 990, Part VI, Section A, line 8b There are no committees with the authority to act on behalf of the governing body.
Form 990, Part VI, Section B, line 11b An electronic copy of the 990 is presented to the Board during their regular board meeting for review and approval.
Form 990, Part VI, Section B, line 12c Annually all employees, the CEO, and board members are required to complete a conflict of interest statement. All possible conflicts of interest are reviewed by the Compliance Committee. The Committee determines what, if any, action is necessary for the individual with the potential conflict to take to avoid said conflict. Compensation grades are adjusted, as necessary, and reviewed with Leadership Council for approval. Physician/provider compensation is reviewed for fair market value with legal counsel upon execution or renewal of an employment agreement or contract. The Board of Directors reviews and approves all physician/provider agreements/contracts.
