| FORM 990 - ORGANIZATION'S MISSION | DONORCONNECT PROVIDES HIGH QUALITY ORGAN AND TISSUE PROCUREMENT SERVICES TO MEDICAL PROFESSIONALS IN UTAH, SOUTHEASTERN IDAHO AND WESTERN WYOMING, AND ACTIVELY EDUCATES PROFESSIONALS AND THE PUBLIC ON THE BENEFITS OF ORGAN DONATION. |
| FORM 990, PAGE 2, PART III, LINE 4B | PROFESSIONAL & PUBLIC EDUCATION - VISITED AND/OR CONTACTED HOSPITALS 455 TIMES AND MADE OVER 96 PRESENTATIONS TO PROFESSIONALS. IN 2019, DONORCONNECT MADE 877 VISITS, MAKING OVER 1,500 PRESENTATIONS TO JUNIOR AND SENIOR HIGH SCHOOLS IN UTAH, IDAHO AND WYOMING, TOTAL, SEEING 67,235 STUDENTS. THIS IS ONE OF, IF NOT THE, MOST AGGRESSIVE SCHOOL PROGRAMS IN THE COUNTRY. DONORCONNECT ALSO WORKED WITH FIVE CHAPTERS OF THE PUBLIC RELATIONS STUDENT SOCIETY OF AMERICA FOR NATIONAL DONATE LIFE MONTH ACTIVITIES ON THEIR CAMPUSES; WEBER STATE UNIVERSITY, OGDEN, UT. DIXIE STATE UNIVERSITY, ST. GEORGE, UT, BRIGHAM YOUNG UNIVERSITY, PROVO, UT, UTAH VALLEY UNIVERSITY, OREM, UT, BOISE STATE UNIVERSITY, BOISE, ID. DONORCONNECT ALSO WORKS WITH MANY EAGLE SCOUTS TO HELP THEM EARN THEIR EAGLE BADGE. IN 2019, WE ASSISTED 17 SCOUTS THAT WILL TYPICALLY CANVAS THEIR NEIGHBORHOOD GIVING OUT INFORMATION. THIS HAS BEEN A GREAT "GRASS ROOTS" PROGRAM TO HELP EDUCATE THE PUBLIC ABOUT THE NEED FOR DONATION. DONORCONNECT LOGGED 931 VOLUNTEER HOURS IN 2019. THE TRANSPLANT GAMES OF AMERICA WERE HELD IN SALT LAKE CITY, AUGUST 2018. THE GAMES IN SALT LAKE CITY HAD A RECORD 5,511 REGISTERED ATTENDEES AND IT IS ESTIMATED THAT THERE WERE ABOUT 8,000 PEOPLE WHO CAME TO SALT LAKE CITY TO ATTEND THE GAMES. THIS WAS THE LARGEST NUMBER OF REGISTERED ATTENDEES IN THE HISTORY OF THE GAMES. A GUINNESS WORLD RECORD FOR THE LARGEST GROUP OF SOLID ORGAN TRANSPLANT RECIPIENTS IN ONE LOCATION WAS SET. THE RECORD WAS ACCOMPLISHED WITH 540 RECIPIENTS GATHERING AT THE SALT PALACE CONVENTION CENTER AND REGISTERING AS OFFICIAL PARTICIPANTS FOR THIS RECORD. TEAM UTAH/IDAHO, MAKING IT THE LARGEST TEAM AT THE GAMES WITH 118 ATHLETES, RANGING IN AGE FROM 3 YEARS OLD TO 79 YEARS OLD. THE TEAM EARNED A TOTAL OF 204 MEDALS. SOME IN TEAM SPORTS AND OTHERS IN INDIVIDUAL SPORTS. |
| FORM 990, PAGE 2, PART III, LINE 4C | ON-LINE REGISTRY - THE YES UTAH DONOR REGISTRY HAS 2,621 ONLINE REGISTRANTS. 80% OF PEOPLE AGED 16 AND ABOVE REGISTERED AS DONORS COMPARED TO 2010 CENSUS DATA, AND 60.49% OF APPLICANTS SAID "YES" AT THE TIME OF GETTING THEIR LICENSE. IN IDAHO THERE WERE 799 ONLINE REGISTRANTS AND 62% OF PEOPLE AGED 16 AND ABOVE REGISTERED AS DONORS. IDAHO HAD 61.25% OF APPLICANTS SAY "YES" AT THE TIME OF GETTING THEIR LICENSE. THE REGISTRIES ARE SECURE, ON-LINE DATABASES THAT PROVIDE LEGAL DOCUMENTATION OF A DECEASED ORGAN AND TISSUE DONOR'S WISHES. THE NATIONAL AVERAGE FOR REGISTRIES IS 59.3%. THE NATIONAL AVERAGE FOR FIRST TIME APPLICANTS SAYING "YES" IS 49%. BECAUSE THE DRIVER LICENSE OFFICES IN BOTH STATES ARE SO INTEGRAL TO WHAT WE DO TO GET PEOPLE REGISTERED, WE MADE 74 VISITS TO DRIVER LICENSE OFFICES TO UPDATE STAFF ABOUT DONATION AND QUESTIONS THEY MIGHT BE ASKED. WE ALSO MEET WITH DRIVER LICENSE LEADERSHIP IN BOTH UTAH AND IDAHO TWICE A YEAR TO KEEP THEM UPDATED AND SEEK THEIR IDEAS AS TO HOW WE CAN FURTHER DELIVER OUR MESSAGE. |
| FORM 990, PAGE 6, PART VI, LINE 11B | THE FORM 990 IS REVIEWED BY THE CFO PRIOR TO SIGNING BY THE PRESIDENT. DONORCONNECT ALSO PROVIDES THE BOARD AN OPPORTUNITY TO REVIEW THE 990 AND GIVE FEEDBACK. THE FEEDBACK FROM THE BOARD WILL BE NOTED IN THE BOARD MEETING MINUTES. |
| FORM 990, PAGE 6, PART VI, LINE 12C | EACH YEAR THE EXECUTIVE DIRECTOR AND THE CORPORATE COMPLIANCE OFFICER REVIEW THE COMPLETED CONFLICT OF INTEREST FORMS. IF A CONFLICT IS IDENTIFIED, THEY MEET WITH THE EMPLOYEE, DIRECTOR OR OFFICER TO DISCUSS. A CONCLUSION IS REACHED AND A PLAN IS DETERMINED AND IMPLEMENTED. AN EXAMPLE IS A TRANSPLANT SURGEON WHO IS ON THE BOARD AND IS CONTRACTED WITH DONORCONNECT TO PERFORM ORGAN RECOVERIES. WE WOULD EXCLUDE THAT PERSON FROM DISCUSSIONS REGARDING CONTRACTED RECOVERY SURGEON PAY. |
| FORM 990, PAGE 6, PART VI, LINE 15A | FOR DETERMINATION OF THE EXECUTIVE DIRECTOR COMPENSATION, THE BOARD MEETS AND FOLLOWS IRS GUIDELINES FOR DETERMINING EXECUTIVE PAY. THE BOARD ALSO REVIEWS THE ANNUAL AOPO COMPENSATION SURVEY TO BENCHMARK WITH SIMILAR ORGANIZATIONS. |
| FORM 990, PAGE 6, PART VI, LINE 15B | FOR DETERMINATION OF COMPENSATION OF OTHER OFFICERS AND KEY EMPLOYEES, THE PRESIDENT AND CFO DETERMINE THE PAY RANGE BY USING SALARY DATA FROM OTHER ORGAN PROCUREMENT ORGANIZATIONS AND UTAH ORGANIZATIONS TO DETERMINE THE RANGE FOR THE POSITION. EMPLOYEES ARE HIRED WITHIN THE RANGE BASED ON THEIR EXPERIENCE AND RELATIVE TO SIMILAR POSITIONS IN THE AGENCY. RAISES ARE GIVEN ON PERFORMANCE EVALUATIONS AND GOAL COMPLETION. |
