| Form 990, Part I, Line 1 BRIEF MISSION | lives in the comprehensive, compassionate care we provide to patients and families while aging or seriously ill. SIGNIFICANT MISSION-RELATED ACTIVITY TOOK PLACE IN 2022: In 2022, Agrace served a total of 5,390 individual patients and clients through its Hospice Care, Supportive Care, Age at Home and Adult Day Center programs. SAGECare Platinum Certification Improves Care for LGBT Elders: LGBT elders who use Agrace's services can feel confident knowing Agrace earned SAGECare Platinum Certification in 2022. At least 80 percent of our staff completed SAGE's cultural competence training about the needs of the LGBT community and the health care disparities they face. Agrace pursued this training because we are committed to equity-and to learning how to make each older patient and client feel comfortable while in our care. Giving Agrace a Voice in National Hospice Policy: Agrace president and CEO Lynne Sexten joined the board of directors of the National Hospice and Palliative Care Organization (NHPCO) in January. As one of the largest nonprofit hospices in the country, Agrace needs to have a seat at the table to advocate for the rural, isolated and underserved patients and families who need our care. Lynne will serve on the board for three years, advocating to ensure quality, compassionate hospice care remains available to everyone. Agrace Announces Plans to Become Carbon Neutral by 2025 After years of planning, Agrace launched an initiative to become carbon neutral by 2025. We've planned a series of actions that will help us care for the environment of southern Wisconsin, just as we care for its people. Climate change caused by carbon emissions creates risks to human health. Agrace is choosing to reduce our "carbon footprint" over the next three years through actions such as installing solar panels, purchasing locally generated power from renewable sources, buying energy-efficient equipment, and allowing staff to work remotely when it's practical. Landmark Investment in its Workforce: In November, Agrace announced an initiative to reinvest in those who work hard every day to provide personalized care and support, where and when it's needed, for those living in southern Wisconsin. As one of the area's largest employers, the Madison-based nonprofit health care organization employs a workforce of nearly 900 individuals who care for more than 1,700 Wisconsin residents each day. Most of the agency's personal care, supportive care and hospice care services are provided in people's homes. Agrace's workforce investment includes three main components: * A new $17 per hour minimum pay rate for all employees * A medical coverage premium break for lower wage earners * A year-end gratitude bonus Grand Opening Celebration for Adult Day Center Takes Place: After opening its doors in Summer 2021, Agrace was able to hold a ribbon-cutting event and officially welcome the public to visit the Adult Day Center. The freestanding center offers a balance of activities and companionship for seniors and older adults, weekdays 7:30 a.m. to 5 p.m. from its location in Madison. |

| Form 990, Part VI, Line 12c Conflict of interest policy | Directors, officers, community committee members, and key employees (interested persons) are required to annually review the organization's Standards of Conduct, including the conflict-of-interest policy/statement. Interested persons are required to disclose potential or actual conflicts with the organization in writing via the annual questionnaire distributed electronically to each interested person. When a disclosure arises, the person shall promptly make disclosure of the interest to the governing board or committee. After disclosure and discussion, the interested person shall leave the governing board or committee meeting while the determination of a conflict of interest is further discussed and voted upon by the remaining disinterested governing board or committee members.
