Explanation:

The clerk who normally filed tax returns had a disabling stroke during the tax year. The treasurer files a tax return by US Mail late that year and did not send it certified with return receipt because of COVID. Clearly the tax return was never received by the IRS, but we just received a notice with a penalty and were totally unaware that the return had not been received by the IRS.
