Form and Line Reference Explanation
Schedule H, Part V, Section B, Line 5-CRAIG HOSPITAL KEY INFORMANT INTERVIEWS WERE CONDUCTED WITH SUBJECT MATTER EXPERTS AND COMMUNITY PARTNERS. THESE INTERVIEWS PROVIDED QUALITATIVE INFORMATION FOR PRIMARY DATA ANALYSIS. ORGANIZATIONS PARTICIPATING IN THESE STRUCTURED INTERVIEWS ARE: BRAIN INJURY ALLIANCE OF COLORADO (BIAC), GAVIN ATWOOD; CCAD/ADAPT, JOSH WINKLER, CHANDA PLAN, CHANDA HINTON; COLORADO RSVP CLINIC, JEFF BERLINER, MD; EASTERSEALS COLORADO, ROMAN KRAFCZYK; HOME BUILDERS FOUNDATION, BETH FORBES, AND PERSONAL ASSISTANCE SERVICES OF COLORADO (PASCO), HOME HEALTH AND COMMUNITY BASED SERVICES, DAMIAN ROSENBERG. COLORADO HEALTH INSTITUTE (CHI) CONDUCTS THE COLORADO HEALTH ACCESS SURVEY (CHAS) EVERY OTHER YEAR TO SURVEY HEALTH CARE COVERAGE, ACCESS AND UTILIZATION IN COLORADO. QUESTIONS FROM THE CHAS WERE USED ON THE CRAIG HOSPITAL SURVEY TO COMPARE ITS POPULATION WITH CHAS' DATA FROM THE 2017 SURVEY. CHI REVIEWED THE DRAFT SURVEY OF CRAIG PATIENTS AND PROVIDED FEEDBACK TO IMPROVE THE SURVEY FORMAT. CHI PROVIDED COMPARATIVE DATA FROM THE CHAS FOR THE GENERAL POPULATION OF COLORADO AND A SUBSET IDENTIFIED AS DISABLED. REBECCA SILVERNALE, DIRECTOR OF OPERATIONS WAS THE CHI STAFF MEMBER WHO PROVIDED THE CONSULTATION. COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT (CDPHE) COLLECTS HEALTH STATISTICS IN COLORADO OBTAINED FROM THE BEHAVIORAL RISK FACTOR SURVEILLANCE SYSTEM QUESTIONNAIRE (BRFSS). THIS QUESTIONNAIRE HAS BEEN USED BY THE STATE TO DEVELOP A PUBLIC HELATH IMPROVEMENT PLAN. DATA FROM THE 2017 BRFSS SURVEY WAS PROVIDED ON SPECIFIC QUESTIONS FOR THE GENERAL AND DISABLED POPULATION AND COMPARED WITH CRAIG SURVEY RESPONSES. JACKIE GARDNER, POPULATION HEALTH DATA ANALYSIS WITH CDPHE IS THE STAFF MEMBER WHO PROVIDED THE DATA ON BEHALF OF CDPHE. A TEAM OF THREE INDEPENDENT PUBLIC HEALTH PROFESSIONALS PROVIDED CONSULTATION ON PRIMARY DATA COLLECTION AND REVIEWED AND MADE RECOMMENDATIONS ON THE FINAL CHNA REPORT. THESE THREE INDIVIDUALS HAVE OVER 50 YEARS OF COMBINED EXPERIENCE IN PUBLIC/POPULATION HEALTH, HEALTH CARE, DATA COLLECTION AND ANALYSIS, AND COMMUNITY HEALTH. THE TEAM MEMBERS ARE CAILA AUBE, MA; ELIZA LANMAN, MS; AMY PULVER, MBA, MA.
Schedule H, Part V, Section B, Line 11-CRAIG HOSPITAL CRAIG HOSPITAL COMPLETED OUR MOST RECENT CHNA SURVEY IN SEPTEMBER 2019. THE CHNA IDENTIFIED TWO HIGH PRIORITY HEALTH NEEDS: ACCESS TO CARE AND SOCIAL/EMOTIONAL WELLNESS AND TWO MEDIUM PRIORITY NEEDS: HEALTHY EATING/ACTIVE LIVING (HEAL) AND INDEPENDENCE. WE ARE WORKING TO ADDRESS THE TWO HIGH PRIORITY NEEDS IDENTIFIED AND DO NOT PLAN TO WORK ON THE TWO MEDIUM PRIORITY NEEDS AT THIS TIME. ACCESS TO CARE - PEOPLE WITH DISABILITIES HAVE HIGHER RATES OF MANY CHRONIC HEALTH CONDITIONS, INCLUDING ARTHRITIS, ASTHMA, CARDIOVASCULAR, DIABETES, HIGH BLOOD PRESSURE, AND HIGH CHOLESTEROL WHEN COMPARED TO THOSE WITHOUT A DISABILITY. THOSE LIVING WITH PHYSICAL DISABILITIES OFTEN FACE ENORMOUS DIFFICULTY FINDING MEDICAL PRACTICES WITH WHEELCHAIR-ACCESSIBLE EQUIPMENT OR CLINICIANS WHO ARE COGNIZANT OF THEIR UNIQUE HEALTH CARE NEEDS AND RISKS WHICH CAN LEAD TO LIFE-THREATENING INFECTIONS. FURTHER, INDIVIDUALS WITH DISABILITIES OFTEN ENCOUNTER PROFESSIONALS UNPREPARED TO IDENTIFY AND TREAT THEIR PRIMARY AND SECONDARY CONDITIONS AND ANY OTHER HEALTH AND WELLNESS CONCERNS AS HEALTH CARE PROVIDERS OFTEN FOCUS ON THEIR DISABLING CONDITION RATHER THAN ON OTHER HEALTH ISSUES THAT MIGHT BE OF CONCERN TO THE INDIVIDUAL. CRAIG HOSPITAL'S AIM IS TO REDUCE BARRIERS TO HEALTHCARE, PROMOTE WELLNESS, AND PREVENT INJURY AND DISEASE. THIS WILL BE DONE BY IMPLEMENTING THE FOLLOWING STRATEGIES: INCREASE KNOWLEDGE OF GAPS IN PREVENTATIVE CARE FOR THE BI/SCI COMMUNITY AND IDENTIFY AND COLLABORATE WITH COMMUNITY PARTNERS AND PROGRAMS ALREADY WORKING TO ADDRESS THE IDENTIFIED GAPS; SERVE AS A CONSULTANT TO CLINICS IN OUR COMMUNITY TO MAKE THEIR ENVIRONMENT, FACILITIES, AND SERVICES MORE ACCESSIBLE AND INCLUSIVE; EDUCATE HEALTHCARE PROVIDERS ON HOW TO BEST TREAT THOSE LIVING WITH BI/SCI. SOCIAL/EMOTIONAL WELLNESS - NATIONAL DATA (NATIONAL HEALTH INTERVIEW SURVEY - NHIS) SHOW THAT 14.4 PERCENT OF ADULTS 18 YEARS OF AGE OR OLDER WITH DISABILITIES EXPERIENCED SERIOUS PSHYCHOLOGICAL DISTRESS; THE HEALTHY PEOPLE 2020 TARGET IS A 10 PERCENT IMPROVEMENT TO 13 PERCENT. SOCIAL RELATIONSHIPS PLAY AN IMPORTANT ROLE IN THE MENTAL HEALTH AND WELLBEING OF PEOPLE LIVING WITH BI/SCI. THE COMMUNITY SERVED BY CRAIG HOSPITAL IS PARTICULARLY IMPACTED BY INCREASED SOCIAL ISOLATION AFTER THEIR INJURY. THE GOAL IS TO REDUCE ISOLATION, INCREASE SOCIAL-EMOTIONAL SUPPORT FOR OUR COMMUNITY, AND SUPPORT INCREASED QUALITY OF LIFE OPPORTUNITIES. THE PLAN IS TO ACHIEVE THIS GOAL BY CREATING A PIPELINE FOR INDIVIDUALS LIVING WITH BI/SCI TO SERVE IN POSITIONS OF INFLUENCE THAT CAN IMPACT ACCESSIBILITY OF PHYSICAL, CULTURAL, AND INSTITUTIONAL ENVIRONMENTS; DEVELOP AND SUPPORT INTERNAL AND EXTERNAL PROGRAMS THAT SUPPORT ENGAGEMENT IN MEANINGFUL ACTIVITIES WITH THE BI/SCI COMMUNITY. THIS FISCAL YEAR WAS AN EXTREMELY CHALLENGING YEAR DUE TO THE COVID PANDEMIC, HOWEVER CRAIG HOSPITAL CONTINUED TO MAKE STRIDES ON MEETING OUR GOALS FOR IMPROVING ACCESS TO CARE AND ADDRESSING SOCIAL/EMOTIONAL WELLNESS IN OUR COMMUNITY. FOR ACCESS TO CARE, CRAIG PROVIDED CONSULTING SERVICES TO A MAMMOGRAM CLINIC TO MAKE THEIR CLINIC MORE ACCESSIBLE TO PATIENTS IN WHEELCHAIRS. THE HOSPITAL ALSO DEVELOPED AND DISTRIBUTED A "TIP SHEET" TO PRIMARY CARE PROVIDERS IN THE DENVER METRO AREA REGARDING HOW TO PROVIDE BRAIN INJURY PATIENTS WITH THE BEST CARE. THE HOSPITAL'S NURSE ADVICE LINE FIELDED CALLS FROM 52 HEALTH CARE PROVIDERS ACROSS THE COUNTRY AND 17 FROM THE STATE OF COLORADO. FOR SOCIAL/EMOTIONAL WELLNESS, CRAIG IS WORKING WITH A LOCAL COMMUNITY COLLEGE PROGRAM FOCUSED ON STUDENTS WITH BRAIN INJURIES. THE PROGRAM WILL HAVE ITS FIRST STUDENTS IN THE FALL OF 2021. CRAIG HOSPITAL DOES NOT PLAN TO ADDRESS THE TWO MEDIUM PRIORITY HEALTH NEEDS: HEALTHY EATING/ACTIVE LIVING AND INDEPENDENCE. FOCUSING RESOURCES ON THE HIGH PRIORITY HEALTH NEEDS WILL ALLOW THE CRAIG CHNA COMMITTEE TO ENGAGE THE ENTIRE CRAIG STAFF IN THE IMPLEMENTATION AS WELL AS PERMIT THE TEAM TO BUILD STRONG AND TARGETED RELATIONSHIPS WITH OTHER COMMUNITY PARTNERS TO ACHIEVE SIGNIFICANT IMPACT. IN ADDITION, BECAUSE THE ISSUES IDENTIFIED IN THE CHNA PROCESS ARE INTERRELATED, SOME OF THE ACTIVITIES PLANNED TO BE UNDERTAKEN THAT ADDRESS THE HIGH PRIORITY NEEDS MAY ALSO ADDRESS SOME ASPECTS OF THE MEDIUM PRIORITY HEALTH NEEDS. OTHER ORGANIZATIONS IN THE COMMUNITY INCLUDE HEALTHY EATING/ACTIVE LIVING IN THEIR CHNAS AND COMMUNITY HEALTH IMPROVEMENT PLANS AND PRESENT OPPORTUNITIES FOR CRAIG STAFF TO COLLABORATE WITH THESE ORGANIZATIONS TO PROVIDE CRAIG'S UNIQUE PERSPECTIVE WITHOUT TAKING ON LEADERSHIP OF ADDITIONAL HEALTH NEEDS.

Form and Line Reference Explanation
Schedule H, Part I, Line 3c IN ADDITION TO THE FPG, CRAIG HOSPITAL EVALUATES PROVIDING DISCOUNTED CARE BASED ON THE PATIENT'S ASSET LEVEL, MEDICAL INDIGENCY, THE PATIENTS INSURANCE STATUS AS WELL AS THE OTHER FACTORS DESCRIBED IN SCHEDULE H, PART V1, LINE 3 DETAILING THE ADMISSIONS DEPARTMENTS EVALUATION OF EACH NEW PATIENT.
Schedule H, Part I, Line 7 COSTS WERE DETERMINED BY USING EITHER ACTUAL COSTS OR CALCULATED COSTS BASED OFF OF A COST-TO-CHARGE RATIO.
Schedule H, Part I, Line 7f DUE TO THE COVID-19 PANDEMIC, CRAIG HOSPITAL LIMITED THE NUMBER OF STUDENTS/SCHOLARS/VISITORS ALLOWED WITHIN THE HOSPITAL TO HELP MITIGATE THE CHANCE OF SPREADING THE VIRUS AS WELL AS FOLLOW THE RESTRICTIONS IMPOSED BY THE CDPHE AND THE STATE OF COLORADO.
Schedule H, Part III, Section A, Line 2 THE HOSPITAL CONTINUED TO FOLLOW THE PRESENTATION AND DISCLOSURE OF PATIENT SERVICE REVENUE, PROVISION FOR BAD DEBTS AND ALLOWANCE FOR DOUBTFUL ACCOUNTS AS PRONOUNCED IN THE ACCOUNTING STANDARDS UPDATES 201107 AND ACCOUNTING STANDARDS CODIFICATION TOPIC 954. THIS REQUIRES ORGANIZATIONS THAT DO NOT HAVE ACCESS TO THE COLLECTABILITY OF A RECEIVABLE PRIOR TO RECOGNIZING REVNUE TO PRESENT THEIR PROVISION FOR BAD DEBT RELATED TO PATIENT SERVICE REVENUE AS A DEDUCTION FROM REVENUE, NET OF ANY CONTRACTUAL ALLOWANCES AND DISCOUNTS ON THEIR STATEMENT OF OPERATIONS. THE ADOPTION ON THIS DID NOT IMPACT THE FINANCIAL STATEMENTS AS THE HOSPITAL DOES NOT RECOGNIZE SIGNIFICANT REVENUE WITHOUT ASSESSING A PATIENT'S ABILITY TO PAY.
Schedule H, Part III, Section A, Line 3 THE HOSPITAL DOES NOT CONSIDER ANY OF ITS BAD DEBT EXPENSE TO BE ATTRIBUTABLE TO PATIENT'S ELIGIBILITY UNDER THE ORGANIZATION'S FINANCIAL ASSISTANCE POLICY.
Schedule H, Part III, Section A, Line 4 THERE IS NO FOOTNOTE IN THE AUDITED FINANCIAL STATEMENTS THAT DESCRIBES BAD DEBT EXPENSE. PLEASE REFER TO PAGE 18 OF THE AUDITED FINANCIAL STATEMENTS THAT ADDRESSES PATIENTS AND UNINSURED PAYERS.
Schedule H, Part III, Section B, Line 8 THE SOURCE OF THE DATA IS THE MOST RECENT FILED MEDICARE COST REPORT USING THE RATIO OF COST TO CHARGES METHOD. THE PAYMENT MECHANISM RECEIVED FOR CARE PROVIDED TO MEDICARE PATIENTS IS NOT NEGOTIATED BUT SET BY FEDERAL REGULATIONS. THE PAYMENT CURRENTLY RECEIVED FOR TREATMENT IS NOT SUFFICIENT TO COVER COSTS AND ALTHOUGH WE DON'T RECOGNIZE THIS SHORTFALL AS CHARITY, WE BELIEVE IT SHOULD BE INCLUDED IN THE CALCULATION OF COMMUNITY BENEFIT. ALL PATIENTS RECEIVE THE APPROPRIATE LEVEL OF CARE AND SERVICES REGARDLESS OF PAYMENT SOURCE.
Schedule H, Part III, Section C, Line 9a THERE IS NOT A SPECIFIC WRITTEN DEBT COLLECTION POLICY AS THOSE PROCEDURES ARE ADDRESSED IN THE FINANCIAL ASSISTANCE POLICY.
Schedule H, Part IV CRAIG HOSPITAL IS NO PART OF ANY AFFILIATED HEALTH CARE SYSTEM.
Schedule H, Part VI, Line 2 IN ADDITION TO CONDUCTING ITS CHNA EVERY 3 YEARS, THE HOSPITAL ASSESSES COMMUNITY NEEDS THROUGH A VARIETY OF DIFFERENT PROGRAMS. AS A CONDITION OF FEDERAL FUNDING, CRAIG HOSPITAL'S RESEARCH DEPARTMENT CONDUCTS ONGOING LONGITUDINAL STUDIES ON THE LONG-TERM EFFECTS AND NEEDS OF PERSONS WITH SPINAL CORD INJURIES AND/OR BRAIN INJURIES. CRAIG HOSPITAL ALSO OPERATES A TOLL-FREE NURSE ADVICE LINE AND COLLECTS DATA ON WHY FORMER PATIENTS ARE CALLING FOR ASSISTANCE. CRAIG HOSPITAL COLLABORATES WITH THE BRAIN INJURY ALLIANCE OF COLORADO, OPERATION TBI FREEDOM, THE HANGOUT, THE CHRISTOPHER & DIANE REEVES PEER NETWORK PROGRAM AND OTHER COMMUNITY ORGANIZATIONS TO ASSESS AND MEET THE LONG-TERM NEEDS OF PERSONS WITH DISABILITIES. CRAIG ALSO LAUNCHED THE PEER MENTOR SOFT LANDING PROGRAM - A THREE YEAR PILOT PROGRAM AIMED AT IMPROVING THE SUCCESS OF PATIENTS AS THEY ARE DISCHARGED HOME AND GAINING NEW INSIGHT ON THE CHALLENGES THESE PATIENTS FACE.
Schedule H, Part VI, Line 3 THE ADMISSIONS DEPARTMENT IDENTIFIES PATIENTS WHO HAVE LIMITED BENEFITS AND COMPARE THE BENEFITS TO THE ESTIMATED LENGTH OF STAY. WHEN THERE IS A SHORTAGE OF DAYS ALLOWED BY THE INSURANCE, THE ADMISSIONS DIRECTOR MEETS WITH THE PATIENT AND/OR FAMILY TO DETERMINE ALTERNATIVE COVERAGE. THIS CONVERSATION ENCOMPASSES THE PATIENT'S ABILITY TO PAY FOR THE CARE, POTENTIAL ELIGIBILITY FOR FEDERAL AND STATE PROGRAMS AND THE HOSPITALS CHARITY CARE POLICY. UPON ADMISSION, THE CASE MANAGER COMPLETES A THOROUGH PSYCHO-SOCIAL ASSESSMENT OF THE PATIENT/FAMILY SPECIFICALLY ANALYZING THEIR FINANCIAL SITUATION. THE CASE MANAGERS CONSIDER HOUSEHOLD INCOME POST-INJURY, FAMILY SIZE, ASSETS, CURRENT AND FUTURE LIVING EXPENSES, AND FINANCIAL OBLIGATIONS INCLUDING FUTURE CARE NEEDS. THE CASE MANAGERS, ALONG WITH THE PATIENT/FAMILY, ASSESS OTHER POTENTIAL RESOURCES/PROGRAMS FOR WHICH THE PATIENT MAY QUALIFY. RESOURCES AND PROGRAMS EVALUATED INCLUDE BUT ARE NOT LIMITED TO MEDICAID, VICTIM'S ASSISTANCE, LIABILITY AND LIQUIDITY, ACCIDENTAL DEATH AND DISMEMBERMENT, AND THE HOPSITAL'S CHARITY CARE PROGRAM. IF IT APPEARS AS THOUGH THE PATIENT WILL QUALIFY FOR STATE OR FEDERAL PROGRAMS, THE CASE MANAGER WILL ASSIST THE PATIENT/FAMILY IN COMPLETING THE APPLICATIONS, OBTAINING THE REQUIRED DOCUMENTATION AND ADVISING THEM ON HOW TO POSITION THEMSELVES TO QUALIFY. IF PATIENTS DO NOT MEET THE CRITERIA FOR OTHER STATE OR FEDERAL PROGRAMS, THE FINANCIAL ASSISTANCE ELIGIBILITY DISCOUNT GUIDELINE IS USED TO DETERMINE IF THE PATIENT'S BILL WILL BE DISCOUNTED. IF APPROPRIATE, THE CASE MANAGER WILL INITIATE AND ASSIST THE PATIENT/FAMILY IN COMPLETING THE FINANCIAL ASSISTANCE APPLICATION FOR THE CHARITY CARE PROGRAM. APPLICATIONS MAY ALSO BE OBTAINED ON THE HOSPITAL'S WEBSITE, COMPLETED OVER THE PHONE OR BY MAIL. IF IT IS NOT CLEAR THAT THE PATIENT/FAMILY WILL HAVE A BALANCE PRIOR TO DISCHARGE (COPAYS, COINSURANCE, ETC) THE PATIENT/FAMILY WILL RECEIVE INFORMATION ON THE CHARITY CARE PROGRAM WHICH IS NOTED ON THE MONTHLY BILLING STATEMENTS AND THE COPY OF THE APPLICATION ALONG WITH THE LETTER FOR PRE-COLLECTION. AT THIS TIME THE PATIENT CAN COMPLETE THE APPLICATION. ALL APPLICATIONS ARE REVIEWED BY THE A/R MANAGER, CFO AND/OR THE CONTROLLER. WHEN IT IS DETERMINED THAT THE CHARITY APPLICATION IS APPROPRIATE, IT IS COMMUNICATED TO THE PATIENT/FAMILY BY THE CASE MANAGER OR THE A/R MANAGER.
