Form 990, Part VI, Section A, Line 6	Any not-for-profit home, facility or service organization providing healthcare, services, or housing for the aging located in the State of Oklahoma may file an application for membership. The Board of Directors may accept into membership any not-for-profit organization providing housing, health care or services to the aging.Associate membership may be granted to any business connected to service to the elderly who is in accord with the purpose of the organization as stated in Article II. For-profit long-term care providers do not qualify. Associates do not have the right to vote or hold office.
Form 990, Part VI, Section B, Line 11b	Treasurer and Executive Director review the 990. The 990 is made available to the board before it is filed.
Form 990, Part VI, Section B, Line 12c	The organization has a conflict of interest policy. The board is required to complete a policy annually. Any potential conflicts are reviewed by the remaining board members.
