When the initial form was completed, a selection was made in Section J to indicate our tax exempt status as a 501(c)(5). The numeral 5 did not populate when the form was printed to be mailed although the selection of 501(c) was still selected to indicate tax status. It was not caught when the form was proofread because the selection was still showing as being completed and it was not obvious that the numeral had not been included.
