| FORM 990, PART III, LINE 4A, DESCRIPTION OF PROGRAM SERVICE: | (CONT'D) ADULTS ARE ALSO OFFERED PSYCHOSOCIAL REHABILITATION, INTEGRATED TREATMENT FOR SUBSTANCE USE, OPIOID TREATMENT, AND EMPLOYMENT SERVICES. BEHAVIORAL HEALTH SERVES A POPULATION THAT IS 63% AFRICAN-AMERICAN, 26% WHITE, NEARLY 5% HISPANIC WITH THE REMAINING COMPOSED OF INDIVIDUALS OF A VARIETY OF RACES. A MAJORITY, 73%, REPORT THEY ARE UNEMPLOYED. OUR CLIENTS FACE TRANSPORTATION, ECONOMIC, AND EDUCATIONAL CHALLENGES. AS CHALLENGES ARE IDENTIFIED, OUR SOCIAL WORKERS, CASE MANAGERS, AND CARE COORDINATORS ASSIST THE CLIENT IN OBTAINING RESOURCES, PROVIDE ADDITIONAL EDUCATION, AND COORDINATE ACROSS MULTIPLE TREATMENT PROVIDERS. SWOPE HEALTH BEHAVIORAL HEALTH, MEDICAL AND DENTAL SERVICES CONTINUE TO IMPROVE OUR SERVICE INTEGRATION. THIS HAS RESULTED IN INCREASED CARE COORDINATION, RESOURCE ATTAINMENT, AND BETTER PATIENT OUTCOMES. WE CONTINUE TO EXPLORE MULTIPLE SOLUTIONS TO PROVIDE THE BEST ARRAY OF SERVICES AS POSSIBLE. BEHAVIORAL HEALTH CONTINUES TO ESTABLISH PARTNERSHIPS WITH OTHER COMMUNITY MENTAL HEALTH CENTERS, MEDICAL SCHOOLS, HEALTH CARE FOUNDATIONS, AND MORE. BY COLLABORATING, WE ARE ABLE TO EXPAND RESOURCES TO OUR CLIENTS. IN 2019, WE EXPANDED TELEHEALTH, MEDICATION-ASSISTED TREATMENT, AND SCHOOL-BASED SERVICES. WE EXPECT ALL WILL CONTINUE THEIR EXPANSION IN 2020 AND BEYOND. ACCESS TO CARE REMAINS A TOP PRIORITY AND THE IMPETUS FOR THESE EXPANDED SERVICES. WE SEE CONTINUAL GROWTH IN THE NUMBER OF INDIVIDUALS SEEKING CARE AND BEING REFERRED TO CARE. OUR CARE COORDINATOR TEAM SERVES AS A LINK BETWEEN HOSPITALS AND THE OUTPATIENT CLINICS, AND THEIR OUTREACH SHORTENS THE LENGTH OF TIME TO ACCESS CARE, INCREASES SERVICES, AND IMPROVES COLLABORATION BETWEEN THE REFERENTS AND THE TREATMENT TEAM. HOMELESS OUTREACH: IN 2019, SWOPE HEALTH ASSISTED MORE THAN 3,600 HOMELESS PATIENTS WITH HEALTH CARE, HOUSING AND OTHER ESSENTIAL SERVICES. SWOPE HEALTH STAFF MEMBERS ARE ACTIVE IN THE HOMELESS COMMUNITY AND OFTEN VISIT SHELTERS, COMMUNITY DAY CENTERS, AND HOMELESS ENCAMPMENTS TO ASSESS BASIC MEDICAL AND BEHAVIORAL NEEDS OF THE HOMELESS. A NEW MOBILE MEDICAL UNIT WITH TWO EXAM ROOMS WAS PURCHASED AND PLACED INTO SERVICE IN 2017. IT CONTINUES TO BE WELL RECEIVED AND IS A TREMENDOUS RESOURCE FOR COMMUNITY OUTREACH. ONCE A PATIENT IS IDENTIFIED AND REGISTERED IN OUR SYSTEM, WE CONDUCT ASSESSMENTS TO DETERMINE WHICH SERVICES ONE MIGHT NEED AND/OR QUALIFY FOR. CASE MANAGERS ASSIST IN OBTAINING PHYSICAL AND MENTAL HEALTH TREATMENT AS WELL AS HOUSING, TRANSPORTATION, AND FINANCIAL ASSISTANCE. CASE MANAGERS ALSO WORK TO FIND PERMANENT HOUSING FOR THE HOMELESS THROUGH PROGRAMS SUCH AS SECTION 8 HOUSING, SUPPORTED COMMUNITY LIVING SERVICES AND SHELTER-PLUS CARE, AS WELL AS LOW-INCOME AND OTHER SUBSIDIZED HOUSING. FOR THOSE WITH MENTAL HEALTH CONCERNS, INDIVIDUAL AND GROUP THERAPY ARE OFFERED TO ASSIST WITH OBTAINING OVERALL MENTAL AND PHYSICAL HEALTH GOALS. SWOPE HEALTH ALSO PROVIDES MEDICATION MANAGEMENT, MEDICATION ASSISTANCE AND DENTAL SERVICES. OUR OUTREACH AND TRANSPORTATION STAFF DRIVE HOMELESS INDIVIDUALS FROM AREA SHELTERS TO APPOINTMENTS AT OUR CLINIC TO ENSURE THEY ARE GETTING REGULAR CHECK-UPS AND PROVIDED EMERGENT HEALTHCARE AS THE NEED ARISES. HEALTHCARE HOME: SWOPE HEALTH PARTICIPATES IN BOTH THE PRIMARY CARE AND BEHAVIORAL HEALTH HOME INITIATIVES SPONSORED BY THE STATE OF MISSOURI. THE PRIMARY CARE MEDICAL HOME HAS AN AVERAGE ENROLLMENT OF 1,500 CLIENTS AND THE BEHAVIORAL HEALTH ENROLLMENT AVERAGES 400 CLIENTS. THE FOCUS OF BOTH HEALTH HOMES IS GREATER UTILIZATION OF THE HEALTH HOME TEAM FOR TREATMENT OF CHRONIC DISEASES AND PREVENTION WITH LESS INAPPROPRIATE USAGE OF EMERGENCY CARE AND REDUCTION OF UNWARRANTED HOSPITALIZATIONS. |
