| General Explanation Supplemental Information for Form 990-PF | Statement 13Form 990-PF, Part XV, Line 2a-dApplication Submission InformationEligibility Considerations and RequirementsGrant requests are considered only from organizations that are established nonprofit, tax-exempt, charitable organizations and public entities. It is the applicants responsibility to ensure that their organization is recognized by the IRS as a tax-exempt organization before submitting an application.The Foundation follows these general practices.* Grants are not made to or for any of the following purposes: Individuals Projects seeking to influence elections, political candidates or legislation. The Foundation does recognize the role faith-based organizations play in addressing Human service needs in our local communities, however we do not give grants for religious purposes. The Foundation is committed to providing an open, diverse and nondenominational environment. We will not support any program which requires exposure, adherence to or conversion to any religious doctrine in order to be a beneficiary of the program. To clarify, a direct service program run by a faith-based organization may be eligible, provided that the programs beneficiaries are not encouraged or required to learn about, adhere to, or convert to that organizations religious doctrine as a condition of receiving services from the program. * The Foundation limits its funding to Southern Oregon, specifically Jackson, Josephine, Klamath and Curry counties.* The Foundation makes grants up to $15,000.00.* Multiyear grants may be awarded. They will be limited to two years with the second year of funding being contingent upon a satisfactory report being received from the grantee at the end of the first years funding period.* The Foundation will consider only one grant request from the same organization in a twelve month period, unless an additional request is invited by the Foundation.* A Final Report for any previous funding must be received by the Foundation before another application from a previous grantee will be considered.Application Procedures and DeadlinesThe Foundation has an online application process. Proposals sent by mail or fax will not be accepted. Late applications will not be considered.There are two grant application deadlines a year: March 1 & September 1Generally, grant applications for the March 1 funding cycle become available December 1 and grant applications for the September 1 funding cycle become available June 1.?If you submit a grant application for the March 1 funding cycle, the beginning date for the use of funds requested may not be before June 1.?If you submit a grant application for the September 1 funding cycle, the beginning date for the use of funds requested may not be before December 1Application ReviewThe Foundation will acknowledge each application received.All applications will go through an initial review. Some will be denied at this stage and some will move forward for further consideration. If an organizations application moves forward, it is likely that a site visit will be arranged.The site visit will be an opportunity for the Foundation to have a deeper conversation and get a more in-depth understanding of your request for a grant. It will require that the Executive Director, a Board member and possibly others from the organization be present at the site visit. Before the site visit, you will be provided with questions that you may be asked by the Foundation. The purpose of the site visit is to have a very candid conversation. The Foundation wants to understand your organization, the leadership and the work you do in order that it can make the most informed and best funding decisions.Final grant decisions will be announced approximately 10-12 weeks after each deadline. Not all applications move through the review process in the same manner or at the same rate. The Foundation cannot expedite a funding decision. Please do not contact the Foundation during the review period with questions regarding the status of your application.All applicants will be informed of their grant request status, denied or awarded. All decisions are final.The Leightman Maxey Foundation endeavors to serve people throughout Southern Oregon. Despite this commitment, the Foundation is unable to support every request received. Denial of a grant should not necessarily be considered a reflection on the quality of a project or the worthiness of an organization.Payments and ReportingGrant agreements are issued electronically. Grant awards are paid upon receipt of the signed grant agreement and Board minutes from the grantee organization acknowledging receipt of the grant.Every organization receiving a grant from the Leightman Maxey Foundation has a responsibility to report on the use of the funds granted. Both an interim and final report are required. The Foundation must receive these reports before considering any further grant applications from the same organization. Reporting requirements are provided with each of the Foundations grant awards. The Foundation values final reports. Failure to report as required by the Foundation could influence future grant decisions.The Leightman Maxey Foundation is continually evolving and refining its processes and priorities.
