THE PATIENT COMPASSION ENDOWMENT PROVIDES FINANCIAL ASSISTANCE TO INDIVIDUALS AND THEIR FAMILIES THROUGHOUT SOUTHWEST MONTANA WHO MUST TRAVEL OUT OF TOWN TO GET SPECIALIZED MEDICAL TREATMENT NOT AVAILABLE IN BUTTE. THE PROGRAM ALSO PROVIDES ASSISTANCE TO PATIENTS FROM THE SURROUNDING AREA TO TRAVEL TO BUTTE FOR THEIR MEDICAL CARE. IT HELPS FAMILIES AND INDIVIDUALS WITH FUEL COSTS, HOTEL EXPENSES, BUS VOUCHERS, AIRLINE TICKETS AND OTHER EXPENSES. IN ADDITION TO TRAVEL ASSISTANCE, THE PROGRAM HELPS PATIENTS AFTER BEING DISCHARGED FROM THE HOSPITAL WITH RECOVERY MEDICATION, EQUIPMENT AT HOME AND CO-PAYS. THE CONNECTING FOR A CANCER CURE ENDOWED FUND WAS STARTED FROM PROCEDES FROM THE UNO'S CHARITY GOLF TOURNAMENT LEAD BY MEG MURPHY. THE COMMUNITY HEALTH NEEDS ASSESSMENT INCORPORATED PRIMARY RESEARCH THROUGH A COMMUNITY HEALTH SURVEY WHICH RANDOMLY SAMPLED 400 INDIVIDUALS AGE 18 AND OLDER IN THE PRIMARY SERVICE AREA. THE SAMPLE DESIGN AND QUALITY CONTROL PROCEDURES USED IN THE DATA COLLECTION ENSURE THAT THE SAMPLE IS REPRESENTATIVE OF THE COMMUNITY. THE 2017 CHNA WAS A FOLLOW-UP TO A SIMILAR SURVEY CONDUCTED IN 2014 WHICH ALLOWED FOR TRENDING OVER TIME. IN ADDITION, AN ONLINE KEY INFORMANT SURVEY WAS CONDUCTED TO SOLICIT INPUT FROM KEY INFORMANTS WHO WERE SELECTED BECAUSE OF THEIR ABILITY TO IDENTIFY PRIMARY CONCERNS OF THE POPULATIONS WITH WHOM THEY WORK AND THE CONCERNS OF THE COMMUNITY OVERALL. IN ALL, 126 COMMUNITY STAKEHOLDERS TOOK PART IN THE ONLINE KEY INFORMANT SURVEY, INCLUDING REPRESENTATIVES FROM HEALTHCARE, EDUCATION, SOCIAL SERVICES, GOVERNMENT, LAW ENFORCEMENT, AND THE FAITH COMMUNITY. ON JULY 10, 2017, APPROXIMATELY 50 REPRESENTATIVES OF VARIOUS COMMUNITY ORGANIZATIONS MET TO EVALUATE, DISCUSS AND PRIORITIZE HEALTH ISSUES FOR THE COMMUNITY, BASED ON FINDINGS OF THE 2017 CHNA. THIS PROCESS WAS CO-LED BY ST. JAMES HEALTHCARE AND THE BUTTE-SILVER BOW PUBLIC HEALTH DEPARTMENT. THE 8 AREAS OF OPPORTUNITY IDENTIFIED IN THE CHNA AND THEIR RANKING INCLUDE:1. SUBSTANCE ABUSE 2. MENTAL HEALTH 3. NUTRITION, PHYSICAL ACTIVITY & WEIGHT 4. INJURY & VIOLENCE 5. TOBACCO USE 6. CANCER 7. ACCESS TO HEALTHCARE SERVICES 8. POTENTIALLY DISABLING CONDITIONS SUBSTANCE ABUSE: SUBSTANCE ABUSE WAS RATED AS THE NUMBER 1 CONCERN WITHIN THE COUNTY AMONG THE KEY INFORMANT GROUP. THIS INCLUDES DRINKING ALCOHOL AS WELL AS ILLICIT DRUG USE. FROM THE PARTICIPANTS IN THE TELEPHONE SURVEYS, 60.3% SAID THEY WERE CURRENT DRINKERS AND 21.4% SAID THEY WERE EXCESSIVE DRINKERS. IN REGARD TO ILLICIT DRUG USE, 2.2% HAD USED AN ILLICIT DRUG IN THE PAST MONTH, 4.6% HAVE SOUGHT HELP AND 47.3% SAID THEIR LIVES WERE NEGATIVELY IMPACTED BY SUBSTANCE ABUSE. OF THE KEY INFORMANTS, 68.6% VIEW SUBSTANCE ABUSE AS A MAJOR PROBLEM IN THE COMMUNITY AND ANOTHER 25.6% VIEWED IT AS A MODERATE PROBLEM.BASED ON THESE FINDINGS, A COMMUNITY WIDE TASK FORCE HAS BEEN FORMED TO ADDRESS THIS PROBLEM FOR BOTH DRINKING AND ILLICIT DRUG USE IN THREE AREAS: PREVENTION, TREATMENT, AND RECOVERY. THE TASK FORCE IS HEADED BY BUTTE CARES, INC., A NOT-FOR-PROFIT AGENCY THAT WORKS WITH AREA TEENS ON PREVENTION OF SUBSTANCE ABUSE. THE TASK FORCE INCLUDES REPRESENTATIVES FROM THE MEDICAL COMMUNITY (ST. JAMES HEALTHCARE AND LOCAL PHYSICIANS), PUBLIC HEALTH (BUTTE SILVER BOW HEALTH DEPARTMENT), SOCIAL SERVICE, LAW ENFORCEMENT, LOCAL GOVERNMENT, AND THE LOCAL SCHOOL DISTRICT (INCLUDING STUDENTS). THE TASK FORCE BEGAN MEETING IN LATE 2017 AND WILL CONTINUE THROUGH 2020. THE CURRENT FOCUS OF THE TASK FORCE IS ADVOCATING FOR ADDITIONAL CONTROLS ON REGULATING THE MEDICAL MARIJUANA SHOPS THAT EXIST IN BUTTE.IN ADDITION, ST. JAMES HEALTHCARE OFFERS SUPPORT FOR PREGNANT MOTHERS WHO HAVE SUBSTANCE ABUSE ISSUES. THE PROGRAM IS CALLED "THE FIRST 1,000 DAYS OF LIFE." ONE THOUSAND DAYS REPRESENT 270 DAYS IN THE WOMB, 365 DAYS OF THE FIRST YEAR AND 365 DAYS OF THE SECOND YEAR. THE PROGRAM OFFERS NUTRITIONAL SUPPORT WHILE THE MOTHER IS PREGNANT AND BEYOND, AS WELL AS BEHAVIORAL HEALTH COUNSELING FOR DEPRESSION, SUBSTANCE ABUSE, DOMESTIC VIOLENCE, ETC. THE PROGRAM'S PURPOSE IS TO STRENGTHEN FAMILIES AND MINIMIZE CHILD ABUSE AND FOSTER CARE PLACEMENT.MENTAL HEALTH: NEARLY 75% OF KEY INFORMANTS CHARACTERIZE MENTAL HEALTH AS A MAJOR PROBLEM IN THE COMMUNITY, WITH DISPARITIES SUCH AS ACCESS TO CARE, A LACK OF PROVIDERS AND STIGMA ESPECIALLY CHALLENGING. WORK CONTINUES ON MENTAL HEALTH ISSUES, FIRST IDENTIFIED IN THE 2014 CHNA. A COUNTY-WIDE REFERRAL SYSTEM FOR MENTAL HEALTH PATIENTS, PROJECT CONNECT, HAS BEEN DEVELOPED TO ENSURE THESE PATIENTS ARE NOT LOST IN THE SYSTEM. ST. JAMES HEALTHCARE IS CONTINUING TO SUPPORT MENTAL HEALTH TRAINING IN THE COMMUNITY. IN 2019, ST. JAMES HEALTHCARE PROVIDED 9 QPR (QUESTION, PERSUADE, AND REFER) SUICIDE TRAINING SESSIONS FOR A TOTAL OF 348 COMMUNITY MEMBERS TRAINED. ADDITIONALLY, MENTAL HEALTH SUPPORTS ARE OFTEN A COMPONENT OF OTHER PROGRAMS OFFERED IN THE COMMUNITY (I.E. THE FIRST 1000 DAYS PROGRAM).NUTRITION, PHYSICAL ACTIVITY & WEIGHT: ST. JAMES CONTINUES TO FOCUS ON NUTRITION, PHYSICAL ACTIVITY & WEIGHT AS INITIALLY IDENTIFIED AS A TOP NEED IN THE 2014 CHNA AND AGAIN IN THE 2017 CHNA. ST. JAMES HEALTHCARE OFFERS THE EVIDENCE-BASED DIABETES AND HEART DISEASE PREVENTION PROGRAM, WHICH PROVIDES NUTRITION EDUCATION AND SUPPORTS HEALTHY LIFESTYLE CHANGES FOR ADULTS WHO ARE AT RISK FOR DEVELOPING TYPE 2 DIABETES. ACROSS THE VARIOUS ADULT CLASSES OFFERED, 768 PEOPLE WERE SERVED BY ST. JAMES HEALTHCARE. AS WELL, IN THE CHILDREN AND FAMILY SUPPORT GROUP, 15 PARTICIPANTS ATTENDED 4 MEETINGS, FOR A TOTAL OF 60 ENCOUNTERS. INJURY & VIOLENCE: ST. JAMES HEALTHCARE PROVIDES ATHLETIC TRAINERS FOR HIGH SCHOOLS AND COLLEGES IN SOUTHWEST MONTANA ALONG WITH CONCUSSION MANAGEMENT PROGRAMS. TOBACCO USE AND CANCER: IN THE 2017 CHNA, 12.4% OF THE RESPONDENTS WERE CURRENT SMOKERS. THIS IS DOWN FROM 20.9% IN THE 2014 SURVEY. HOWEVER, BASED ON THE INCIDENCE OF LUNG CANCER AND OTHER CHRONIC DISEASES ATTRIBUTED TO TOBACCO USE WITHIN THE COMMUNITY, THIS DECREASE MAY NOT FULLY REFLECT PARTICIPANT BEHAVIORS. AMONG THE KEY INFORMANTS, 42% CONSIDERED TOBACCO USE A MAJOR PROBLEM. ALTHOUGH 70.6% OF THE RESPONDENTS SAID THEY HAD RECEIVED ADVICE TO QUIT SMOKING, CONTINUING TO PROVIDE THAT ADVICE AND ASSISTANCE TO TOBACCO USERS IS VERY IMPORTANT. AT ST. JAMES HEALTHCARE, EVERY PATIENT WHO USES TOBACCO IS PROVIDED WITH TOBACCO CESSATION EDUCATION BY THE RESPIRATORY SERVICES STAFF AND IS GIVEN A REFERRAL TO THE MONTANA TOBACCO QUIT LINE. OVER THE PAST TWO YEARS, THE COMMUNITY OF BUTTE HAS HAD MORE REFERRALS TO THE STATE QUITLINE THAN MOST OF THE COMMUNITIES OF SIMILAR SIZE IN MONTANA. IN ADDITION TO OUR TOBACCO-FREE CAMPUS, ST. JAMES HEALTHCARE CONTINUES TO WORK CLOSELY WITH THE HEALTH DEPARTMENT AND THEIR TOBACCO CESSATION PROGRAM. IN REGARD TO CANCER OVERALL, THE SURVEY SHOWED THAT 7.4% OF RESPONDENTS HAD BEEN DIAGNOSED WITH SOME FORM OF CANCER OTHER THAN SKIN CANCER. IN THE KEY INFORMANT GROUP, 31.6% SAW CANCER AS A MAJOR PROBLEM AND 39.3% RATED IT AS A MODERATE PROBLEM. ST. JAMES HEALTHCARE IS FOCUSED ON INCREASING EARLY DETECTION OF CANCERS THROUGH INCREASED SCREENINGS. IN THE ST. JAMES HEALTHCARE CANCER DEPARTMENT, LOW DOSE LUNG SCREENINGS ARE NOW BEING OFFERED IN AN ATTEMPT TO IDENTIFY LUNG CANCER IN A VERY EARLY AND TREATABLE STAGE. IN 2019, 98 LOW-DOSE LUNG CANCER SCREENINGS WERE PROVIDED. ST. JAMES PROVIDES EDUCATION THROUGH DIRECT MAIL TO WOMEN WHO HAVE NOT HAD A MAMMOGRAM WITHIN THE LAST TWO YEARS AND WOMEN OVER 40 WHO HAVE NEVER HAD A MAMMOGRAM. ACCORDING TO THE CHNA, ONLY 62.6% OF THE WOMEN HAD A MAMMOGRAM IN THE PAST TWO YEARS, SIGNIFICANTLY BELOW THE 80.3% NATIONWIDE. IN 2019, 2,137 MAMMOGRAMS WERE PROVIDED. ACCESS TO HEALTHCARE SERVICES: 37.1% OF BUTTE SILVER BOW COUNTY ADULTS REPORTED SOME TYPE OF DIFFICULTY OR DELAY IN OBTAINING HEALTHCARE SERVICES (CHNA 2017). BUTTE SILVER BOW COUNTY IS DESIGNATED AS A HEALTHCARE PROFESSIONAL SHORTAGE AREA. ST. JAMES HEALTHCARE IS ADDRESSING HEALTHCARE SERVICE ACCESS BARRIERS BY EXPANDING THE HOURS OF OPERATION FOR ITS PRIMARY CARE CLINIC, OFFERING WALK-IN SERVICES FOR BOTH ADULTS AND CHILDREN, AND CONTINUING TO RECRUIT PRIMARY CARE AND SPECIALTY PHYSICIANS TO THE COMMUNITY. IN 2019, 30 SPECIALISTS (INCLUDING MID-LEVEL PROVIDERS) WERE RECRUITED TO ST. JAMES HEALTHCARE, INCLUDING THE SPECIALTIES OF RADIATION ONCOLOGY, PEDIATRICS, UROLOGY, OB/GYN, FAMILY MEDICINE, PRIMARY CARE, AND HOSPITALISTS. THE FOLLOWING OUTREACH CLINICS WERE AVAILABLE IN 2019: A PRIMARY CARE CLINIC IN BOULDER, UROLOGY CLINIC IN DILLON AND ONCOLOGY CLINICS IN DILLON AND DEER LODGE. POTENTIALLY DISABLING CONDITIONS: AMONG SURVEYED PRIMARY SERVICE AREA ADULTS AGE 45 AND OLDER (2017 CHNA), 36.0% FELL AT LEAST ONCE IN THE PAST YEAR, INCLUDING 9.1% WHO FELL THREE OR MORE TIMES. ST. JAMES HEALTHCARE TAKES ONGOING STEPS TO DETERMINE WHERE THERE IS THE MOST NEED TO PROVIDE THE GREATEST GOOD. ST. JAMES REGULARLY PARTICIPATES IN AN ANNUAL REVIEW OF NEEDS TO IDENTIFY CHANGING NEEDS OF THE COMMUNITY IN PARTNERSHIP WITH THE BUTTE HEALTH DEPARTMENT, ACTION, INC., AND A NUMBER OF OTHER AGENCIES. PART VI, LINE 3: THE ORGANIZATION NOTIFIES PATIENTS ABOUT THE FINANCIAL ASSISTANCE POLICY UPON ADMISSION AND PRIOR TO DISCHARGE. NOTICES ABOUT THE FINANCIAL ASSISTANCE POLICY ARE DISPLAYED THROUGHOUT THE HOSPITAL. IN ADDITION, PATIENTS RECEIVE INFORMATION ABOUT THE FINANCIAL ASSISTANCE POLICY WITH THEIR PATIENT BILLS. THE FINANCIAL ASSISTANCE POLICY AND APPLICATION ARE POSTED ON THE HOSPITAL'S WEBSITE. THE POLICY AND APPLICATION ARE ALSO AVAILABLE UPON REQUEST. PART VI, LINE 4: GEOGRAPHY: SILVER BOW COUNTY, THE 2017 CHNA-DEFINED COMMUNITY, IS LOCATED IN SOUTHWEST MONTANA AND SPANS 716 SQUARE MILES.POPULATION: THE 2017 ESTIMATED POPULATION OF SILVER BOW COUNTY IS 34,680 REPRESENTING A 1% CHANGE FROM 2010.GENDER: IN 2017, 48.7% OF THE POPULATION WERE MALES AND 51.3% FEMALES.AGE: IN 2017, 33.5% OF THE POPULATION WERE AGES 18 TO 39; 44.2% WERE AGES 40 TO 64; AND 22.3% WERE 65 AND OVER.RACIAL AND ETHNIC DIVERSITY: IN 2017, 93.9% OF THE POPULATION WERE NON-HISPANIC WHITE; 0.7% WERE HISPANIC; AND THE REMAINDER IS MADE UP OF ASIAN, AFRICAN-AMERICAN AND NATIVE AMERICAN.ECONOMICS: THE MEDIAN ANNUAL HOUSEHOLD INCOME IS $37,503, AND THE PERCENTAGE OF INDIVIDUALS LIVING IN POVERTY IN SILVER BOW COUNTY IS 40.5%.HEALTH BEHAVIORS: ADULT OBESITY IS 34.4% (31.6% IN 2014) AND CHILD OBESITY IS 17.7% (AGES 5 TO 17) (15.7% IN 2014). 12.4% ARE SMOKERS (20.9% IN 2014) AND 20.5% (22.9% IN 2014) ARE IN FAIR/POOR PHYSICAL HEALTH.ACCESS TO CARE: 37.1% HAD DIFFICULTY ACCESSING PRIMARY CARE PHYSICIANS (39.2% IN 2014).

JAMES HEALTHCARE SERVES ITS COMMUNITY BY PROVIDING COMPREHENSIVE MEDICAL SERVICES INCLUDING CARDIOLOGY, ONCOLOGY, ORTHOPEDIC, OB/GYN FOR WOMEN, UROLOGY, EMERGENCY AND TRAUMA, NEONATAL INTENSIVE CARE, NEUROLOGY, GENERAL SURGICAL, PRIMARY CARE, PEDIATRIC CARE, FAMILY AND INTERNAL MEDICINE.COMMUNITY ACTIVITIES INCLUDE PUBLIC PRESENTATIONS ON A VARIETY OF HEALTH TOPICS; STEPPING-ON CLASSES (BALANCE) FOR SENIOR CITIZENS, SUPPORT GROUPS AND CLASSES FOR DIABETES PREVENTION AND LAMAZE CLASSES FOR EXPECTING PARENTS.
