THE CLINIC WAS AWARDED STATE FUNDING AS WELL TO REDUCE HOSPITAL AND EMERGENCY ROOM ADMISSION RATES BY PROVIDING PRIMARY CARE AND CARE COORDINATION SERVICES TO INDIVIDUALS WITH SEVERE AND CHRONIC MENTAL ILLNESS.
