	As of December 31, 2019, the ND Banks Benefit Trust (NDBBT) covered approximately 3,484 persons for Health Insurance. In addition, approximately 1,810 persons were covered for Dental Insurance and approximately 1,187 were covered for Vision Insurance at the end of the year. The NDBBT completed and sent out the Form 1095B's to the employees of the participating banks with under 50 employees - and electronically filed the corresponding Form 1094C with the IRS. The Trust also provided guidance on completion of the Forms 1095C and 1094C for the participating banks with over 50 employees.The NDBBT sponsored booths at both the ND Banker's Association and Independent Community Banks of ND conventions. The Trust also sponsored wellness focused events (i.e. Meditation Breaks, healthy sack lunches for golf outings and chair massage breaks).The NDBBT continues to promote and encourage healthy lifestyles through monthly articles published in both the ICBND and NDBA monthly magazines and promoting participation in Blue Cross Blue Shield's Wellness Summits and Wellness Program Trainings. The NDBBT (working through Middaugh Benefits Consulting) offers ancillary benefits (life, AD&D, LTD, STD, voluntary life) to North Dakota banks.The ND BANKS Benefit Trust had 53 participating banks and 2 banking associations in the year from January 1, 2019 - December 31, 2019.

Part X, Line 2:	Accounting principles generally accepted in the United States of America require trust management to evaluate tax positions taken by the trust and recognize a tax liability (or asset) if the trust has taken an uncertain tax position that more likely than not would not be sustained upon examination of the Internal Revenue Service. The trust is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress. Accounting principles generally accepted in the United States of America require trust management to evaluate tax positions taken by the trust and recognize a tax liability (or asset) if the trust has taken an uncertain tax position that more likely than not would not be sustained upon examination of the Internal Revenue Service. The trust is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress. The program goal is to teach participants simple skills to help them lose weight, improve their health and boost their overall quality of life with ultimate goal of reducing the prevalence of obesity-related diseases such as type-2 diabetes, heart disease and metabolic syndrome. For the NDBBT members that applied and were accepted into the program - the NDBBT paid the fee.
