Part VI, Line 6	The Trust's members are comprised of the retirees spouses and dependents covered by the Voluntary Employees' Beneficiary Association VEBA.
Part VI, Line 7a	The Board of Directors of the Copolymer Retirees Trust are elected by members on a ballot election.
Part VI, Line 7b	All of the members of Copolymer Retirees Trust have equal voting rights in the election of all board members.
Part VI, Line 11b	The 990 is prepared in-house and reviewed by the trust administrator and legal counsel using audited financial statements which includes input from the investment advisors and contracted vendors. The form is then submitted to the Board of Directors for their review and ratification.
Part VI, Line 12c	All vendors are interviewed as to their business and personal relationships with other contractors members of the trust board members and the administrator. In the event of a disclosure the full board will evaluate the relationship to determine if the services should continue. A material change in vendor would be reviewed by the Board of Directors after recommendations by the administrator who had previously taken into account conflicts of interest and other business related issues prior to recommendation.
