Explanation:

Three reasons for late filing. I was in an auto accident 10/17/2020 and did not return to work until January 2021 and was behind in my work about three months. In addition I only work three days a week for a half day. I confused the 990 with the filing of the 5500 that was due July 2021 for year ended 2020. I filed the 990 06/01/2021 thinking I was on time. In addition I filed by mail and not electronically. Please waive any penalty and interest. Thank you
