DURING THEIR STAY, THOSE FAMILIES WORK WITH STAFF TO SECURE PERMANENT AFFORDABLE HOUSING UPON THERE EXIT, 7 OUT OF 13 TRANSITIONED TO PERMANENT HOUSING. IN ADDITION, SHELTERCARE ALSO PROVIDED SHORT TERM HOUSING FOR 110 HOMELESS ADULTS WHO NEEDED A SAFE PLACE TO CONTINUE RECOVERY AFTER A HOSPITAL STAY. |

SHELTERCARE PROVIDED LONG TERM SUPPORTED HOUSING TO 246 HOMELESS ADULTS WHO ARE LIVING WITH A MENTAL ILLNESS. 275 INDIVIDUALS RECEIVED BEHAVIORAL HEALTH SERVICES AND DEVELOPED TREATMENT PLANS TO LEARN HOW TO MANAGE THEIR MENTAL HEALTH SYMPTOMS. 31 INDIVIDUALS RECEIVED CARE AT OUR MEDICALLY- ENHANCED RESIDENTIAL TREATMENT FACILITY WHICH PROVIDES A SAFE HOME-LIKE SETTING FOR ADULTS TRANSITIONING OUT OF THE STATE HOSPITAL. |
