Part VI, Section B, Line 11b	The 990 is mailed to all Board members to review. It is first approved by the Executive Director then the audit committee reviews and approves the 990. The form is not filed until all board members have reviewed.
Part VI, Section B, Line 12c	Annually board members and key employees are required to review the conflict of interest policy and sign a document affirming that they have read and understand the policy. They must indicate if any conflict of interest exists and fully describe such conflict. They are also required to bring to the Board's attention any conflict of interest that may arise.
