| PART III, LINE 4A | Faith in Prevention programming expands the role of faith based organizations (FBO) in the delivery of health prevention services, and establishes partnerships and collaborations between FBOs and healthcare systems in Newark. The program targets obesity and related chronic illnesses using the Faithful Families curriculum(University of North Carolina.) GNHCC awarded mini grants to FBOs for programming to increase opportunities for physical activity and availability to healthy foods, and increase the spiritual and physical health of congregants. In 2021, there were more than 300 congregants participating and 41 lay leaders trained. Under the Healthy Women Healthy Family grant, GNHCC's work includes: the on-boarding of the required staff of community health workers and supervisors, beginning to make referrals for case management, engaging over 100 people through community events, identifying local partners to implement the fatherhood empowerment sessions and facilitating male focus groups to assess needs and determine what is needed to develop topics and activities for the empowerment sessions. THE PROMISE NEIGHBORHOOD IS A COLLABORATIVE PARTNERSHIP WORKING TO ENSURE EDUCATIONAL SUCCESS AND A SAFE AND HEALTHY COMMUNITY FOR CHILDREN IN THE SOUTH WARD OF NEWARK. HEALTH IS ONE OF THE INITIATIVE'S DOMAINS AND GNHCC IS THE HEALTH ANCHOR PARTNER. |
