| SCHEDULE O | Part I Line 6: Total number of volunteers is estimated based on members who participate in lodge functions and are not compensated. Part Ill Line 3: The consolidation of Liberty Lodge No. 134 and Englewood Lodge No. 166 was approved by the MW Grand Lodge of Colorado, AF & AM at its Annual Session on January 24, 2020. Effective that date, the two lodges ceased operating independently and commenced operations as a new lodge designated as New Frontier Lodge No. 134, A.F. & A.M. A copy of the Grand Lodge certification is included with this 990 Return. Part VI Section A, Lines 2 & 7b: Douglas Peterson is the nephew of Bob Applegate. - Decisions of the governing body are subject to approval of the lodge unless otherwise provided by the bylaws or the governing requirements of the Grand Lodge of Colorado. Part VI Section B Line 12 a/b/c: The Conflict of Interest Policy is signed by the members who have financial management responsibilities. The policy is reviewed by these members which include the five principal officers, the lodge finance committee and the other members designated by the Worshipful Master. The policy requires annual disclosure of interest(s) that could give rise to conflicts. Part VI Section B Line 15 and Part VII Section A: No salary or compensation was paid to the secretary, treasurer, or any other lodge member in 2020, given the anticipated cessation of lodge activities and its merger with another lodge upon approval by the Grand Lodge at its annual session on January 24, 2020. A copy of the Grand Lodge approval for the merger, dated January 29, 2020, is included. Part VI Section C Lines 18 and 19: All governing documents, conflict of interest policy, and financial statements as well as the Form 990 are available for public inspection or for copies through the former lodge Secretary, Douglas Peterson. (See Section C item 20 for contact information.) Part VI Line 11 b & 19 and Part XII Line 2a/2b: Although the lodges financial statements are not prepared, reviewed, or audited by an independent accountant, the finance committee and the Worshipful Master reviewed these statements in accordance with the lodge bylaws. The Worshipful Master and the Finance Committee reviewed a complete copy of the 2020 Return before filing with IRS. The documents provided for this review are full 990 return and associated financial and accounting schedules and statements produced by the accounting system. Part Xl Reconciliation of Net Assets: Liberty Lodge was chartered as lodge #150 on Sept 18, 1918. In January 2004, Liberty Lodge #150 and Berkley Lodge #134 merged, becoming Liberty Lodge #134. On January 24, 2020 Liberty Lodge #134 and Englewood Lodge #166 merged becoming New Frontier Lodge #134. The earliest year for which Liberty Lodge has a retained filed Form 990 is 2002. It has copies of filed 990s for each year except for 2005 and 2006. Beginning in 2007, calculation of net assets was made by separately distinguishing realized and unrealized net asset balances. Prior to 2007, realized and unrealized net assets were merged together in the single category of Net Assets. This Gross Net Assets number is the base Net Assets number for subsequent Net Asset calculations and includes an unknown unrealized amount in the beginning Net Asset Balance for 2007. The net amount of this unrealized portion becomes clear in 2020 with the book value of the securities of $295,204.65 and the checking account balance. The amount of the overstatement in Net Assets totaling $334,222 results from the accumulation of unrealized net assets prior to 2007. |
