1-26-2023 Dear Dept. Mgr Hussey or to Whom it May Concern: My name is Tom Leverso, and I am one of the principals of Club 1565, NFP, an Illinois 501(c)(7) not-for-profit entity. I am its president. I wish to apologize on behalf of the entity for BOTH (1) submitting the paper/physical form 990-EZ and (2) exceeding your 10-Day deadline in LTR2695C. I humbly request that you not punish the entity for tardiness in e-filing the return and limit the penalty to $60.00, which I have immediately tendered concurrently with the e-filing. I have paid $360.00 due to the e-filing occurring today, January 26, 2023. As for the initial submission, we erroneously trusted the quick advice of a retired tax professional in submitting the form. The entity was timely in completing this physical submission, having sent it on or about March 11, 2022, for Tax Year 2021. As for the delay in immediately e-filing within 10-days, my offices only received LTR2695 and the improperly-filed return on or about Monday, January 9, 2023. I was only into the office for the first time on January 11, 2023, adding two-days further delay. IMMEDIATELY on January 12, 2023, I submitted my credentials to permit the e-filing and through the IRS portal and ID.me for e-filing on behalf of the entity, and I am in the process of retaining a new tax professional to e-file sooner, if possible. I was finally able to locate a suitable e-filing medium for Club 1565, NFP, on 1/24/2023, and reformatted the original Form 990-EZ for e-filing and submitted it today, 1/26/2023. This will NEVER HAPPEN AGAIN. Thank you for your time in consideration of these matters. Please consider that we are just a group that wishes to organize to discuss religion as it relates to charity?
