    Applications should be submitted in the form of a completed Cooper Tire & Rubber Company Matching Gift Program Form. The top half of the form is to be completed by the employee and sent with the employee's donation to the receiving organization.     The foundation does impose restrictions on certain situations where gifts will not be matched.
