    IN MARCH OF 2021 ONE OF THE FOUNDATION MANAGERS WAS APPROACHED BY THE LUBBOCK COALITION FOR HEALTHCARE ACCESS (THE "COALITION") REQUESTING A DONATION. THE FOUNDATION MANAGER REQUESTED INFORMATION ON THE COALITION'S TAX-EXEMPT STATUS, AND WAS TOLD THAT THERE WERE SEPARATE SECTION 501(C)(3) AND SECTION 501(C)(4) ORGANIZATIONS. THE COALITION PROVIDED INSTRUCTIONS TO DESIGNATE ON THE CHECK WHICH OF THE TWO ORGANIZATIONS THE GRANT WAS INTENDED FOR. THE FOUNDATION MANAGER DID SO BY NOTING "C3" IN THE MEMO FIELD OF THE CHECK.
