| Form 990, Part VI, Section B, Line 11b | The books and records are sent to a licensed CPA to prepare the 990. The CPA prepares the 990 and returns the completed return to the executive director of the organization. Once reviewed by the Executive Director, the return is filed to the IRS. Upon completion a copy of the form 990 is available to the entire board of directors upon request. |
| Form 990, Part VI, Section B, Line 12c | A Board member has the duty to recuse him or herself from participation in any event when there is an actual or apparent conflict of interest. |
| Form 990, Part VI, Section C, Line 19 | Organizational records include ABMDI's Articles of Incorporation, By-laws and IRS Form 1023, Application for Exemption. Organizational records should be retained permanently. IRS regulations require that Form 1023 be available for public inspection upon request. Policy and Procedure Manual and its revisions should also be retained permanently. |
