Form 990, Part V, Line 14b	Moon Lake Electric Association, Inc., does not receive any payments for indoor tanning services.
Form 990, Part VI, Section A, Line 6	Moon Lake Electric Association, Inc., was organized in 1938 as a rural electric cooperative by residents of Northeastern Utah and still operates as a member-owned cooperative in Northeastern Utah and Western Colorado.
Form 990, Part VI, Section A, Line 7a	All Directors on the Board are elected by members of their district and serve three-year terms.
Form 990, Part VI, Section A, Line 7b	Members are given a chance to visit with board members at the annual meetings prior to monthly meetings via our web page, email, and phone calls.
Form 990, Part VI, Section B, Line 11b	Moon Lake Electric Association, Inc., staff presented a copy of the Form 990 to each of the governing board members on April 14, 2021, prior to filing the form with the IRS. A motion was made to accept the Form 990 as presented. The 990 form was then sent to the IRS.
