| General explanation attachment | Part III Line 4 (Statement of Program Accomplishments):The mission of Green Valley Assistance Services Inc., dba Valley Assistance Services (VAS) is: Combining social services and community health, Valley Assistance Services guides seniors, individuals and families to live healthy, resilient and vibrant lives, staying safe and independent. For over 50 years, VAS has served southern Pima County and northern Santa Cruz county, including the communities of Sahuarita, Green Valley, Amado, Arivaca, Tubac, Carmen, and Tumacacori, and the surrounding rural areas. Our staff of Registered Nurses, Benefits Advocates, and support staff, lead cutting-edge programs to address lifes challenges by embracing prevention and education; empowering clients with resources and advocacy; and compassionately enriching the lives of families, seniors, and individuals by seeking to work through their difficulties holistically. Our core programs that support safety and independence include: 1) Going Home With Care, an RN-led post-discharge program to guide the transition from hospital to home; 2) SHiM (Safety and Health in Motion), an evidence-based fall-prevention program. Working in partnership with the Green Valley Fire Volunteer Corps, RNs identify medical and environmental fall risks in and around the home by providing an in-home evaluation of fall-risks, medication risks, lifestyle risks, and risks caused by chronic illness; 3) A Touch That Cares, RN Medical Advocacy and Comprehensive Care Management for seniors facing chronic illness, including in-home health education, coordination with medical practitioners, and medical support; 4) Neighbors Helping Neighbors, a program to provide transportation services and friendly visits to seniors, in their home, by volunteers; 5) MAP-a-Plan (Monetary Assistance Program), a program that provides monetary assistance for rent, utilities, and nutritional needs, to qualifying individuals and families, to prevent evictions, homelessness, and hunger. All clients in the program receive six hours of financial education to provide clients an avenue to break the cycle of poverty; 6) Workforce Training/Career Building, a program providing access to a computer lab, resume-writing assistance, mock interviews, job searches, and continued advocacy for unemployed and underemployed persons; 7) Benefits Advocacy, assistance applying for SNAP (formerly known as food stamps), TANF (formerly known as welfare), AHCCCS (formerly known as Medicaid), and Medicare-Premium Assistance by trained and certified Arizona Assistors; 8) Neighbors AIM (Awareness in Memory Loss), a program for the caregivers, family, and friends of persons suffering from dementia providing support groups; educational presentations; socialization; and Virtual Dementia Tours, an immersion program to show the effects of dementia on common activities; 9) Community-Centered Approach to Care, an RN-led program offered in partnership with Green Valley Fire District, offers wrap-around services to clients based on referrals from the fire department, providing advocacy, resources, and support to assist with financial, health, medical, and life concerns; 10) SASS (Socialization and Support for Seniors), peer-led support and discussion groups, providing socialization for seniors at risk for loneliness and isolation; 11) connectwithU, a program providing friendly phone calls one to two times per week, by volunteers, to clients at risk for social isolation.For the year 2019, 5,900 older persons and families received services through our core programs. Over 7,500 referrals were provided to these clients.Our volunteers have donated (all programs combined) a total of 5,630 hours for an estimated monetary value of $140,750.00 |
