| FORM 990, PART III, LINE4A | THE NETWORK - ROOM, BOARD AND WATCHFUL OVERSIGHT (RBWO): By using these partner agencies, the Organization is able to provide a full continuum of services ranging from intensive psychiatric care, adoptions to residential group homes, therapeutic foster care and maternity care. The Organization receives a set rate in order to provide placement as well as additional support services for children and adolescents who are placed within the continuum ensuring that youth are able to maintain the most appropriate and lowest level of care. The Organization is a statewide program and receives referrals from the Georgia Division of Family and Children Services. |

The Organization receives referrals for Pathways to Permanence meeting coordination and the Organizations PACT referrals from Amerigroup. |
| FORM 990, PART III, LINE 4D | 1. Teen Parent Connection - Teen Parent Connection (TPC) is a system of care designed to incorporate a broad array of services and supports that are organized into a coordinated network. This network focuses on the whole needs of the child and family coordinating prevention, intervention, and treatment services for pregnant and parenting youth in child welfare. MAAC is the administrator of this program. 2. The Network - Crisis Continuum - MAAC receives significant number of calls from all over the state of Georgia for youth who are in crisis and who are in need of an immediate placement. There are a variety of reasons why these youth are in placement crisis. MAAC has developed a Crisis Continuum using partner agencies to provide placements that will be "on hold" waiting for referrals and will have extra supports available in order to meet any need a youth has while in crisis. 3. Independent Living Program (ILP) Training - Training for the Independent Living program is coordinated and facilitated by MAAC throughout the state of Georgia. The goal of the program is to reach out to the over 3,000 eligible youth currently in foster care and out of foster care between the ages of 14-21 to provide life skills and education necessary to become self-sufficient, living independently and maintain employment. MAAC also provides training to foster caregivers and group home staff on developing and implementing life skills to their transitional age youth. In addition to these activities MAAC also coordinates and supports Individual Development Accounts. This encompasses financial literacy training and matching funds for youth who saved using the Opportunity Passport model from Jim Casey Youth Opportunities Initiative. 4. Youth Support - Youth Support began as an idea created by young adults facing the transition process to benefit other young adults facing the transition process to benefit other young adults facing transition. Youth transitioning from care are served by Youth Support Partners, young adults with lived experience in the Foster Care, Juvenile Justice and/or Mental Health systems. Services include case planning, peer mentoring, education about resources geared toward youth facing transition and an expanded support network of peers and caring adults. 5. Georgia Youth Opportunities Initiative - In addition to network services, MAAC has expanded its collaboration to include the Georgia Youth Opportunities Initiative which encompasses the Georgia EmpowerMEnt Group. Georgia EmpowerMEnt is one of the five strategies of the Georgia Youth Opportunity Initiative. MAAC also provides administrative oversight for two additional strategies which are the Community Partnership Group and the Self-Evaluation Team. Other key partners are the Jim Casey Youth Opportunities Initiative, The Community Foundation of Greater Atlanta, and the Georgia Department of Family and Child Services. Georgia EmpowerMEnt consists of current and former foster youth ages 14-26 that grew up in foster care in the state of Georgia. MAAC offers support for these young advocates by providing coordination and administrative oversight to support their advocacy efforts. Activities include regional tribal development, state and national advocacy, and court improvement. |
