Bon Secours St. Francis Health System (BSSFHS) primarily serves Greenville County, which is comprised of greater than 514,000 residents and has experienced a growth rate double that of the United States and significantly higher than that of South Carolina. St. Francis Hospital provides medical care to patients without regard to their ability to pay. St. Francis HOspital provides support for the community services. Greenville has a similar age distribution compared to South Carolina, with slightly more children (age <18) and fewer older adults (age >65). Greenville has a higher median income and per capita income than South Carolina, but less than the nation. 13.5% (2018) of residents live in poverty in Greenville County. Schedule D, Part X, Line 2 FIN 48 (ASC 740) footnote Schedule D, Part X, Line 2 requires that the organization provide the text of the footnote to the organization's financial statements that reports the organization's liability for uncertain tax positions under ASC 740. ASC 740 addresses the accounting for uncertainty in income taxes recognized in an entity's financial statements and prescribes a threshold of more-likely-than-not for recognition and derecognition of tax positions taken or expected to be taken in a tax return. The adoption of ASC 740 by Bon Secours Mercy Health, Inc. on September 1, 2007 did not have a material impact on BSMH's consolidated financial statements. As the organization does not conduct a separate audit of its financial statements, below is the related statement from the Bon Secours Mercy Health, Inc. consolidated audited financial statements: The Company and most of its subsidiaries (including certain joint venture entities) are exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code of 1986, as amended. The Company accounts for uncertain tax positions in accordance with ASC Topic 740, Income Taxes. Their related income is exempt from federal income tax under Section 501(A). The Company accounts for uncertainty in income tax positions by applying a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. The Company has determined that no significant unrecognized tax benefits or liabilities exist as of December 31, 2018. Accounting for uncertainty in income taxes, ASC Topic 740-10 prescribes a comprehensive model for how an organization should measure, recognize, present and disclose in its financial statements uncertain tax positions that an organization has taken or expects to take on a tax return. The Company is subject to routine audits by taxing jurisdictions; with currently the IRS audit of Mercy for 2016 tax period in progress. The Company believes it is no longer subject to income tax examinations for years prior to 2013. As of December 31, 2018, the Company has no uncertain tax positions. The Company's taxable subsidiaries had $169,666 of gross net operating loss carryforwards as of December 31, 2018, which expire in varying periods through 2037 and are available to offset future taxable income. The Company accounts for income taxes under the asset and liability method. Under this method, deferred tax assets and liabilities are recognized for the estimated future tax consequences attributable to differences between the financial statement carrying amounts of existing assets and liabilities and their respective tax bases. Deferred tax assets and liabilities are measured using enacted tax rates expected to be in effect during the year in which those temporary differences are expected to be recovered or settled. The effect on deferred tax assets and liabilities of a change in tax rates is recognized in income in the period that includes the enactment date. Interest and penalties related to income taxes are accounted for as income tax expense. The Company's deferred tax assets are fully reserved at December 31, 2018 as the Company considers it more likely than not that these amounts will not be recognized. On December 22, 2017, the President signed into law H.R. 1, originally known as the Tax Cuts and Jobs Act. The Act significantly revises the U.S. corporate income tax by, lowering the statutory corporate tax rate from 35% to 21% and eliminating certain deductions. The new law also includes several provisions that result in substantial changes to the tax treatment of tax-exempt organizations and their donors. The Company has reviewed these provisions and the potential impact and concluded the enactment of H.R. 1 will not have a material effect on the operations of the organization.

Schedule H, Part V, Section B, Line 5 Facility A, 1 Facility A, 1 - St. Francis Hospital, Inc.. A community-based approach was taken to complete the Community Health Needs Assessment. Data was collected as outlined below from primary and secondary sources during the time period of April 2019 - July 2019. National, state, and county-specific data was collected from a broad set of reliable data sources. Additional attention was placed on assessing Healthy People 2020 Leading Indicators, review of the previous CHNA data, and gathering information from community residents, providers of health and human services, and other stakeholders and representatives of Greenville County. Efforts were made to ensure that the research was conducted in a manner that was representative of the communities within Bon Secours Mercy Health's service area. Collection of statistical (secondary, quantitative) data at national, state, regional and local levels - key data sources included Census Bureau American Fact Finder, Healthy People 2020, Robert Wood Johnson County Health Rankings, Centers for Disease Control, and South Carolina DHEC biostatistics, vital records, county health profiles and hospital discharge data. Collection and analysis of qualitative data was completed through a community survey, a town hall meeting and a stakeholder focus group. The community survey was completed by 1,014 individuals from January - March 2019. A Town Hall meeting was held in April 2019 to allow discussion among leaders of social service agencies, senior health groups, free medical clinics and others in tune with the needs of at-risk populations in Greenville County. Community Focus Group consisted of neighborhood leaders, organizers and advocates. This group represented the interests of urban and suburban residents, mostly from less affluent communities that are home to lower-income families, minority families and the elderly. Lastly, a Stakeholder Focus Group was held for small group discussion with additional health and social service providers. The meetings with stakeholders were facilitated to review the assessment findings and identify priorities and potential actions. The assessment was completed in partnership and with local public health professionals, health and human service agencies, the South Carolina Hospital Association and Bon Secours St. Francis Health System leaders and board.
Schedule H, Part V, Section B, Line 11 Facility A, 1 Facility A, 1 - St. Francis Hospital, Inc.. Nine health issues and conditions were identified by the community as being of greatest concern. Key findings from the survey, focus groups and town hall, as well as secondary data helped determine these results: behavioral health, transportation, community safety, homelessness, education, social environment and housing, adult obesity and nutrition and exercise, jobs and fair wages, and access to health care services for low income people (children, women, seniors). BSSFHS has developed a strategic three-year implementation plan to address the most paramount needs that were determined by the community: behavioral health, housing, and obesity/wellness. Due to limitations in capacity, resources and overall expertise, BSSFHS will work with other agencies and organizations to address remaining needs. A listing of these agencies and organizations has been included in the CHNA report. How significant needs are being addressed Behavioral Health Bon Secours Medical Group and Community Ministries continues to participate in the South Carolina Hospital Association (SCHA) Behavioral Health Coalition being developed within South Carolina. The SCHA's Mission: to establish a comprehensive system at the state and community level that ensures access to, coverage for, effective coordination and awareness of mental health and substance use disorders services and resources for individuals and families living in SC. Purpose Statement: to implement a multi-sector coalition built on a common improvement agenda and focused on a mutually defined set of strategic goals and actions for collectively improving the availability and access to mental health and/or substance use disorders services for all SC residents. Collaborations/Partnerships: * Greenville Crisis Response Team (GCRT) - developed to provide a seamless and effective system of intervention for those with mental illness within our community. * The Bon Secours Behavioral Health team members are actively involved in the Steering Committee and the development of an Upstate Behavioral Health Coalition with United Way of Greenville County as the convener of the Coalition. The Coalition is committed to improving the lives of Greenville County residents who are struggling with behavioral health issues and strive to help each person achieve their fullest potential. * Greenville Shared Solutions was a crisis intervention center to offer treatment and care for the mentally ill. It was designed to keep individuals out of jail and the ER. Unfortunately, in 2018 ongoing funding for Shared Solutions could not be found and the effort ceased to exist. * Through partnership, Bon Secours St. Francis & Anderson University created a hub for the placement of student interns studying to be psychiatric nurse practitioners within Bon Secours St. Francis. * Sixty-seven (67) patients in the Bon Secours St. Francis ED received intervention services from FAVOR. Greenville achieved its Opioid KPIs. * The Homeless Management Information System contract was formally signed in 2018 between Bon Secours St. Francis and United Housing Connections. Community health nurses, social workers & Emergency Department case management staff were trained to collect data to better serve the homeless. * The Bon Secours Medical Group Depression Screening Initiative implemented depression screening education for staff with ongoing process improvements to meet goals. Dr. Carson Felkel, MD was appointed as Lead Physician for the Behavioral Health Program in 2017. Dr. Carson Felkel implemented an educational video message in support of our commitment to utilizing depression screening tools with confidence and competence to enhance the exceptional delivery of patient care. * The Bon Secours Medical Group (BSMG) continues to use the Behavioral Health/Primary Care Integrated model that was implemented in March of 2018. The Integrated Care model includes an Integrated Care Manager within the Primary Care setting. * BSSFHS/BSMG instituted a collaborative data analytics and informatics team focused on collecting, analyzing and disseminating key mental health and substance use disorders metrics. * Dr. Carson Felkel, Bon Secours Lead Physician Behavioral Health Programs presented to the SC state Coalition March 2018 on the Integrated Care Model and the collaborative work being developed in the Greenville community. * Practitioners of BSMG practices were surveyed to assess needs with respect to behavioral health care for their patients. * Launched BSMG depression screening initiative, depression screening education for staff on adult depression clinical guidelines. * Ongoing process improvements via LEAN transformation. Affordable Housing * Bon Secours St. Francis was instrumental in leading the cause for affordable housing along with nonprofits, local government and real estate developers. Gentrification is a significant issue that has pushed the poor out of the city where the cost of living is higher. Bon Secours St. Francis' Director Community Health co-chaired an affordable housing study in 2016 which led to the development of the Greenville Housing Fund a year later. The Director Community Health currently serves on the Housing Fund's board of directors. The Housing Fund was developed to make low interest loans to developers. * In 2017, the City of Greenville initially designated $2M with an additional $3M plus $5M over the next 5 years to be allocated for affordable housing. In 2018, Greenville County committed $5M for housing to be disbursed over the next 5 years. * Philanthropic funds or investments in affordable housing were made via donations to the Greenville Housing Fund to support affordable housing projects. * Advocacy at the local, state & national levels occurred. Housing and social determinants of health Op-eds were written by Bon Secours staff and printed in the local newspapers. Advocacy at County Council and City Council meetings helped to raise awareness of housing needs for those at the lowest income levels. * Coalition Work established an additional advocacy platform with the Homeless Alliance, Homeless Coalition, United Ministries and the Continuum of Care. * An advocacy letter was prepared by the director Healthy Community Initiatives on behalf of the Homeless Alliance supporting Bill S346. The letter was approved by the Alliance. In April 2017 the director Healthy Community Initiatives spoke before a State Senate committee supporting Bill S346. A letter was drafted from Alex Garvey, SVP, Mission in support of Bill 346 which was sent to Greenville's state legislators indicating the Health System's support for inclusionary zoning. * Provided nursing and social work assistance for the homeless at Triune Mercy Center, First Christian Fellowship for the homeless in need of medical care, health insurance, housing and benefits. * The Sterling Land Trust with financial support from Bon Secours St. Francis, Community Works, and the SC Association for Community Economic Development, the City of Greenville and the Graham Foundation completed its first affordable home in 2019 in partnership with the Nehemiah Revitalization Corporation who builds affordable housing in Counties in the Upstate. The home is rented in the Sterling neighborhood. The County of Greenville in 2019 quick deeded a building and a couple parcels of surrounding land to the Sterling Land Trust to build affordable housing. The Land Trust owns several other parcels of land and worked with Clemson University School of Architecture on several projects between 2017-2019 to create a design for the use of land owned by the Sterling Land Trust. The Nehemiah Revitalization Corporation is interested in partnering with the Land Trust for future builds. * An application for funding a part time Community Development Manager for the Sterling Land Trust was approved via a Mission Grant through the Bon Secours corporate office from 2018-2019. This position provided the Sterling Land Trust an opportunity to build its internal operations and capacity to focus on community development projects that will benefit the Sterling community. * Bon Scours St. Francis built 3 Habitat homes in Sterling at an investment of $210,000 between 2017 and 2019. * Mercy Housing SE & Bon Secours joined efforts to bring 46 units of affordable housing to Mauldin. Mercy's SC State Tax Credit application was approved in 2019 and building will begin in 2020. In 2019 Mercy Housing SE made an offer on 5 acres of land in Greenville. This has the potential to bring an additional 60 units of senior citizen housing to Greenville County by 2022. Description continued below.
Schedule H, Part V, Section B, Line 11 Facility A, 2 Facility A, 2 - St. Francis Hospital, Inc. (Continued). Continued Description From Above Obesity * In 2019, Bon Secours joined a partnership with several organizations called Build Health. The lead agency LiveWell is working with Bon Secours St. Francis, PRIS'MA Health, and the County Health Department. The purpose of the grant is to reduce obesity in Hispanic youth who reside in the Berea neighborhood. Obesity rates are extremely high particularly for Hispanic boys. According to our CHNA every 2 out of 3 people residing in Greenville are obese or overweight. This is a 3 year grant. * Bon Secours St. Francis funded Communities in Schools, to address the risk of obesity in youth. The initiative took place at Berea Middle School between March and June 2018. After school activities were provided for 77 children: nutrition, healthy eating, exercise, sports, gardening & dance. Students reported enjoying the program, changing habits and behaviors. * Bon Secours St. Francis helped fund a University of SC Peer Support study: Yellow Jackets Choose to Bee Healthy. Focus: Applying peer support among students, teachers and parents to encourage healthy eating. * Benefits of participating in the study included weight loss, creating a routine for meals, improvement in how students felt, increased energy level and improved sleep. * Bon Secours St. Francis Health conducted an obesity intervention study in partnership with the Greenville Free Medical Clinic. Intervention included: Evidence-based Diabetes Prevention. * Bon Secours St. Francis Health conducted an obesity intervention study in partnership with the Greenville Free Medical Clinic in 2017-2018. Intervention included: Evidence-based Diabetes Prevention Program curriculum, restorative exercise and group counseling sessions, coordinated patient participation with BSMG Behavioral Health Therapist and Community Wellness Outreach RN and Healthy Self Exercise Specialist, and free fresh market produce for participants. Target: BMIs of 50-55; 60 participants recruited; 12 participated. Four finished the program. Outcomes for those finishing the program: 3 out of 4 achieved a greater than 5% weight loss and maintained the loss and A1c was lowered. There clearly were lessons learned from the program. A major learning was to consider a much smaller sample as a way to address the individual needs and to explore the interest and commitments of participants. * Bon Secours as a partner in the Live Well Coalition became the fiscal agent of the Duke Endowment Grant in 2017. The focus was in three areas: 1) Early childhood (technical assistance and implementation of healthy eating & active living). Measures were established and data was interpreted and presented back to early childhood centers who were participating, 2) Wellness policies developed in the work place and 3)the At Worship program with 130 congregations who are majority African-American. * Major in-roads were created in 2018/19 in the Hispanic faith community on health and wellness. Bon Secours St. Francis' Faith Community Coordinator who is bilingual worked closely with LiveWell to discuss ways to address health promotion and healthy eating habits in Hispanic communities. Several Hispanic ministers committed their congregations to be involved in health related activities.

Schedule H, Part II Community Building Activities Bon Secours Mercy Health addresses various community concerns including health improvement, poverty, workforce development, and access to health care. Bon Secours Mercy Health hospitals conduct community health education and support groups, health fairs and screenings for the communities served. Bon Secours Mercy Health hospitals work with state and local leadership to address community needs and provide healthcare services to the poor and underserved. Bon Secours Mercy Health hospitals provide programs to improve the physical surroundings and housing in the communities served. Inadequate housing has a negative impact on the health of residents in the area by leading to violence in the neighborhoods. A robust economy positively impacts residents covered by health insurance and improves the capacity of the community to support health services. Social support services such as HELP (Hospital Eligibility Link Program) are also important to support the ability of residents to gain and retain employment and therefore access to health insurance and improved access to health services.

Schedule H, Part III, Line 8 Community benefit & methodology for determining medicare costs Medicare allowable costs reflected in Part III come directly from the facility's Medicare cost report. The cost report segregates the total facility actual expenses into costs for support departments, clinical departments (routine and ancillary) and non-reimbursable departments. The cost report uses appropriate statistical bases to "step down" support costs to allowable clinical and non-reimbursable departments. The charges for clinical departments are matched to the total cost for these departments for a cost-to-charge ratio. Medicare-specific allowable costs for in- and outpatient ancillary departments are computed by applying the department-specific cost-to-charge ratio to the Medicare program charges by department. For routine departments, a per diem total cost is computed and applied to Medicare program days for the Medicare routine program cost. The organization feels the total shortfall of Medicare reimbursement compared to computed Medicare allowable costs should be treated as community benefit. The hospital improves access to patient care by providing services regardless of a patient's ability to pay or the hospital's ability to receive full cost reimbursement for services. The hospital also relieves the government of a financial burden when it provides care to publicly insured patients where reimbursement is less than cost of providing the service.
Schedule H, Part III, Line 9b Collection practices for patients eligible for financial assistance The hospital has a written policy that describes collection practices applying to patients who qualify for financial assistance. If full assistance is approved, no collection efforts are pursued on that patient's account(s). If partial assistance is approved, the patient is responsible for the adjusted account balance and collection efforts will follow the established practices for all patients where a self-pay balance exists. Collection efforts are not pursued on any pending FAP account. Specific criteria exists for how much financial assistance, partial or total, will be provided to the patient based on the assessed need. Criteria is based on Federal Poverty Guidelines and is reviewed and updated annually. Once a patient has been deemed eligible for the Patient Financial Assistance Program (FAP), the patient is notified by letter within 60 days after receipt of the application and supporting documentation. The patient retains eligibility for a period of eight months from the date of the application. This eligibility is identified by hospital admissions, billing and collection staff by the assignment of a specific financial class with accompanying eligibility dates. At the end of the eight months, the patient is responsible for reapplying for FAP eligibility.

Schedule H, Part VI, Line 3 Patient education of eligibility for assistance The Patient Financial Assistance program is communicated to patients verbally upon scheduling, registration, visible postings of the program are in common areas throughout the hospital, along with brochures and the program is detailed on our Bon Secours website. In addition, patient billing statements and letters inform patients regarding our financial assistance program. Bon Secours proactively screens patients to identify individuals and their families who may qualify for federal, state or local health insurance programs or the Bon Secours Patient Financial Assistance Program ("FAP") and assist the potential eligible patients through the qualification process. Potentially eligible patients that do not qualify for a federal or state health insurance program are referred to the Financial Assistance Coordinator located in Patient Financial Services for assistance in completing the documentation required to establish FAP eligibility. Bon Secours is also dedicated to meeting the needs of non-English speaking patients by having on-site Spanish translators, and other language translation services. In addition, Bon Secours employs a telephone language service which assists in meeting any language needs that arise. The translation services are offered to non-English speaking patients from admission to discharge, including the financial assistance process. Based on market need, our financial assistance policy and application are translated into multiple languages and placed on our Bon Secours website for patients to access.
Schedule H, Part VI, Line 4 Community information Approximately 514,000 residents reside in Greenville County. Greenville County has experienced a growth rate double that of the United States and significantly higher than the growth rate of South Carolina. As of 2017, Greenville County is predominantly White (75.3%), with a large Black/African American population (18.2%). Compared to South Carolina, Greenville has a larger percentage of Hispanics/Latinos. There is a slow, but steady increase in minority populations in Greenville County. Greenville has a similar age distribution compared to South Carolina, with slightly more children (age <18) and fewer older adults (age >65). The unemployment rate is lower in Greenville County than in South Carolina and the U.S. Greenville has a higher median income and per capita income than South Carolina, but less than the nation. At 13.5%, Greenville has a lower percentage of people living below the poverty level than the state (16.6%); however, there are differences by race/ethnic group. Hispanic/Latinos, Black/African Americans, and those who identify as Other have twice the Greenville percentage. Similar to South Carolina and the U.S., 33.2% of Greenville County residents aged 25 or over have a Bachelor's degree or higher.

Schedule H, Part VI, Line 6 Affiliated health care system Bon Secours Mercy Health, Inc., a Maryland nonprofit, nonstock membership corporation (BSMH), and all of the other entities that are controlled directly or indirectly by BSMH are described collectively as the System. The System was organized in June 1983 to fulfill the healthcare mission of the United States Province of the Congregation of the Sisters of Bon Secours of Paris, a congregation of religious women of the Roman Catholic Church founded in France in 1824. Schedule I, Part II SCHEDULE I, PART II Description of the Bon Secours Mercy Health Mission Fund: Bon Secours Mercy Health performs its philanthropic work through its mission department. This initiative, called the Bon Secours Health System Mission Fund ("Mission Fund"), was developed to promote the Catholic Health Ministry and the Bon Secours Mercy Health, Inc. Mission. This purpose is realized through the funding of initiatives that improve the health and well-being of communities, particularly for disenfranchised and marginalized people, served by Bon Secours Mercy Health Local Systems ("Local Systems"), Cosponsors and the Congregation of the Sisters of Bon Secours. The scope of its purpose and use of funds would be to: -promote healthy community coalition initiatives in conjunction with local system efforts, -develop local system and community excellence for a specific health condition and preventive need, and -improve access for uninsured populations and reduce health disparities among populations in the community. The Strategic Quality Plan of the health system calls for focused efforts to Build Healthier Communities. The health system understands "health" to include social and communal dimensions and has adopted the following articulation of a healthy community. The conditions of communities and individuals served by Bon Secours Mercy Health reflect the interaction of significant factors and complex behaviors at the individual, communal, and societal level. It is not likely that interventions by any one organization will result in substantial improvement or benefit to the community. Rather, increased participation by stakeholders and greater cooperation among entities with appropriate skills and resources is necessary for systemic change and improved outcomes. Mission Fund grants place emphasis on increased collaboration among community based members (Healthy Community Initiative), public health officials and other providers of services. The Mission Fund anticipates that most endeavors that seek to bring meaningful improvement require time and commitment. Consequently, local system grant recipients may expect continuity of support (several years) to establish and track outcomes. At the same time, grant applicants need to cultivate and achieve a wide array of financial resources necessary to sustain promising projects and service programs.
Schedule I, Part I, Line 2 Procedures for monitoring use of grant funds. The organization complies with Bon Secours Mercy Health, Inc.'s system-wide financial and accounting policies. Contributions are generally made as reimbursements for funds spent. In such cases, the donee/grantee organization must provide documentation to the filing organization before funds are approved for disbursement. Schedule J, Part I, Line 4b Terms and conditions of the Mercy Health System SERP The Mercy Health System SERP is a non-qualified deferred compensation plan which provides supplemental retirement benefits to persons selected by the Board of Trustees or its delegate. The plan provides for annual credits of a specified percentage of an eligible participants base salary paid in a plan year and interest credits. Plan participants vest in plan credits after completing a three year class vesting schedule or earlier for death or total disability or reaching age 60 while employed, or due to involuntary separation of employment other than for cause. Payments during employment are made for required tax withholding and reduce the participants account balance. Distribution of the vested account balance in a lump sum occurs after termination of employment. Amounts includible as taxable compensation for listed individuals due to SERP participation in the reporting year were as follows: Matthew Caldwell $0; William Kusnierz $15,327.
Schedule J, Part I, Line 4b Terms and conditions of the St. Francis 457F plan Bon Secours St. Francis Health System, Inc. established the Bon Secours St. Francis Health System Nonqualified Deferred Compensation Plan (the "St. Francis 457F plan") that allows for deposits of employee deferrals and employer contributions and is available only to eligible Bon Secours St. Francis physicians. The St. Francis 457F plan is a non-qualified plan and is subject to a minimum deferred vesting date of at least two years after the first day of the calendar year to which the deferred compensation relates, provided the participant is employed, or upon specified events, such as death or disability. The committee establishes and maintains a compensation philosophy; reviews pay practices against local, regional and national healthcare organizations and approves all remunerative decisions for this group of individuals. The committee reviews and receives assurances that all levels of pay within the organization are reasonable based on performance and validates incentives are met. These decisions are documented in the BSMH board of directors' and compensation committee minutes. Compensation Process Other Officers/ Key Employees: For those key employees and highest paid employees that are not reviewed by the BSMH compensation committee, the process included a review and approval by independent persons, comparability data, and contemporaneous substantiation of the deliberation and decision. In the review, the other officers or key employees of the organization were compared to other hospitals' employees in the area that hold the same title. During the review and approval of the compensation, documentation of the decision was recorded by human resources.
