Explanation:

This organization has an excellent history of filing on time. The accounting firm that prepares and files the returns for this organization had one of their staff members that was responsible for filing the extensions abruptly leave the firm. As a result, the extension was never filed. When the accounting firm realized that the extension was not filed, they immediately prepared the tax return. To assure this never happens again, they have several staff members working together to check deadlines and to assure all extensions and tax returns are filed timely. We believe the above constitutes reasonable cause and not willful neglect. Therefore we respectfully request that you not access a penalty for late filing.
