(THE "FOUNDATION") WAS FORMED IN 2014. THE FOUNDATION WAS CREATED FOR THE FOLLOWING THREE PRIMARY PURPOSES: 1) IMPROVING THE LIVES OF PEOPLE IN NEED; 2) PROMOTING EDUCATION AT ALL LEVELS, WITH A PARTICULAR (BUT NOT EXCLUSIVE) FOCUS ON EDUCATION OF UNDERSERVED POPULATIONS AND ON CATHOLIC EDUCATION; AND 3) SUPPORTING COMMUNITY HEALTH PROGRAMS, MEDICAL RESEARCH, INNOVATION IN HEALTHCARE, AND ADVANCES IN CLINICAL EXCELLENCE.DURING 2017, THE BOARD OF DIRECTORS OF THE FOUNDATION DECIDED TO CREATE A SET ASIDE OF $330,000 TO BE PAID OUT IN EQUAL ANNUAL INSTALLMENTS OVER A THREE-YEAR PERIOD FROM 2018 THROUGH 2020.
