Explanation:

Our Form 990 was mailed on time prior to the due date in May 2020. In October I received notification that our mailed form was not accepted and the Form 990 should have been submitted online thru an e-filer. While preparing to do that I contracted COVID-19 in mid-November and was hospitalized until December 5, 2020. As a result I completely forgot about the need to re-file thru an e-filer. I did file Form 843 on 3/15/2020 with this same cause explained.
