| FORM 990, PART VI, SECTION A, LINE 5 | A FORMER MEMBER OF THE EXECUTIVE STAFF ALLEGEDLY STOLE APPROXIMATELY $108,000. THEY DID SO BY WRITING COUNTER CHECKS ON OUR ACCOUNTS EITHER TO THEMSELVES OR TO CASH. THEY WOULD THEN CASH THE CHECKS AND TAKE THE CASH. THEY ALSO WOULD TAKE THE CASH RECEIPTS FROM OUR THRIFT STORE STATING THAT THEY WOULD DEPOSIT THEM. THE CASH NEVER MADE IT TO THE BANK AND WAS NEVER GIVEN TO ANYONE ELSE IN ORDER TO DEPOSIT THE MONEY. THE SAME EXECUTIVE STAFF MEMBER WOULD NOT GRANT BOARD MEMBERS ACCESS TO ANY OF THE BANK ACCOUNTS, SALES PLATFORM ACCOUNT, OR TO THE ORGANIZATION'S QUICKBOOKS SO THAT THEY COULD OVERSEE AND REVIEW THEIR WORK. THE BOARD ASKED MULTIPLE TIMES TO NO AVAIL. UPON LETTING THE EXECUTIVE STAFF MEMBER GO, THE FOLLOWING CHANGES WERE MADE BY THE ORGANIZATION TO PREVENT THIS TYPE OF ALLEGED MALFEASANCE TO HAPPEN AGAIN: CHECK WRITING PROCEDURES HAVE BEEN SUBSTANTIALLY TIGHTENED: THE EXECUTIVE DIRECTOR IS NO LONGER ALLOWED TO SIGN ANY CHECKS WRITTEN TO HIM OR TO CASH. ONES WRITTEN TO HIM MUST BE SIGNED BY AN EXECUTIVE COMMITTEE MEMBER. THERE IS A CLEAR DIVISION OF LABOR WITH CHECKS AND BALANCES BETWEEN ROLES IN THE ACCOUNTING ARENA. THE BOOKKEEPER IS TO WRITE ALL CHECKS AND HAS NO SIGNING AUTHORITY. THE AUTHORIZED SIGNERS ARE NOT ALLOWED TO WRITE CHECKS UNDER ANY CIRCUMSTANCES. THE BOOKKEEPER IS TASKED TO ENSURE THAT ALL PAPERWORK/ DOCUMENTATION IS COMPLETE BEFORE ANY CHECK IS TO BE WRITTEN. THE AUTHORIZED SIGNER IS TO DOUBLE CHECK ALL PAPERWORK TO ENSURE THAT EVERYTHING IS IN ORDER. NO CHECK IS ISSUED WITHOUT AN INVOICE OR THE PROPER REQUISITION FORM EXPLAINING DISBURSEMENT AND SUPPORT DOCUMENTATION. CHECKS TO 'CASH AND/OR 'COUNTER' CHECKS ARE NO LONGER ALLOWED IN ANY CIRCUMSTANCE. THE BANK IS ON NOTICE FOR THIS RESTRICTION. ANY CHECK $2,500 OR ABOVE MUST HAVE TWO SIGNATURES ON THE CHECK - ONE FROM THE EXECUTIVE DIRECTOR AND ONE FROM AN EXECUTIVE COMMITTEE MEMBER. ANY CHECK $20,000 AND ABOVE REQUIRES A BOARD RESOLUTION AND VOTE TO DISPENSE FUNDS. ALL BANK ACCOUNTS ARE RECONCILED AT THE END OF EACH MONTH, CHECKED BY THE BOARD TREASURER, AND REVIEWED BY THE BOARD AT THEIR MEETINGS. HANDLING OF MAIL: MAIL IS NOW OPENED BY TWO PEOPLE; ENVELOPES ARE MAINTAINED FOR THE FISCAL YEAR WITH AMOUNTS AND METHOD OF PAYMENT WRITTEN ON ENVELOPES AS A BACKUP VERIFICATION METHOD. HANDLING OF CASH: NOW TWO EMPLOYEES CROSS VERIFY CASH AMOUNTS TO BE DEPOSITED. THEN THOSE AMOUNTS ARE CROSS CHECKED WITH THE ELECTRONIC RECORD OF SALES RECEIPTS FOR THE DAY. COPIES OF DEPOSITS AND LOG SHEETS ARE MAINTAINED IN A BINDER WHICH ARE THEN TRIPLE CHECKED BY EXECUTIVE DIRECTOR. MOVEMENT OF CASH BETWEEN BANK ACCOUNTS: ANY MOVEMENT OF MONEY FROM THE SWEEP ACCOUNT TO THE OPERATING ACCOUNT MUST HAVE A BOARD APPROVED RESOLUTION WITH A SPECIFIC AMOUNT OUTLINED IN THE RESOLUTION. THE RESOLUTION IS GOOD FOR ONLY ONE TRANSFER AND MAY NOT BE A BLANKET AUTHORIZATION TO TRANSFER MONIES.: BUDGET REPORTING: EACH BOARD MEETING HAS A FULL REPORT ON THE FINANCIAL STANDING OF THE ORGANIZATION WITH CASH BALANCES, PROFIT AND LOSS STATEMENTS FOR REVIEW AND MONITORING PURPOSES. BOARD OVERSIGHT: THE EXECUTIVE COMMITTEE HAS FULL REVIEW ACCESS TO ALL BANK ACCOUNTS, SALES DATA FROM THE THRIFT STORE, AND OUR ACCOUNTING SOFTWARE TO FOLLOW, CHECK ON, AND REVIEW AT THEIR DISCRETION. THEY CAN AND DO RUN REPORTS AND DOWNLOAD ANY INFORMATION THEY DEEM NECESSARY OR PRUDENT FOR OVERSIGHT PURPOSES. |
