Form 990, Part VI, Section A, Line 6	Each individual who qualifies under field of membership criteria and that has a credit union share account is a member of the state-chartered credit union. The credit union is comprised of a single class of approximately 7,259 member-owners. Each member has equal rights in ownership, governance and voting rights at the annual meeting.
Form 990, Part VI, Section A, Line 7a	The board of directors is elected by the member-owners of the credit union at the annual meeting. The board is elected for three-year terms on a rotating basis. Candidates are elected by a simple plurality vote.
Form 990, Part VI, Section B, Line 11b	Form 990 and the related schedules are reviewed by the CEO prior to submission. The board of directors receives a copy of Form 990 and the related schedules.
Form 990, Part VI, Section B, Line 12c	The board members are required to disclose any potential conflict of interest. They sign a Conflict of Interest statement annually after the annual meeting.
Form 990, Part VI, Section B, Line 15	The CEO's compensation is reviewed and approved by the board of directors annually. The compensation for the CEO is based on credit union industry surveys. Employees' compensation is reviewed annually and is based on a CUNA salary survey, local phone surveys and union negotiations. The compensation is within the budget guidelines established by the board of directors.
Form 990, Part VI, Section C, Line 19	The credit union's governing documents, financial statements and Conflict of Interest Policy can be reviewed by appointment at the credit union office during posted business hours. The credit union's monthly financial statements are posted in the lobby of the main office and each branch office.
