Form 990, Part VI, Section B, line 11b	Form 990 is reviewed and approved by the president/CEO. Each board members receives an electronic copy of the form 990 for review and responds with their acceptance or any questions to the president/CEO.
Form 990, Part VI, Section B, line 12c	The organization requires that all officers, directors, and employees execute a contract in connection with the organization's conflict of interst policy. The contract requires that each conflict be disclosed immediately upon discovery. The contracts are monitored and enforced on a daily basis.
Form 990, Part VI, Section B, line 15	The organization has an executive committee that is responsible for reviewing and approving compensation levels for the organization's president and other employees.
