| Form 990, Part VI, Section B, Line 11b | The Form 990 is distributed to the Board president by the controller who has prepared the form. The controller discusses the Form 990 in detail with the Board president and answers any questions raised. The Board president then distributes the Form 990 to the other four Board members with his comments, if any, for their review. The other Board members then may discuss the Form 990 with the Board President and/or controller. After full review by the Board members and with Board approval, the Board president signs and dates the form 990 and files it with the Internal Revenue Service. |
| Form 990, Part VI, Section B, Line 12c | Annually the directors and officers review the Conflicts of Interest Policy adopted by the organization. At a meeting of the Board each year, both the directors and officers sign a statement which affirms that such person received a copy of the Conflicts of Interest Policy, has read and understands the policy, has agreed to comply with the policy, and understands that the organization in order to maintain its Federal tax exemption, must engage primarily in activities which accomplish its tax-exempt purpose.
